Once-daily therapies.

Abstract

Combination antiretroviral therapy is now standard treatment for HIV infection. However, adherence to these treatment regimens remains a formidable barrier, owing to their complex and demanding nature and the severity of the associated toxicities. As suboptimal adherence increases the risk of treatment failure, the need for simpler and safer regimens is imperative. A number of strategies have been used to simplify dosing regimens and reduce pill burdens. At present, several antiretroviral drugs can be used once daily, while others have demonstrated pharmacokinetic potential for once-daily dosing and a number of compounds are in clinical development. It is possible that in the future, once-daily dosing will become the standard.

Conformational studies of novel antiretroviral analogs of zidovudine.

Conformational properties of three novel zidovudine analogs, namely 3'-azido-3'-deoxy-5'-O-isonicotinoylthymidine (AZT-Iso, 2), (-)-trans-(5S,6S)-5-bromo-6, 5'-epoxy-5,6-dihydro-3'-azido-3'-deoxythymidine (3) and (+)-trans-(5R,6R)-5-bromo-6,5'-epoxy-5,6-dihydro-3'-azido-3'-deoxythymidine (4), have been investigated by AM1 calculations and NMR studies, and compared with those of the parent nucleoside (AZT, 1). Based on the results obtained the following correlation may be established, a) AZT and AZT-Iso exhibit a conformational behavior analog to other pyrimidinic nucleosides, displaying a dynamic equilibrium in solution where the two conformers (North and South) undergo a constant transformation. b) Compounds 3 and 4 show a different conformational profile. The estimate of the pseudorotation phase angle reveals the rigid structures of the latter compounds, which do not evidence conformational equilibrium in solution; the azide group being the only group free to rotate. c) Diastereoisomers 3 and 4 exhibit an extra conformational parameter compared with other pyrimidinic nucleosides: the chair or boat conformation in the third ring formed between the sugar and the base. In all cases, a reasonable correlation was obtained between theoretical and NMR spectroscopic data.

Can drug resistance mutations influence the measurement of plasma HIV-RNA by different viral load techniques?

Guidelines for the use of antiretroviral agents consider that viral load values may differ using the branched DNA (bDNA) and polymerase chain reaction (PCR) methods. Whereas the most commonly used PCR assay (Amplicor v1.5, Roche) targets HIV gag sequences, the bDNA (Quantiplex v3.0, Bayer) is based on the recognition of HIV pol sequences. The possibility that accumulation of drug-resistance mutations at the reverse transcriptase and/or protease genes could influence viral load measurements using bDNA was examined in a case control study. Plasma samples collected from 46 HIV-infected individuals receiving antiretroviral therapy and carrying different number of drug resistance mutations were analyzed. The performance of both bDNA and PCR assays was found to be comparable irrespective of the presence or absence of drug resistance mutations.

Challenges facing health professionals caring for HIV-infected drug users.

Although injection drug use accounts for only 5% to 10% of cumulative HIV infections globally, it is a more efficient way of spreading HIV than sexual intercourse. HIV epidemics among injection drug users (IDUs) have a potential for rapid spread of the virus within the IDU community and outward into the general population. Effective interventions addressing this mode of HIV transmission are needed because part of a comprehensive strategy to curb the spread of HIV infection. IDUs, often marginalized, pose serious health challenges that can no longer be overlooked. Health care providers need to familiarize themselves with these challenges in order to meet the needs of this disenfranchised population. Providers need to have a clear understanding of targeted behavior and their patients' environmental context. Efforts must be made to foster healthy behavior among IDUs to assist them in managing their infection effectively and become productive members of society. This paper reviews these challenges and outlines selected models that may assist health care providers in fostering behavior changes among HIV-positive injection drug users.
