Acantholytic acanthoma.

Abstract

This article describes 31 examples of acantholytic acanthoma, a newly recognized, solitary, benign cutaneous tumor. Acantholytic acanthoma was typically an asymptomatic, keratotic papule or nodule. Patients ranged in age from 32 to 87 years (median 60 years); the ratio of men to women was 2:1; the most frequent clinical diagnosis was keratosis; and half of the growths were on the trunk of the body. Histologically, the lesions showed hyperkeratosis, papillomatosis, and acanthosis. Acantholysis was an outstanding finding in all cases; the patterns resembled pemphigus vulgaris, pemphigus vegetans, superficial pemphigus, or Hailey-Hailey disease, but no patient had evidence of any of these disorders. The term acantholytic is used because acantholysis is the outstanding histologic feature in these neoplasms; acanthoma was chosen because the growths are benign tumors of epidermal keratinocytes. The relationship of acantholytic acanthoma to acantholytic blistering disease is similar to that of solitary lichen planus-like keratosis to lichen planus and epidermolytic acanthoma to bullous congenital ichthyosiform erythroderma.

Mucocutaneous cholinergic system is targeted in mustard-induced vesication.

The purpose of this research is to elucidate the pharmacological mechanism mediating vesicating effects of sulfur mustard (HD) and identify an antidote to its action. HD causes blisters because epithelial cells lose their attachments. Epithelial cell adhesion is under control of the local cytotransmitter acetylcholine (ACh) working through the muscarinic and nicotinic receptor, mAChR and nAChR, classes expressed by epithelial cells. In this study, nitrogen mustard (NM)-a structural analog of HD-was used to elucidate the mechanism of vesicating effects of mustards in mucocutaneous tissues. NM caused cell detachment and cholinergic agents antagonized its effect. Radioligand binding inhibition experiments showed that NM binds to the ligand-binding site of ACh receptors (AChRs) of both classes. Ligation of AChRs on the cell membrane of keratinocytes (KC) and bronchial epithelial cells (BEC) with NM increased total esterolytic activity of serine proteinases (TEASP). Antagonists of both classes of AChRs, atropine and mecamylamine, diminished NM-induced changes, suggesting that the pathobiological effects of NM on KC and BEC result from an agonist-like degradation of ligated AChRs, predominantly of the muscarinic class. Thus, biological effects of NM on cell adhesion were antagonist-like, whereas its pharmacological effect on TEASP was agonist-like. These findings support a hypothesis that pharmacologic protection from the vesicating action of HD can be achieved by using cholinergic drugs.

Induction of pemphigus phenotype by a mouse monoclonal antibody against the amino-terminal adhesive interface of desmoglein 3.

Pemphigus vulgaris (PV) is a life-threatening autoimmune blistering disease that is caused by IgG autoantibodies against the cadherin-type adhesion molecule desmoglein (Dsg)3. Previously, we have generated an active mouse model for PV by adoptive transfer of Dsg3(-/-) splenocytes. In this study, we isolated eight AK series, anti-Dsg3 IgG mAbs from the PV mouse model, and examined their pathogenic activities in induction of blister formation. Intraperitoneal inoculation of the AK23 hybridoma, but not the other AK hybridomas, induced the virtually identical phenotype to that of PV model mice or Dsg3(-/-) mice with typical histology of PV. Epitope mapping with domain-swapped and point-mutated Dsg1/Dsg3 molecules revealed that AK23 recognized a calcium-dependent conformational epitope on Dsg3, which consisted of the V3, K7, P8, and D59 Dsg3-specific residues that formed the adhesive interface between juxtaposed Dsg, as predicted by the crystal structure. The epitopes of the mAbs that failed to show apparent pathogenic activity were mapped in the middle to carboxyl-terminal extracellular region of Dsg3, where no direct intermolecular interaction was predicted. These findings demonstrate the pathogenic heterogeneity among anti-Dsg3 IgG Abs due to their epitopes, and suggest the direct inhibition of adhesive interaction of Dsg as an initial molecular event of blister formation in pemphigus.

Kaposi's varicelliform eruption in a patient with Grover's disease.

Kaposi's varicelliform eruption is a secondary herpes simplex virus infection that affects patients in the setting of primary dermatologic conditions. It occurs with a variety of skin diseases, although association with Grover's disease has never been reported in the literature. This report describes the manifestations and clinical course. A review of the literature on Kaposi's varicelliform eruption includes disease associations, pathogenesis, and treatment.
