Decrease in pulsatile flow in the internal carotid artery in fetal hydrocephalus.

Abstract

The effect of ventriculomegaly on pulsatile flow in the internal carotid arteries has been studied by Doppler ultrasound in four hydrocephalic fetuses. The pulsatility index showed progressive elevation proportional to the developing ventriculomegaly and thus may be a valuable index for the study of the mechanism of brain injury and for the determination of optimal timing of intervention.

Cardiac cycle extraction from projection data using static signal suppression.

As an extension of the projection-data-based cardiac cycle extraction that we developed previously (W. S. Kim, K. J. Jung, K. D. Lee, J. B. Ra, and Z. H. Cho, "Proceedings, Seventh Annual Meeting of the Society of Magnetic Resonance in Medicine, 1988," p. 958; W. S. Kim, C. W. Mun, D. J. Kim, and Z. H. Cho, Magn. Reson. Med. 13, 25, 1990), a new technique with which one can extract the cardiac cycle by flow-based projection data measurements is described. Because the method uses flow-dependent projection data, it is possible to obtain the cardiac cycle even from regions such as the head, which has virtually no geometrically varying parts, unlike the cardiac region. The signal-to-noise of this flow (such as blood and CSF) based projection data acquisition is further enhanced by the use of the static sample signal suppression technique developed for angiography (J. H. Kim and Z. H. Cho, Magn. Reson. Med. 14, 554, 1990).

Enhanced surgical exposure for the high extracranial internal carotid artery.

The need for enhanced surgical exposure for the high extracranial (Zone III) internal carotid artery is not uncommon. In certain circumstances, the posterior border and angle of the mandible may interfere with access to the distal internal carotid artery (ICA). The use of modified mandibular osteotomies has provided vascular surgeons in our institution with improved exposure of the ICA in selected cases. The intraoral sagittal split and extraoral vertical ramus osteotomies of the mandible allow manipulation of the posterior border and angle of the mandible with low morbidity and minimal postoperative complications. These procedures can be performed for both dentate and edentulous patients without the need for intermaxillary fixation. This paper introduces these modifications and discusses the benefits over previously described methods of mandibular manipulation.
