Linear accelerator output variability.

Abstract

The Wellington Cancer Centre is equipped with two matched linear accelerators (Varian 2100CD) linear accelerators (identified as SN1027 and SN42). Each morning, before treatments commence, a radiation therapist carries out an output constancy check of the radiation output and every fortnight a physicist measures, in a phantom, the delivered radiation dose to check on the machine calibration. The daily output checks have been recorded into a database (Argus QA for Radiation Oncology) since August 1997 and in June 1998 the fortnightly calibration measurements were added. The information in the database, up to April 2003, has been analysed to consider the quality of the daily constancy checks as compared with the fortnightly calibration measurements and whether the data contains useful information on machine performance. After allowance for the effects of machine recalibration the fortnightly calibration measurements had an average standard deviation of 0.4% and the daily constancy checks 0.8%. The daily constancy checks had a greater number of large deviations than would be expected assuming a normal distribution and were not a good predictor of the need for a recalibration. The fortnightly calibration measurements with a much lower spread give a reliable indication of the need for a recalibration allowing the adoption of a +/- 1% tolerance. Over the period analysed one accelerator (SN42) was relatively stable with the output generally drifting between +/- 1% while the other (SN1027) had a consistent increase in the average output of about 2.5% per year.

Determination of loratadine and its active metabolite in human plasma by high-performance liquid chromatography with mass spectrometry detection.

A new sensitive and selective liquid chromatography coupled with mass spectrometry (LC/MS/MS) method for quantification of loratadine (LOR) and its active metabolite descarboethoxyloratadine (DSL) in human plasma was validated. After addition of the internal standard, metoclopramide, the human plasma samples (0.3 ml) were precipitated using acetonitrile (0.75 ml) and the centrifuged supernatants were partially evaporated under nitrogen at 37 degrees C at approximately 0.3 ml volume. The LOR, DSL and internal standard were separated on a reversed phase column (Zorbax SB-C18, 100 mmx3.0 mm i.d., 3.5 microm) under isocratic conditions using a mobile phase of an 8:92(v/v) mixture of acetonitrile and 0.4% (v/v) formic acid in water. The flow rate was 1 ml/min and the column temperature 45 degrees C. The detection of LOR, DSL and internal standard was in MRM mode using an ion trap mass spectrometer with electrospray positive ionisation. The ion transitions were monitored as follows: 383-->337 for LOR, 311-->(259+294+282) for DSL and 300-->226.8 for internal standard. Calibration curves were generated over the range of 0.52-52.3 ng/ml for both LOR and DSL with values for coefficient of determination greater than 0.994 by using a weighted (1/y) quadratic regression. The lower limits of quantification were established at 0.52 ng/ml LOR and DSL, respectively, with an accuracy and precision less than 20%. Both analytes demonstrated good short-term, long-term, post-preparative and freeze-thaw stability. Besides its simplicity, the sample treatment allows obtaining a very good recovery of both analytes, around 100%. The validated LC/MS/MS method has been applied to a pharmacokinetic study of loratadine tablets on healthy volunteers.

High performance liquid chromatography-electrospray ionization mass spectrometric determination of isosorbide 5-mononitrate in human plasma.

A novel, selective and sensitive high performance liquid chromatography-mass spectrometric (HPLC-MS) method has been developed for the determination of isosorbide 5-mononitrate (5-ISMN) in human plasma. With acetaminophen as internal standard, sample pretreatment involved one-step extraction with diethyl ether of 0.5 mL plasma. Analysis was performed on an ACQUITY UPLC BEH C(18) column (100 mm x 2.1mm, 1.7 microm) with mobile phase consisting of acetonitrile-water (20:80, v/v). The detection was carried out by means of electrospray ionization mass spectrometry in negative ion mode with selected ion recording (SIR). Standard curves were linear (r(2)> or =0.99) over the concentration range of 1.04-1040 ng/mL. The lower limit of quantification (LLOQ) was 1.04 ng/mL. The intra- and inter-day precisions (RSDs) were less than 8.6% and 13.4%, respectively, and the accuracy (RE) was within +/-0.45%. The method herein described was fully validated and successfully applied to the pharmacokinetic study of 5-ISMN in compound extended-release tablets in 18 healthy male volunteers after oral administration.
