Colonoscopy, tumors, and inflammatory bowel disease - new diagnostic methods.

Accurate detection of premalignant lesions and early cancers in the colon is essential for curative endoscopic or surgical therapy, since the prognosis for the affected patients is closely related to the size and stage of the neoplastic lesion. Total colonoscopy is the accepted gold standard for screening and surveillance of colorectal cancer. This review summarizes recently published diagnostic developments and key findings in the areas of colonoscopy, colonic tumors, and inflammatory bowel diseases. Relevant findings have been reported for chromo-endoscopy in the diagnosis of colitis-associated neoplasia, as well as flat and depressed adenomas. Real-time Doppler capabilities have now been added to endoscopic optical coherence tomography; the results of large-scale testing of narrow-band imaging endoscopy in the colon are being awaited; and fluorescence imaging has recently been added to the facilities available in video endoscopy. Most importantly, endomicroscopy now for the first time allows single-cell subsurface imaging during ongoing colonoscopy procedures, opening the way to in-vivo molecular and functional imaging.

[Results of serotyping of strains isolated from patients with ulcerative colitis (colitis gravis), adaptive colitis and chronic catarrhal colitis].

Serotyping of 565 E.coli strains isolated from 273 patients with ulcerative colitis (70 patients); chronic catarrhal colitis (96 patients), adaptive colitis (107 patient) and of 72 E. coli strains isolated from 50 practically healthy persons (control group) was performed. Serotyping was performed in reaction of agglutination, on stage-glass, by use of commercial set consisting of O and H monovalent serum. As a result of performed investigation it was determined, that in patients with different intestinal inflammatory pathologies, there were found significant pathologic changes in intestinal microbiocenosis, in which main role are playing by strains of E. coli. Such violations of intestinal microbiocenosis often are leading to complications of basic disease. Serotyping of E. coli strains has shown, that in formation of pathologic microbiocenosis with more or less equal frequency are taking part enteropathogenic, enteroinvasive, enterotoxic strains of E. coli and also some definite serotypes of E. coli - representatives of normal intestinal microflora with ability of synthesis of thermolabile enterotoxin.

Indeterminate colitis: a pathologist's view.

Indeterminate colitis is only a partial diagnosis, which is provisional until sufficient information becomes available for a more precise diagnosis of either ulcerative colitis or Crohn's disease. As originally defined, i.e. based only on the pathological features in the colon resected for fulminant colitis, a few patients eventually developed Crohn's disease. If the term is used only after all available clinical and investigative evidence is considered, 'indeterminate colitis' behaves clinically like ulcerative colitis. This has important implications for pouch surgery.
