The medial arm flap.

The reports of transfer of the medial arm flap have been inconsistent, and confusion remains about its anatomy and use. In 40 fresh cadaver arms, the vascular supply to the medial side of the arm was studied after latex injection. The superior ulnar collateral artery was present in 39 of 40 dissections and was the most consistent prominent blood supply to this area. In 57.5% of dissections, direct cutaneous arteries > 1.0 mm were present, and in 47.5%, multiple muscular cutaneous arteries > 1.0 mm were present. Although the blood supply to the medial arm flap is multiple, at least one pedicle > 1.0 mm can be found in all dissections. To date, 10 consecutive medial arm flaps were used to cover upper and lower extremity defects without any flap loss. With knowledge of the anatomical variations, the medial arm flap is a plausible reconstructive option.

The versatile metacarpal and reverse metacarpal artery flaps in hand surgery.

Vascular anatomy of dorsal metacarpal arteries was investigated in fresh cadaver dissections and by injection of methylene blue. Origin and termination of the vessels were studied, as well as their connection to the palmar vasculature. Vascular territories and elevation of flaps based on these vessels are also discussed. The second dorsal metacarpal artery was found to be the most consistent. This vessel has a large distal connection to the palmar vessels which allows both proximal and reverse elevation. Representative cases and surgical technique are described.

Down-regulation of vesicular stomatitis virus transcription by the matrix protein of influenza virus.

The matrix (M1) protein isolated from influenza A/WSN/33 virus, when reconstituted with ribonucleoprotein (RNP) cores of vesicular stomatitis virus (VSV), resulted in inhibition of VSV transcription in vitro. The presence of endogenous wild-type (wt) or mutant (tsO23) VSV matrix (M) protein on RNP cores did not prevent down-regulation of VSV transcription by reconstituted influenza virus M1 protein. In fact, endogenous VSV wt M protein augmented transcription inhibition by M1 protein reconstituted with RNP/M protein cores, whereas mutant tsO23 M protein endogenous to RNP cores had no effect on down-regulation of VSV transcription by M1 protein. These data suggest that VSV M protein and influenza virus M1 protein recognize two different sites on RNP cores responsible for down-regulation of VSV transcription. Monoclonal antibodies (MAbs) directed to epitope 2 of M1 protein had been previously shown to reverse transcription inhibition by M1 protein on influenza virus RNP cores, but the same epitope 2-specific MAb had little effect on transcription inhibition by M1 protein reconstituted with VSV RNP cores. VSV M protein bears a striking resemblance biologically and genetically to the M1 protein, including, as shown here, their capacity to bind viral RNA. However, the VSV wt M protein exhibited no capacity to down-regulate transcription by influenza virus RNP cores. The significance of these studies is the identification on VSV RNP templates of at least two separate sites for recognition of protein factors that repress VSV transcription.

[Study on casting precision of plat castable ceramic crowns].

The plat castable ceramic crown was made with investment material prepared in our college with our own casting technique by a Chinese-made casting machine. The accuracy and fitness of the crown were studied and appraised. Results show that the margin of fitness is 46 microns and that the cement film thickness of the dies in the spacer material group is 65.13 microns. Hence the results are satisfactory. The Plat castable ceramic crown might be used in clinical dentistry.

Case-control study of cigarette smoking and primary hepatoma in an aflatoxin-endemic region of China: a protective effect.

Aflatoxin is believed to be a major causative agent in the high incidence of primary liver cancer seen in certain regions of the world. In Fujian Province, an aflatoxin-endemic region of China, we compared the cigarette smoking habits of 200 primary hepatoma patients with those of 200 matched nonhepatoma controls. We excluded from our study all individuals with evidence of hepatitis B virus serum antigen and/or alcoholic cirrhosis. Interestingly, two groups of hepatoma patients could be discerned. In patients more than 50 years of age, a significantly higher number of cases of primary hepatoma was found among nonsmokers than smokers (odds ratio = 2.06; 95% confidence interval = 1.32-3.20). In patients less than 50 years of age, this difference was not seen. Previous studies in the rat, mouse and duck had suggested that agents present in cigarette smoke might induce a cytochrome P450-mediated detoxication pathway, leading to protection against aflatoxin-induced primary liver cancer. Our clinical data in the present study are therefore consistent with the previous laboratory animal experiments.

Multivariate comparison of the forelimb between stump-tailed and rhesus macaques.

Rhesus macaques and stump-tailed macaques are sympatric in western Yunnan (China), coexisting or occupying habitats that show little difference. This paper tests hypotheses based on theoretical expectation from the differing biomechanical demands of terrestrial and arboreal quadrupedalism in stump-tailed macaques and rhesus macaques, respectively. Individuals of these two macaque taxa were markedly separated by the first two principal components and discriminant analyses based on 18 variables of the upper limb. The rhesus macaques appear to be more adapted for arboreal quadruped habits because of elongation of the clavicle and forearm, a larger humeral head and greater midshaft sagittal diameters of the radius and ulna.

[Spermine inhibition of in vitro fertilizing ability of human spermatozoa and its possible mode of action].

