Abstract

The development of the International System of Units (Systeme International d'Unites--SE Units), based on seven fundamental quantities--length, mass, time, electric current, thermodynamic temperature, luminous intensity, and amount of substance is described. Units (coherent and noncoherent) for other measurable quantities that are derived from the seven basic quantities are reviewed. The rationale for the use of SE units in medicine, primarily as applied to clinical laboratory data, is discussed, and arguments are presented for the rigid adoption of SI units in medicine and for exceptions. Tables are given for the basic and derived SI units used in medicine and for conversion factors from the quantities and units in current use to those in SI units.

A study of unit cost of routine investigations in Srinagarind Hospital.

We have used the step up allocation accounting method to determine the actual costs of complete blood count, urinalysis, chest radiograph and electrocardiogram. We found that the actual costs were quite different from the price set up by the hospital. The actual cost were much higher than the price for the complete blood count and urinalysis and much lower for the chest radiographs and especially for electrocardiogram. We propose that in performing economic analysis involving all these investigations, the actual cost should be used instead of the price.

An evaluation of the Ramco kit for serum ferritin assay.

The determination of serum ferritin levels may be of diagnostic importance in medicine. To establish whether values obtained using a commercially available kit (Ramco) were adequate for this purpose, a comparison was undertaken using a two-site immunoradiometric assay that had been developed and standardized in our laboratories. Over the range 6 micrograms/l to greater than 2 000 micrograms/l there was a correlation coefficient between the two methods of 0,8284 (P less than 0.001). It is concluded that the Ramco kit is suitable for use in clinical practice.

Blood Chemical Analysis

Use of microbore columns for the separation of substances of biological origin.

A gradient system for use with microbore columns is described and the repeatability of the system determined. A simple sampling device for use with aqueous biological samples is described, and the advantages of the high mass sensitivity of microbore columns for the analysis of such samples are discussed. The sensitivity of the system is demonstrated by the determination of acetophenone in water at a level of one part per billion. The use of the apparatus for the separation of low-molecular-weight, non-ionic compounds contained in blood is also demonstrated. It is shown that excellent separations of such components can be obtained from as little as 400 microliter of serum.
