Great advances have been made in the past half-century in our understanding of meibomian gland function and of the many inflammatory, hormonal, and infectious processes leading to its dysfunction. These advances have led to an exploration of exciting new avenues for treatment of this common but frustrating disease entity.
We describe a new approach for the closure of eyelid margin defects. We conducted a prospective evaluation of patients undergoing the buried vertical mattress technique for full-thickness eyelid margin repair and found that 90 patients (follow-up, 3 to 18 months; average, 6 months) had eyelid margin defects closed with the use of a buried vertical mattress technique. There were no cases of dehiscence, and only 5 (5.5%) patients had minor notch formation. We conclude that the buried vertical mattress suture technique can be a safe and effective method to close eyelid margin defects while saving postoperative time and the cost of multiple sutures.
We report a case of a man who had undergone gold eyelid loading for lagophthalmos 22 years earlier. During examination for a complaint of foreign body sensation in the operated eye, the gold weight was found under the palpebral conjunctiva. Although extrusion of a gold weight through the skin anteriorly is well recognized, posterior extrusion has not been previously reported. Although such an extrusion involves tarsal plate erosion, the patient's complaints in this case were minor, probably due to the chronic nature of the process. This finding should serve to draw the attention of surgeons using gold eyelid loading to the possible, albeit rare, complication of posterior extrusion.
