Isolated jejunal varices.

Abstract

Isolated jejunal varices are an uncommon manifestation of portal hypertension. A one and a half year old boy presented with recurrent, massive gastrointestinal bleeding from jejunal varices. The bleeding site was identified at exploratory laparotomy. Jejunal resection and anastomosis resulted in complete resolution of the bleeding and there has been no recurrent bleeding over an eight month follow-up period.

[Deflazacort and colonic anastomotic healing. An experimental study in rats].

BACKGROUND: Deflazacort, a new corticosteroid is claimed to induce less collateral effects than other corticoids. The aim of this study was to investigate the effect of this drug on the healing of colonic anastomosis. MATERIAL AND METHOD: 75 Wistar rats were randomized to receive daily by gavage, one of the following treatments: control group (N=24) - 1 ml of saline; deflazacort (N=25) - 1 ml of this drug containing 0.15mg/ml (0,1 mg/kg/day), and dexamethasone (N=26) - 1 ml containing 0.03 mg/ml of this drug (0.5 mg/kg/day). After 6 days of treatment, the animals underwent laparotomy followed by section and immediate colonic anastomoses in a point 2 cm above peritoneal reflexion. Treatment in each group was carried out until the animals were killed on the third of a six postoperative day. At necroscopy, signs of peritonitis and anastomotic dehiscence were registered. Anastomotic site was tested for tensile rupture and biopsies containing all layers were sent for hydroxyproline and protein measurements. RESULTS: There was no difference in the peritonitis and anastomotic dehiscence rates among the groups. Bowel rupture pressure was similar at day 3 but significantly higher in controls (180 [150-230] mmHg) than in deflazacort (150 [120-180] mmHg) and dexamethasone groups (140 [100-180] mmHg) at day 6. No difference occurred in the hydroxyproline concentration. Protein content was higher in dexamethasone group than in controls on the third postoperative day. CONCLUSIONS: Corticosteroids may impair colonic anastomotic resistance and deflazacort may have similar deleterious effects of dexamethasone in anastomotic healing.

Chronic expansion of the renal pelvis: a new method for reconstruction of upper ureteral defects.

OBJECTIVES: To evaluate whether the dilated renal pelvis can be used as an autologous source for the surgical reconstruction of upper ureteral defects or strictures. METHODS: In 7 female pigs, the renal pelvis was expanded by a percutaneously placed Council balloon catheter. Every other day for 4 weeks, the renal pelvis was progressively dilated with a bolus injection of saline and contrast medium, which allowed expansion of the renal pelvis to 70 to 75 mL. Four to six weeks after the initial intervention, 5 to 7 cm of the proximal ureter was resected in an open operation and replaced with a tubularized spiral flap made from the expanded renal pelvis. Three weeks later, the animals were killed, and the area of manipulation was resected for pathologic evaluation. RESULTS: All animals reached the desired expansion of the renal pelvis, and in all cases, the spiral flap was fashioned and anastomosed to the distal ureteral segment, bridging the initial defect (mean length 7 cm). Two animals died from sepsis, 4 and 6 days after the spiral flap reconstruction. A viable ureteral lumen with a patent anastomosis and a functioning pelvocaliceal unit was observed on intravenous urography in all animals. The main histologic findings were a chronic inflammatory process with concomitant mucinous metaplasia and reactive atypia of the tubules. CONCLUSIONS: The use of a balloon expander in the renal pelvis is a safe and effective technique for producing native tissue for the reconstruction of defects or strictures of the upper ureteral segment.

Cryptorchidism: fowler-stephens procedure or autotransplantation-a new experimental model.

OBJECTIVES: To investigate the effect of different operative procedures on testicular development in cryptorchidism. METHODS: One hundred ten postpubertal Wistar-WU rats were divided into four groups. Group 1 (35 rats) underwent microsurgical autotransplantation of the right testis with end-to-end anastomosis of the testicular vessels. Group 2 (35 rats) underwent the Fowler-Stephens maneuver with intra-abdominal fixation of the testis. To determine the influence of temperature on testicular development, group 3 (35 rats) underwent the Fowler-Stephens maneuver with scrotal replacement of the testis. Group 4 (5 rats) was a control group without operative treatment. To assess the early and late changes of testicular ischemia and reperfusion, groups 1 to 3 were divided into seven subgroups (5 rats each) with defined postoperative intervals (0.5 hour, 2 hours, 4 hours, 8 hours, 16 hours, 14 days, and 60 days). Testicular development, sperm density, pH value, tissue lactate concentration, and tubular diameter were the parameters used to determine success or failure. RESULTS: After autotransplantation, testicular development exhibited an atrophy rate of less than 50% and a significant decrease in groups 2 and 3 after 60 days. The sperm density was significantly different between groups 2 and 4 (control). The pH value was significantly different after 2 hours in groups 2 and 3. The tissue lactate concentration revealed no significant differences. The tubular diameters were significantly diminished in group 2 after 60 days. CONCLUSIONS: The smaller atrophy rate of the testes suggests that we successfully established a model in the rat to compare the operative outcome after autotransplantation and the Fowler-Stephens procedure in cryptorchidism. The significant difference in the tubular diameters for the intra-abdominally placed testes might be caused by the exposure of the testis to a higher temperature.
