Double-balloon endoscopy.

Abstract

In 1997, we devised a method of enteroscopy that uses 2 balloons, 1 balloon attached to the tip of the endoscope and the other balloon attached to the distal end of a soft overtube. We reported that endoscopic observation of the entire small intestine was possible with this method by using an endoscope of 200-cm working length. This double-balloon endoscopy system now is available commercially. We have experience with more than 250 cases of enteroscopy since September 2000 using the double-balloon endoscope including our experience with the prototype. Recently, a new therapeutic type of double-balloon endoscope was developed. The new type has an accessory channel of 2.8 mm, which is bigger than the 2.2-mm accessory channel of the standard double-balloon endoscope.

[Microcatheter in neuroradiology].

The application of neuroradiologic microcatheters may be beneficial for the patient as well as preoperative intervention or as sole therapeutical procedure. However, it is associated with a relatively high procedural risk, which is also supported by critical dimensions of the microcatheter. It is regrettable, that to some extent, the performance of manufacturers of microcatheters stay below today's technical possibilities. Conducted investigations of ruptured catheters could not prove, that ruptures could have been avoided by using better manufacturing techniques, however, it should be expected, that manufacturers ensure the highest possible technical performance. Attempts to re-process those products, because of their high price show, that not all technical problems and resulting risks are well understood by all parties concerned. The demand for re-processing is not congruous with current medical responsibility.

[New CT program simulating kidney displacement during retroperitoneal laparoscopic nephrectomy].

A total of 12 patients with malignant localized renal or ureteral neoplasms underwent multi-slice computed tomography. Imaging data were sent to the dedicated workstation to create volume rendering and virtual laparoscopic images of the kidney which was displaced ventrally with retroperitoneal balloon. These findings were compared with video images obtained during retroperitoneal laparoscopic nephrectomy. The kidney displacement simulator depicted all renal arteries (100% sensitivity) and 13 of 14 renal veins (93% sensitivity). Hilar anatomy, including the tumor, major vessels and their relationships were visualized as in the actual laparoscopic views. The desired portions of major vessels as well as the left adrenal and gonadal veins visualized with this system completely corresponded with the actual laparoscopic images during surgery. The kidney displacement simulator is useful to foresee desired portions of major vessels and branched small vessels such as the adrenal or gonadal veins in advance of surgery. It is thus able to guide surgeons and reduce operative risks and possible complications.

Intrauterine insemination catheters for assisted reproduction: a systematic review and meta-analysis.

BACKGROUND: Intrauterine insemination (IUI) is the oldest and most practised form of assisted reproduction worldwide. We systematically reviewed the literature so that we could evaluate the use of soft versus firm catheters in subfertile women undergoing IUI. METHODS: Extensive searches were conducted for full-text manuscripts, conference abstracts, ongoing and unpublished trials. Primary outcomes were clinical pregnancy (CPR) and ongoing pregnancy (OPR)/live birth rates (LBRs) per woman. Secondary outcomes were multiple pregnancy rate (MPR) per clinical pregnancy, difficulty cannulating the cervix, bleeding and patient discomfort. Meta-analysis was performed using the Peto-modified Mantel-Haenszel fixed-effect model. RESULTS: Seven randomized trials were identified, and four were excluded. No significant differences were noted for CPR and LBR per woman [OR = 0.96, 95% CI = 0.70-1.32 and OR = 0.82, 95% CI = 0.43-1.58, respectively]. As for the secondary outcomes, MPRs per cycle were also not significantly different. More difficulty was noted with soft catheters and more patient discomfort with firm catheters. Bleeding following the procedure was similar between the two groups. CONCLUSIONS: Catheter choice during IUI does not seem to be a detrimental factor for success, as in other assisted reproduction techniques (ART). More studies are warranted to draw definitive conclusions and support the results of this systematic review.
