[The formaldehyde problem].

Abstract

Regarding formaldehyde (FA), we face the following problems: (1) It is a wide-spread substance being found as free FA, in FA releasers and FA resins. (2) FA contact allergy occurs either as occupational or non-occupational disease. (3) In spite of its frequency there are still problems concerning patch testing of FA. We report on the results of patch testing with FA in various vehicles and concentrations performed on patients with dermatitis and those suffering from a proven FA contact allergy. Our investigations confirm the superiority of water as a vehicle.

Compensatability and contact dermatitis in the canning industry.

If reports from the Workmen's Compensation Commissioner are compared with international figures, dermatitis in South African industry appears to be underestimated. Compensation for occupational dermatitis in South Africa relies on the presence of a positive patch test. A prevalence study at a canning factory in the western Cape showed high rates of dermatoses. No association could be shown between dermatitis, intertrigo or chronic paronychia and patch-test positivity. It is argued that patch-test positivity is purely coincidental to the dermatoses and that patch testing has little role to play where the majority of occupational dermatoses are irritant in nature. Reform of the system of compensation for occupational skin disorders is urgently needed and should be integrated in workplace-based preventive and curative programmes.

Long-standing dermatological manifestations in a patient with chronic heavy metal intoxication.

A patient with chronic metal intoxication is described, presenting during four years after the cessation of her exposure to industrial substances, maculo-papular eruptions with several ulcerated lesions and excoriations on her abdomen and buttocks. She also had pallor of her face, greyish-dark discoloration of the hair, while the fingernails were brittle and sensitive. Scrupulous physical examination excluded further cutaneous involvement. The immunological workup revealed both phenotypic and functional defects in cellular immunity.
