Congenital right diaphragmatic hernias through posterolateral and anterolateral defects associated with extralobar pulmonary sequestration: a case report.

Multiple diaphragmatic hernias in the unilateral diaphragm are extremely rare. The authors report a neonate with diaphragmatic hernias through two defects in the right diaphragm: a posterolateral defect without a hernia sac and an anterolateral defect with one. After excision of the anterolateral hernia sac, each defect was closed. Histology studies showed extralobar pulmonary sequestration in the removed hernia sac. The presence of sequestrated pulmonary tissue indicates the possibility of interference with the closure of the pleuroperitoneal canal and muscularization in the diaphragm, which may result in multiple defects.

Retroperitoneal pulmonary sequestration: imaging findings, histopathologic correlation, and relationship to cystic adenomatoid malformation.

OBJECTIVE: Retroperitoneal bronchopulmonary sequestrations are rare congenital lesions that have been increasingly reported as incidental findings in utero. We present our case material of congenital retroperitoneal sequestration, discuss the reported imaging and histopathologic characteristics of this entity, and provide an approach to subsequent clinical and surgical management. CONCLUSION: Our data suggest that the imaging findings in retroperitoneal sequestration are characteristic and that faulty mesenchymal induction of pulmonary tissue within the retroperitoneum renders internal development into cystic adenomatoid malformation the rule rather than the exception. In the typical case, surgical removal is nonemergent.

Bronchopulmonary Sequestration

Video-assisted thoracoscopic resection of pulmonary sequestration in an infant.

Pulmonary sequestration is a congenital anomaly of lung parenchyma that can be definitively treated only with surgical resection. We report a case of an intralobar sequestration of the right lower pulmonary lobe in an infant successfully treated with video-thoracoscopic surgical removal of the involved lobe at 6 months of age.
