Pulsatile tinnitus.

Abstract

Pulsatile tinnitus is an uncommon otologic symptom, which often presents a diagnostic and management dilemma to the otolaryngologist. This symptom always deserves a thorough evaluation to avoid disastrous consequences from potentially life-threatening associated pathology. In most pulsatile tinnitus patients a treatable underlying etiology can be identified.

Accuracy of ICD-9 codes in identifying ischemic stroke in the General Hospital of Lugo di Romagna (Italy).

We assessed the sensitivity and the positive predictive value (PPV) of the ICD-9 codes in identifying ischemic strokes. The study involved the cross-sectional comparison between patients with an ischemic stroke diagnosis made by neurologists and patients with the 434 or 436 discharge codes. Sensitivity of the codes (all diagnostic levels and first level respectively) was 82% and 76%; PPV: 71% and 76%. The annual crude incidence of ischemic stroke was 2.62 per 1000 based on verified strokes and 3.03 per 1000 based on 434 or 436 coded medical records (at all diagnostic levels). Thirty-day case fatality ratio was 22.3% in verified strokes and 36.8% among patients diagnosed with codes 434 or 436 but without stroke (all levels). Our results disclosed inaccuracy in use of the ICD-9 codes in the diagnosis of ischemic stroke in the general hospital of Lugo di Romagna, Ravenna Province, Italy. The misdiagnosis of patients could be influenced by the degree of severity of clinical features. Epidemiological data and cost-analysis forecasts based only on the ICD-9 system must be considered with caution.

Vascular cognitive impairment.

Cerebrovascular disease is the second most common cause of acquired cognitive impairment and dementia and contributes to cognitive decline in the neurodegenerative dementias. The current narrow definitions of vascular dementia should be broadened to recognise the important part cerebrovascular disease plays in several cognitive disorders, including the hereditary vascular dementias, multi-infarct dementia, post-stroke dementia, subcortical ischaemic vascular disease and dementia, mild cognitive impairment, and degenerative dementias (including Alzheimer's disease, frontotemporal dementia, and dementia with Lewy bodies). Here we review the current state of scientific knowledge on the subject of vascular brain burden. Important non-cognitive features include depression, apathy, and psychosis. We propose use of the term vascular cognitive impairment, which is characterised by a specific cognitive profile involving preserved memory with impairments in attentional and executive functioning. Diagnostic criteria have been proposed for some subtypes of vascular cognitive impairment, and there is a pressing need to validate and further refine these. Clinical trials in vascular cognitive impairment are in their infancy but support the value of therapeutic interventions for symptomatic treatment.

California Verbal Learning Test: performance by patients with focal frontal and non-frontal lesions.

Although frontal lobe lesions do not cause classic amnesia, they may disrupt learning and memory in a number of ways. To investigate in finer detail the regions of frontal injury that are associated with impaired learning and to define the cognitive processing deficits specific to each region that disrupt memory, we compared 33 patients with focal frontal injury with patients with non-frontal injury and with normal controls on a standard neuropsychological instrument, the California Verbal Learning Test (CVLT). Subgroups of patients with distinct lesion site profiles were compared in a number of learning measures. All of the subgroups of patients with frontal lesions (with one exception) had inefficient learning due to poor implementation of a strategy of subjective organization. Despite this organizational deficiency, the performance of patients with frontopolar lesions normalized across trials. Only the subgroups with lesions centred either on the left posterior dorsolateral frontal region or the posterior medial frontal region had overall impaired learning and recall. The left posterior dorsolateral frontal group was most significantly impaired on all measures. This recall impairment was secondary to a mild lexical-semantic deficit. A recognition memory deficit in the same group was due to an abnormal response bias. Several groups had a modest increase in perseverative recalls; the underlying mechanisms differed. Disruption of different cognitive processes associated with specific frontal regions underlies the varied patterns of memory impairment. This study has demonstrated even finer differentiations within the frontal region than previously known.
