Thyroid carcinoma.

Abstract

Eleven cases of thyroid cancer occurring in patients below 15 years of age are presented. The youngest patient was 4 years old. There were 4 boys and 7 girls in the study group. None of the patients had received head and neck irradiation. All patients had well differentiated cancers; there were 4 follicular carcinomas and 3 of these occurred in patients below 10 years of age. Six patients presented with solitary thyroid nodules. Lymph node involvement had occurred in 63.7% of cases. One patient presented with CNS metastasis. Treatment aimed at near total thyroidectomy was performed in 9 patients. Two patients died of the disease during the follow up period and another 3 patients are alive. Postoperatively radioactive iodine was routinely used for evaluating and treating residual and recurrent disease.

Gastric amphicrine carcinoma in the stomach: an unexpected presentation of MUTYH-associated polyposis.

Amphicrine carcinomas of the stomach, defined by dual exocrine and neuroendocrine differentiation within the same neoplastic cell, are exceedingly rare. MUTYH-associated polyposis (MAP) is an autosomal recessive polyposis syndrome characterized by multiple colorectal adenomas and variable upper gastrointestinal involvement; however, amphicrine carcinomas have not been previously documented in this setting. We report a gastric amphicrine carcinoma arising in the background of extensive fundic gland polyposis in a patient with MAP. Endoscopy revealed a 3.5-cm flat elevated lesion in the gastric fundus amid extensive fundic gland polyposis. Histologically, the tumor consisted of a single population of cells exhibiting combined glandular and neuroendocrine differentiation without zonal or biphasic architecture, and many of these cells demonstrated true amphicrine morphology. Immunohistochemistry confirmed co-expression of cytokeratin and the neuroendocrine markers chromogranin A and synaptophysin in the same cell population. Germline targeted next-generation sequencing identified biallelic MUTYH variants in trans (c.733C>T, p.Arg245Cys [likely pathogenic]; c.842C>T, p.Ala281Val [variant of uncertain significance]), supporting a diagnosis of MAP. To our knowledge, this is the first reported case of a gastric amphicrine carcinoma in a MAP patient, expanding the spectrum of MAP-associated upper gastrointestinal neoplasia and underscoring the importance of vigilant endoscopic surveillance in hereditary polyposis syndromes.

Serine/threonine protein phosphatase type 1gamma1 is required for the completion of cytokinesis in human A549 lung carcinoma cells.

In lower eukaryotic organisms, the loss of serine/threonine protein phosphatase type 1 (PP1) results in growth arrest after the onset of mitosis. In humans, four highly homologous isoforms of PP1 (PP1alpha, PP1delta, PP1gamma1, and PP1gamma2) have been identified. Determining the roles of these phosphatases, however, has proven difficult due to the lack of subtype-specific inhibitors. In this study, we developed chimeric antisense 2'-O-(2-methoxy)ethylphosphothioate oligonucleotides targeting human PP1gamma1 that specifically inhibit PP1gamma1 gene expression. Two potent antisense oligonucleotides (ISIS 14435 and 14439; IC(50) approximately 50 nM) were then employed to elucidate the cellular functions of PP1gamma1 during cell cycle progression. In A549 cells, the inhibition of PP1gamma1 expression resulted in a dose-dependent inhibition of cellular proliferation, with growth arrest occurring after approximately 36-48 h, when PP1gamma1 mRNA expression was inhibited by >85%. Fluorescence-activated cell sorter analysis revealed that ISIS 14435/14439-induced growth arrest was associated with an increase in the number of cells containing 4N DNA. Immunostaining of treated cells revealed that the inhibition of PP1gamma1 expression had no apparent effect on the formation of mitotic spindles. However, decreased expression was associated with the failure of cell division in a late stage of cytokinesis and the formation of dikaryons.
