Ovulation induction.

Abstract

Ovulation induction has developed as a valuable noninvasive therapy for the infertile couple. The nurse's role focuses on assisting the couple, as a unit, to understand and cope with the necessary regimen, tests, examinations, and therapies necessary to reach the goal of conception. The physiologic regulation of the menstrual cycle, current pharmacologic therapy, treatment protocols, and the unique role of the infertility nurse are reviewed.

Postvaccinal parkinsonism.

A 5-year-old boy, with a history of fever beginning 15 days after a vaccination for measles, developed a rigid-akinetic syndrome 3 days after the fever began. A spinal tap obtained 1 week after the onset of fever showed pleocytosis with a monocellular pattern. A CT scan of the head and EEG did not disclose any abnormality. An MRI performed 3 months after the event, however, showed clear-cut evidence of bilateral substantia nigra lesions, suggesting secondary gliosis. The response to levodopa was good, but adverse reactions appeared early. The child is now 7 years old. Bromocriptine, deprenyl, and levodopa have produced a remarkable improvement of the parkinsonian features.

Octreotide therapy of growth hormone excess in the McCune-Albright syndrome.

We report a patient with the McCune-Albright syndrome and growth hormone excess. Biochemical evaluation demonstrated characteristic changes typical of acromegaly, and an unusual pattern of delayed somatotropin response to hGHRH40, not previously described in this syndrome. Therapeutic trial of low-dose octreotide successfully reversed his growth hormone excess, whereas bromocriptine failed to reduce growth hormone levels. Previous reports of acromegaly and McCune-Albright syndrome are reviewed, and the unique features of this case discussed.
