Infant and child feeding index.

Traditional indicators of child feeding practices are widely used and appear to be useful, although the validity and reliability of those related to complementary feeding have not been established. Composite child feeding indices have the potential to address some of the methodological constraints related to the quantitative measurement of child feeding practices. They can address the multi-dimensionality of child feeding practices i.e. the need to consider the type, quality, texture, nutrient density of food, frequency of feeding and diversity of the diet; the age-specificity of child feeding practices; and the fact that feeding practices tend to cluster. The main advantage of creating a composite index is that it allows construction of one variable representing various dimensions of feeding or care practices. This variable, in turn, can be used to illustrate graphically the importance of child feeding or care for child outcomes or to model their determinants. This can be invaluable for advocacy, apart from being an indispensable tool for the purposes of research, monitoring and evaluation. Experience with creation of composite child feeding indices with the studies in Accra and DHS data sets from Latin America and Ethiopia has been encouraging and suggests that this is a promising area for future development and program applications.

Infant and toddler polysomnography.

The acquisition process for infant and toddler polysomnography requires pediatric-specific equipment and specially trained personnel. Pediatric laboratories must keep in mind the importance of including the parent through-out the process. The demand for infant and pediatric polysomnography continues to grow as new research increasingly demonstrates the value of studying physiologic variables collected during the study.

Child care effects in context: quality, stability, and multiplicity in non-maternal child care arrangements during the first 15 months of life.

Main and interactive effects of child care quality, stability, and multiplicity on infants' attachment security, language comprehension, language production, and cognitive development at 15 months were examined using data from the National Institute of Child Health and Human Development (NICHD) Study of Early Child Care. Thirty-nine percent of the infants in this sample experienced arrangement change, and 46% experienced multiple concurrent arrangements during the first 15 months. As in previous studies, concurrent quality, average quality, and quality slope significantly predicted cognitive and language development. There was some evidence that certain forms of unstable child care--including nonfamilial change, familial to nonfamilial change, and within-home to out-of-home change--were associated with poorer language development. Multiple child care arrangements involving family members positively predicted language comprehension; multiple care involving a mix of family and non-relative caregivers negatively predicted language comprehension. Interactions among variables exhibited "effects in context." That is, under conditions of low or moderate quality in the primary care arrangement, the use of fewer multiple arrangements was associated with higher language scores; under conditions of high primary care quality, the use of more multiple arrangements was associated with higher language scores.
