[Cochlear implantation (problems, methods, perspectives)].

Abstract

We suppose that topical problems of cochlear implantation comprise preoperative diagnosis of the acoustic nerve condition, programmimg of the speech processor using objective evaluation of acoustic function, design of new strategies of stimulation of speech processors and criteria of choice for existing strategies. In solving the above problems, the following methods are of special importance: registration of electrically evoked potential of acoustic nerve action (EPAN), registration of electrically evoked acoustic potentials (EAEP), registration of electrically evoked reflex of the musculus stapedius (ERMS). 27 patients with cochlear implants were examined to evaluate the above methods in terms of speech prosesses programming. The findings enable the conclusion that all three methods can be successively used in programming of the speech processor. While EPAN and EAEP can be used for approximate estimation of sensation threshold, ERMS registration is highly effective for determinaion of maximal comfortable level of stimulation.

[Current developments in cochlear implantation].

Over the last 20 years, cochlear implantation has become a well accepted treatment in patients suffering from hearing loss or congenital deafness. Results have been impressive,and indications for a cochlear implant have been extended continuously. Thanks to a sophisticated diagnostic procedure, optimization of the surgical technique, and the progression of interdisciplinary cooperation in the field of rehabilitation, the acceptance of cochlear implantation is growing significantly. The manufacturers of the devices are making great efforts to miniaturize the external components of the system, and new developments in electrode arrays, combined with new speech coding strategies, result in better speech understanding. The new developments in electrode design, however, are not necessarily improvements over recent years. Nevertheless, a very well functioning network of physicians, scientists, and manufacturers has acted and reacted in an outstanding manner to identify possible causes of post-implantation meningitis, have taken immediate counter measures and presented possibilities of prevention.

Evaluating the health-related quality of life effects of cochlear implants: a prospective study of an adult cochlear implant program.

This paper prospectively documents the health-related quality of life (HRQoL) and social participation benefits of adult patients receiving cochlear implants in Australia and New Zealand. Thirty-four consecutively implanted patients completed the Assessment of Quality of Life (AQoL) and Hearing Participation Scale (HPS) instruments before implantation, and at 3- and 6-month follow-ups. Implantation resulted in significant improvements in AQoL and HPS scores, The effect size was 1.09 for both measures. Those in the top socio-economic tertile obtained the greatest gains. The HRQoL and social participation benefits were slightly larger than those reported elsewhere. This may be because participants used more recent technology (Nucleus 24 rather than Nucleus 22) and received auditory and self-efficacy training as part of their rehabilitation. The results suggest that cochlear implants have a large beneficial effect. They show that social and HRQoL outcomes can be parsimoniously measured using the HPS and AQoL instruments.
