Electrogastrography: basic knowledge, recording, processing and its clinical applications.

Abstract

The slow wave (SW) of the gastrointestinal (GI) tract mainly functions to trigger the onset of spike to elicit smooth muscle contraction, which provides the essential power of motility. Smooth muscle myogenic control activity or SW is believed to originate in the interstitial cells of Cajal (ICC). The electrical coupling promotes interaction between muscle cells, and ICC additionally contribute to SW rhythmicity. Stomach SW originates in the proximal body showing the continuous rhythmic change in the membrane potential and propagates normally to the distal antrum with a regular rhythm of approximately 3 c.p.m. A technique using electrodes positioned on the abdominal skin to pick up stomach rhythmic SW refers to electrogastrography (EGG). The stomach SW amplitude is very weak, while many visceral organs also produce rhythmic electricities, for example heartbeat, respiration, other organs of the GI tract and even body movements. Thus noise other than SW should be filtered out during the recording, while motion artifacts are visually examined and deleted. Finally, the best signal among all recordings is selected to compute EGG parameters based on spectral analysis. The latter is done not only to tranform frequency domain to time domain but also to provide information of time variability in frequency. Obtained EGG parameters include dominant frequency/power, % normal rhythm, % bradygastria, % tachygastria, instability coefficient and power ratio. Clinical experience in EGG has been markedly accumulated since its rapid evolution. In contrast, lack of standardized methodology in terms of electrode positions, recording periods, test meals, analytic software and normal reference values makes the significance of EGG recording controversial. Unlike imaging or manometrical studies, stomach motility disorders are not diagnosed based only on abnormal EGG parameters. Limitations of EGG recording, processing, computation, acceptable normal parameters, technique and reading should be known to conduct subjective assessments when EGG is used to resolve stomach dysfunction. Understanding basic SW physiology, recording methodology and indications may open EGG as a new domain to approach the stomach motor dysfunction.

Association of different prediction methods for determination of the efficiency and selectivity on neuron-based sensors.

A technique has been developed to determine the efficiency and the selectivity of a single neuron-based sensor in identifying the nature of the chemical agents in an unknown sample. This has been achieved by exploiting the unique electrical identifiers, also known as "signature patterns", generated by the neuronal cell membrane. These were generated based on the variations to the extracellular electrical activity, due to the effect of a broad range of chemical agents. We demonstrate the prediction capability of the sensor in identifying the nature of an unknown test sample from a combination of three chemical agents, namely, ethanol, pyrethroid, and hydrogen peroxide. This was achieved through a two-step process. The first step was experimentally achieved by in situ recording of the changes to the extracellular electrical activity from the sensing sites or the array of microelectrodes that form the platform for patterning neurons. Simultaneous optical characterization of the cell array during the sensing process was performed to identify the associated physiological changes. The second step was mathematical and was based on developing a library of signature patterns for a set of concentrations of the various combinations of the three chemical agents. Two variants of the nearest neighbor algorithm scheme - (a) partial distance search method, and (b) search tree method, were implemented for the accurate detection of all the components with varying concentrations in the test samples of unknown nature. This technique exhibits reliability in identification up to parts-per-billion (ppb) sensitivity. The capability of standardization of this technique for potential commercial applications is also discussed.

Functional verification of pulse frequency modulation-based image sensor for retinal prosthesis by in vitro electrophysiological experiments using frog retina.

The functioning of a 16 x 16 pixel pulse frequency modulation (PFM) image sensor for retinal prosthesis is verified through in vitro electrophysiological experiments using detached frog retinas. This image sensor is a prototype for demonstrating the application to in vitro electrophysiological experiments. Each pixel of the image sensor consists of a pulse generator (PFM photosensor), a stimulus circuit, and a stimulus electrode (Al bonding pad). The image sensor is fabricated using standard 0.6 microm CMOS technology. For in vitro electrophysiological experiments, a Pt/Au stacked electrode is formed on the Al bonding pad of each pixel and the entire sensor is fixed in epoxy resin. The PFM image sensor is confirmed experimentally to provide electrical stimulus to the retinal cells in a detached frog retina.

Chronic inflammatory demyelinating polyradiculoneuropathy with tumefactive central demyelination.

A 46-year-old man with a 1-year history of distal paresthesias and mild distal weakness subacutely developed paralysis of the left hand. Electrodiagnostic evaluation revealed a demyelinating peripheral neuropathy that met criteria for chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). Magnetic resonance imaging of the brain revealed a mass that enhanced with contrast, but revealed focal myelin loss with intense macrophage activity and axonal preservation on biopsy. The mass and hand weakness improved following steroid therapy. The combination of CIDP and central demyelination with mass effect broadens the spectrum of demyelinating disease in association with CIDP.