The direct effect of spermine at various concentrations (0.25-8.0 mmol/L) on in vitro fertilizing ability of human spermatozoa was evaluated by the penetration test of zona-free hamster egg. To study the effect of spermine on capacitation, as judged by the rate of penetration, spermatozoa were incubated in BWW with various concentrations of spermine for 6 h at 37 degrees C. The hyperactivated motility of spermatozoa was markedly inhibited by spermine at a concentration of 4.0 mmol/L. The penetration rate was decreased proportionally to the dose of spermine used. Spermatozoa were incubated in BWW with 0.5 mmol/L spermine for 6 h and another 4 h after spermine was washed off with spermine-free BWW. The percentage of penetration was comparable to that of the control. Therefore, spermine-mediated inhibition of capacitation was reversible. Moreover, exogenous dbcAMP (0.5-1.0 mmol/L) or caffeine (10 mmol/L) could antagonize significantly the spermine-induced inhibition of capacitation with a correlation coefficient of 0.990. The content of spermine in fertile men spermatozoa was assayed by HPLC. Before capacitation spermine in spermatozoa was 7.05 micrograms/10(7) cells, whereas after capacitation it was no longer detectable, indicating that spermine may be an inhibitor of in vitro capacitation in human sperm. To study the effect of spermine on capacitated sperm, spermine was added to the BWW medium after sperm had been preincubated in spermine-free BWW. The persistent presence of spermine could interfere with spermatozoa attachment to, binding to and penetration into zona-free hamster eggs, which was related to the concentration (r = 0.820) used.(ABSTRACT TRUNCATED AT 250 WORDS)

Analysis of cutaneous perfusion: an aid to lower extremity reconstruction.

The use of cutaneous and musculocutaneous flaps has been greatly enhanced by a better understanding of vascular anatomy. Cutaneous vessels reach the skin by three basic pathways: over the muscle layer, through the muscles, and between the muscles. The authors' anatomic investigation of each of these three types of vessels and the clinical application of this information is presented.

[Diagnosis of myocardial infarction and its complications with magnetic resonance imaging].

Ten normal volunteers and 25 patients with myocardial infarction were examined by ECG-gated magnetic resonance imaging (MRI). In normal volunteers, the thickness and signal of left ventricular muscle between different septum are not significantly different. As an effective imaging technique through the changes of wall thickness and signal, 24 of 25 patients with myocardial infarction were identified by MRI. At the same time, ventricular aneurysm and thrombosis etc were found.

X-ray microanalysis of Plasmodium falciparum and infected red blood cells: effects of qinghaosu and chloroquine on potassium, sodium, and phosphorus composition.

Cryosections of human red blood cells infected by Plasmodium falciparum were analyzed by energy dispersive x-ray microanalysis to determine the elemental composition of the parasites and their red cell hosts separately. The effects of two antimalarial drugs, qinghaosu and chloroquine, on potassium, sodium, and phosphorus concentrations were studied. Malarial infection causes a decrease in potassium concentration and an increase in sodium concentration in the host red cells. The drastic change in the cation composition, however, occurs only in red cells infected by late stage parasites (late trophozoite and schizont). Red cells infected by early stage parasites (ring stage) show only small changes in sodium concentration. Furthermore, the noninfected red cells in parasitized cultures show no difference in composition from those of normal red cells. Treatment of the parasitized cultures with qinghaosu (10(-6) M) or chloroquine (10(-6) M) for 8 hr causes phosphorus concentration of both early and late parasites to decrease. An 8 hr treatment with qinghaosu also produces a reduction in potassium and an increase in sodium concentrations in early and late parasites. In contrast, 8 hr treatment with chloroquine only causes a change in the sodium and potassium concentrations of the late stage parasites and does not affect the early stage parasites.

Monoclonal antibodies to the matrix protein of vesicular stomatitis virus (New Jersey serotype) and their effects on viral transcription.

Of 33 hybridomas raised by immunization of BALB/c mice with the matrix (M) protein of the New Jersey serotype of vesicular stomatitis virus (VSV), 17 secreted monoclonal antibodies (mAb) of the IgG isotype and, unexpectedly, 16 of the IgM isotype. All these monoclonal antibodies bound strongly to VSV-New Jersey M protein by ELISA, immunoprecipitation, and immunoblotting assays, but exhibited only slight or no cross-reactivity with the M protein of VSV-Indiana. Four antigenic determinants of VSV-New Jersey M protein could be identified by competitive binding of 125I-labeled monoclonal antibodies but three of these epitopes exhibited partial overlap. Monoclonal antibodies to two epitopes reversed the inhibitory effect of M protein on in vitro transcription of VSV-New Jersey ribonucleoprotein. However, monoclonal antibodies to the other two epitopes had little effect on M-protein transcription inhibition but actually increased significantly the transcriptional inhibitory effect of M protein under certain experimental conditions. Monoclonal antibodies to all four epitopes reacted strongly with the M protein of the tsC1 mutant of VSV-New Jersey which is restricted in transcription inhibition.

Upregulation of the phosphorylated form of CREB in spinothalamic tract cells following spinal cord injury: relation to central neuropathic pain.

Spinal cord injury (SCI) often leads to the generation of chronic intractable neuropathic pain. The mechanisms that lead to chronic central neuropathic pain (CNP) following SCI are not well understood, resulting in ineffective treatments for pain relief. Studies have demonstrated persistent hyperexcitability of dorsal horn neurons which may provide a substrate for CNP. We propose a number of similarities between CNP mechanisms and mechanisms that occur in long-term potentiation, in which hippocampal neurons are hyperexcitable. One biochemical similarity may be activation of the transcription factor, cyclic AMP response element-binding protein (CREB), via phosphorylation (pCREB). The current study was designed to examine whether tactile allodynia that develops in segments rostral to SCI (at-level pain) correlates with an increase in CREB phosphorylation in specific neurons known to be involved in allodynia, the spinothalamic tract (STT) cells. This study determined that, in animals experiencing at-level allodynia 35 days after SCI, pCREB was upregulated in the spinal cord segment rostral to the injury. In addition, pCREB was found to be upregulated specifically in STT cells in the rostral segment 35 days after SCI. These findings suggest one mechanism of maintained central neuropathic pain following SCI involves persistent upregulation of pCREB expression within STT cells.
