Chylothorax is a rare but serious complication after oesophagectomy procedure. We report the case of a 59-year-old man who underwent an oesophagectomy by Akiyama procedure. A persistent postoperative chylothorax occurred requiring drainage and conservative management. After one week, the failure of this management motivated the institution of continuous infusion of somatostatin. This led to a rapid cessation of chyle production without side effect and to the discharge of the patient from the intensive care unit.
A 2-month-old premature infant developed a spontaneous chylothorax. Medical therapy (chest tube drainage, parenteral alimentation) and thoracic duct ligation at 103 days of age had no effect. Drainage ceased during an octreotide infusion. The effusion recurred after octreotide was stopped, and responded completely to a longer course of therapy. Octreotide offers a substantial advantage over current medical management to some babies with spontaneous chylothorax.
We report a neonate with severe congenital chylothorax. Subcutaneous octreotide was added to the standard treatment regime. The chylothorax resolved with no observed side effects.
