[Weakness, bone pain].

Abstract

A 57-year-old man complains of a reduced exercise tolerance and bone pain for years. There is a past medical history of hyperuricemia and anemia of unknown etiology. In a routine examination a leukocytosis of 64,000 per ml, increased number of thrombocytes and an elevated level of lactic dehydrogenase is found. The suspected diagnosis of myeloproliferative disorder, in the present case of chronic myelogenous leukemia, is confirmed by a diminished neutrophil alkaline phosphatase activity, bone marrow biopsy and the demonstration of the Ph1 (Philadelphia) chromosome. Magnetic resonance imaging reveals an infiltration of the bone marrow. After the initial treatment with busulfan the white cell count is lowered.

[How was the referral sheet interpreted? Questionnaire on priority assessment of patients referred to orthopedic surgeons showed great differences between reviewers].

A questionnaire containing 15 simulated referrals for orthopedic specialist treatment was distributed to physicians in charge of determining priority. They were asked to consider each referral in a normal way and grade them in order of priority. The results overall showed great divergence; units with longer waiting lists assigned the patients lower priority than did units with short waiting lists. There is a pressing need for national guidelines in order to improve the referral sheet as an instrument of communication.

[Bone diseases of the hip].

Hip pain is likely to be due to a local or regional bone disease when the hip joint is painless and of normal mobility by physical examination. Targeting the right diagnosis is easy in a patient affected by bone disease (osteoporosis, Paget's disease) or when the background of the patient is suggestive (malignancy, pregnancy). However it could be more tricky in most cases: transient osteoporosis of the hip, bone fissures, Paget's coxopathy, and tumoral cell osteolysis of bones surrounding the hip are the main causes, in addition to femoral head osteonecrosis, which is detailed in another chapter.
