Arteriovenous access: managing common problems.

Abstract

The creation and maintenance of hemoaccess occupies a significant portion of most vascular and general surgery practices. In this article, the methods used to detect hemoaccess at risk for failure and the endovascular and surgical techniques used to prolong or restore their patency are reviewed. Also, the management of hemoaccess infection, aneurysmal degeneration, false aneurysm formation, and symptomatic arterial steal syndrome are described.

Popliteal artery pseudoaneurysm after a revision of total knee arthroplasty: endovascular treatment with a stent graft.

Injury to the popliteal artery during total knee arthroplasty is a very rare but dangerous complication. Several mechanisms are capable of generating a direct trauma to the vessel, like posterior retractor and oscillating saw. We report a case of popliteal artery pseudoaneurysm in a 52-year-old woman that occurred during revision of total knee arthroplasty, requiring emergency repair by means of percutaneous endovascular covered stenting.

Pseudoaneurysm of the inferior gluteal artery: an unusual complication after abdominoperineal resection for rectal cancer. Report of a case.

We report an unusual case of profuse, sporadic hemorrhage from a chronically infected perineal wound after an abdominoperineal resection. Surgical exploration and angiography on two occasions failed to identify the source of bleeding. A pseudoaneurysm of the internal iliac artery was eventually identified with a provocative angiogram using tissue plasminogen activator. This aneurysm was successfully treated with embolization with coils.

Pseudoaneurysm of the common carotid artery due to ongoing trauma from the hyoid bone.

We report a left common carotid artery pseudoaneurysm in a 47-year-old bricklayer with no prior surgical procedures or acute neck injury. At operation, the pseudoaneurysm was found to be related to ongoing injury to the carotid artery from the tip of the hyoid bone. The most likely explanation for this was that the patient's job required that he carry cement panniers weighing about 15 kg on his left shoulder and that these panniers repeatedly compressed the left side of his neck, thus forcing the carotid against the hyoid.
