Benzodiazepine problems.

Abstract

Benzodiazepines present problems related to both unwanted and withdrawal effects. Dosage adjustments usually obviate unwanted effects except for paradoxical reactions such as hostility. Patients with apparent benzodiazepine dependence need careful assessment with respect to personality, social situation and psychiatric disorder. The patient must be motivated and carefully prepared for withdrawal and taught anxiety management techniques. Withdrawal must always be gradual over at least 6 weeks but very prolonged schedules are counter-productive. Substituting a long-acting for a medium-acting benzodiazepine may be helpful in the more intractable cases. An antidepressant may be needed if a depressive disorder supervenes, but other adjunctive therapies are usually unhelpful.

Bayesian predictive approach to interim monitoring in clinical trials.

This paper reviews Bayesian strategies for monitoring clinical trial data. It focuses on a Bayesian stochastic curtailment method based on the predictive probability of observing a clinically significant outcome at the scheduled end of the study given the observed data. The proposed method is applied to derive efficacy and futility stopping rules in clinical trials with continuous, normally distributed and binary endpoints. The sensitivity of the resulting stopping rules to the choice of prior distributions is examined and guidelines for choosing a prior distribution of the treatment effect are discussed. The Bayesian predictive approach is compared to the frequentist (conditional power) and mixed Bayesian-frequentist (predictive power) approaches. The interim monitoring strategies discussed in the paper are illustrated using examples from a small proof-of-concept study and a large mortality trial.

[Treatment of postictal (emergence) delirium after electroconvulsive therapy].

Most patients treated with electroconvulsive therapy (ECT) develop cognitive and motor aspects of delirium afterwards. These symptoms are often self-limiting, lasting 10 to 20 minutes. However, about 10% of these patients develop a state characterised by confusion, restlesness, repetitive movements, lack of response to commands and amnesia regarding the period. This state, called emergence delirium, usually has a short duration but may persist for hours. In the present case a 50-year-old woman had 18 ECT treatments for severe depression. She experienced excellent effects as regards the depressive symptoms, but postictally she developed all the above-mentioned symptoms of delirium, which lasted for several hours. The ECT continued, and the patient was successfully treated for the condition with administration of i.v. diazepam (5 mg) shortly after the end of each seizure.

Extraction and analysis of flunitrazepam/7-aminoflunitrazepam in blood and urine by LC-PDA and GC-MS using butyl SPE columns.

The forensic toxicology community has recognized flunitrazepam and its metabolite (7-aminoflunitrazepam) as compounds of concern for several years. In this procedure, the analytes were extracted from whole blood and urine onto single mode solid phase cartridges (butyl) using nitrazepam as an internal standard. The columns were washed with distilled water and hexane. All three compounds were eluted from the sorbent using an ethyl acetate-methanol solvent mixture. After collection and evaporation of the solvent, the residue was dissolved in A, 0.1% (v/v) aqueous trifluoroacetic acid for HPLC-PDA analysis or B, ethyl acetate for derivatization with pentafluoropropionic anhydride (PFPA) for analysis by gas chromatography-mass spectrometry (selected ion monitoring, SIM). A limit of quantitation for this method using HPLC-PDA was found to be 5 and 1.0 ng mL(-1) by SIM.
