Optimized brainstem auditory evoked potentials estimation using simulated annealing.

Abstract

In this paper, we use a new approach based on Simulated Annealing for estimating the BAEPs (brainstem auditory evoked potentials). Each BAEP is obtained through a hundred of responses to stimulations. In case of endocochlear pathologies, it has been assumed that these signals could be randomly delayed from a response to another one. In such cases, the application of the averaging method systematically leads to "smoothed" BAEPs, thus complicating both identification and interpretation operations. The method presented in this paper consists in minimizing a non linear criterion in order to obtain an alignment of the various responses, before averaging them. Simulated and experimental results are presented, and compared to those produced by the classical method.

Isolated Dandy-Walker malformation associated with brain stem dysgenesis in male sibs.

We describe two brothers with isolated Dandy-Walker malformation (DWM). Interestingly, brain stem dysgenesis and abnormal gyral pattern were also observed in the sibs. They presented with psychomotor retardation and macrocrania. Both suffered from hypotonia with brisk deep tendon reflexes and ataxic gait. They had bilateral optic atrophy and the visual evoked potentials documented prolonged latencies. Further, motor and sensory conduction velocities were normal. Chromosomal examinations for the sibs and their parents showed normal results. The majority of cases are sporadic but rare reports of recurrence in siblings exist. The parents' consanguinity and the recurrence in a subsequent pregnancy suggest an autosomal recessive inheritance pattern. Our report adds more weight that brain stem dysgenesis could be associated with DWM, increasing the spectrum of heterogeneity of this malformation.

Bickerstaff's brainstem encephalitis: can it recur?

Bickerstaff's brain-stem encephalitis is usually a monophasic post-viral inflammatory illness characterized by progressive ophthalmoplegia, ataxia and disturbance of consciousness (or hyper-reflexia). Since the clinical spectrum of Bickerstaff encephalitis may overlap with the Miller-Fisher and Guillain-Barré syndromes, the presence of anti- GQ1b antibodies and abnormal brain MRI can help to support its diagnosis. However, absence of anti-GQ1b antibodies and normal MRI do not exclude the diagnosis, which remains based on clinical criteria and exclusion of other etiologies. We report a case of recurrent Bickerstaff's brainstem encephalitis with no identifiable antecedent illness, and overlapping features of Miller Fisher and Guillain-Barré syndromes, in the presence of negative anti-GQ1b antibodies and repeatedly normal MRI of the brain.
