An analysis for death causes in 45 cases of liver cancer treated with traditional Chinese drugs.

Among the 165 cases of late-stage liver cancer treated in our hospital, 65 (39.4%) died, with an average survival time of 8.1 months and a median survival time of 7 months. Among the 65 dead patients, 45 were treated with traditional Chinese drugs and 20 with western medicine. The average survival time was 8.4 months in the former and 7.3 months in the latter group. The direct causes of death for the 65 patients were hepatic coma, severe hemorrhage of the upper digestive tract, Heyd's syndrome, hepatorrhexis, respiratory failure, cardiac failure, etc. The incidence rates of hemorrhage of the upper digestive tract and hepatorrhexis in the 45 patients treated with traditional Chinese drugs were obviously lower than those treated with western medicine.

[The clinical study on urinary albumin and calcium output in 24 hours to serve as early markers for pregnancy induced hypertension].

OBJECTIVE: To assess the efficacy of urinary calcium and albumin output in 24 hours for prediction of the occurrence of pregnancy induced hypertension (PIH). METHODS: 24-hour urine samples were collected at 24-32 weeks' gestation in 98 health women for the measurement of calcium and albumin, at the same time plasma calcium and creatinine were also determined. All patients were followed up until delivery and were classified subsequently according to the occurrence of PIH. Microalbumin was measured by RIA and calcium by automatic biochemistry analysis. RESULTS: The patients in whom PIH developed later (n = 14) had significant lower urinary calcium excretion [(1.43 +/- 0.37) mmol/24 h VS (3.26 +/- 0.75) mmol/24 h] and higher microalbuminuria [(12.68 +/- 6.81) micrograms/24 h VS (6.08 +/- 3.48) micrograms/24 h] as compared with the group which remained normal (n = 84). CONCLUSIONS: The decrease of calcium and increase of albumin per 24 hours of urine were observed 4-8 weeks before the onset of PIH. The above observation may serve as early markers for PIH.

[Changes in cardiac renin-angiotensin system after severe burn injury in rats].

OBJECTIVE: To observe changes in cardiac renin-angiotensin and its effect on sarcoplasmic reticulum(SR) calcium transport function, and investigate the mechanism underlying cardiac dysfunction in early postburn stage. METHODS: Rats were divided into three groups: control group (n = 8, without burns), burn group (n = 40, 30% TBSA full-thickness burns), and treatment group (n = 40, gavage Lisinopril for 3 days before 30% TBSA burns). The left ventricular(LV) contractile function was measured at 3, 8, 24 hours postburn. Angiotensin converting enzyme(ACE) activities, the contents of angiotensin II (AII) and calcium in myocardium were also measured. In addition, Ca(2+)-ATPase activities and calcium transport function in SR were determined. RESULTS: The maximal rates of LV pressure rise (+/- dp/dtmax) and fall (-dp/dtmax) in burn group were significantly lower than that in control group. The ACE activities, AII levels, as well as calcium contents in myocardium increased markedly in burn group. Ca(2+)-ATPase activities and Ca2+ uptake function in SR decreased significantly in burn group. After treatment with ACE inhibitor-Lisinopril, the ACE activities, AII levels and calcium contents were significantly decreased compared to the burn group. Meanwhile, Ca(2+)-ATPase activities, calcium uptake function and LV contractile function decreased markedly. CONCLUSION: The cardiac renin-angiotensin system is activated rapidly after severe burns and inhibits the calcium transport function which may play an important role in cardiac contractile dysfunction following burns.

[The protective effects of prompt escharectomy on myocardial injury following burn in rats].

OBJECTIVE: To investigate the protective effects of prompt escharectomy on myocardial injury following severe burns. METHODS: Rats were inflicted with 30% TBSA III degree scald. They were randomly divided into 3 groups: control group (C, n = 10), non-escharectomy group (NE, n = 50), escharectomy group (E, n = 50). In the E group, burn wounds were excised promptly after the injury. The levels of troponin T (TnT) and tumor necrosis factor (TNF) in plasma and TNF in myocardium were measured at 1, 3, 6, 12 and 24 hours postburn. RESULTS: TnT levels increased markedly at 3 hours post burn, plasma TNF levels increased markedly at 6 hours postburn, and TNF levels in myocardium also increased markedly at 12 hours postburn compared with the C group. Although the levels of TnT and TNF were a little lower in NE group than those in E group at 3 hours post burn, the levels of TnT 6 hours postburn and levels of TNF 12 hours postburn were significantly higher in NE group than those in E group. There were good positive correlations between TNF with TnT levels. CONCLUSIONS: TNF played important roles in the pathogenesis of myocardial injury following burns, its level was correlated with the severity of myocardial injury. TNF levels in E group was much lower that in NE group, suggesting that prompt escharectomy could reduce the production and release of inflammatory cytokines, and lessen the myocardial injury induced by uncontrolled inflammatory response.

Electrophoresis of creatine kinase isoforms: a highly sensitive fluorescence scanning method.

OBJECTIVE: To develop an agarose electrophoretic method for creatine kinase (CK) isoforms, using highly sensitive fluorescence scanning. METHODS: A discontinuous buffer system was used. Electrophoresis on agarose gel was performed under constant current and low voltage. CK isoforms were separated within 30 minutes and detected by fluorescence scanning. RESULTS: There were no significant differences when the activities of CK-MM were between 853.0 U/L and 14.0 U/L and those of CK-MB between 152.0 U/L and 2.4 U/L. The detection limits of stain method for CK-MM and CK-MB isoforms were 36.0 U/L and 12.3 U/L, respectively; while those of fluorescence method were 12.0 U/L and 2.1 U/L. The experimental results showed good precision for CK-MM isoforms, as well for CK-MB isoforms and isoenzymes. CONCLUSION: An agarose electrophoretic method has been developed to measure CK isoenzymes and isoforms clinically. This method is rapid, simple, sensitive, highly reproducible and inexpensive. It is suitable for general laboratories.

Intercellular adhesion molecule-1 and accumulation of eosinophils in nasal polyp tissue.

OBJECTIVE: To investigate the relationship between intercellular adhesion molecule-1 (ICAM-1) and the accumulation of eosinophils in nasal polyp tissue to better understand the mechanism of airway eosinophilic inflammation. METHODS: The expression of ICAM-1 and its natural ligand, lymphocyte function-associated antigen-1 (LFA-1), in normal nasal mucosa from 6 controls and in nasal polyp tissue from 19 patients with nasal polyposis were determined with immunohistochemistry. With dual immunohistochemistry and May-Griünwald Giemsa stain (MGG), the expression of LFA-1 and infiltrating eosinophils in nasal polyp tissue was observed. RESULTS: The expression of ICAM-1 and LFA-1 was stronger in the nasal polyp tissue than in normal nasal mucosa. There was a positive relationship between the infiltration of eosinophils and the expression of LFA-1 on eosinophils. CONCLUSION: Accumulation of eosinophils in nasal polyp tissue is associated with the counter-effect between adhesion molecules and its ligand on eosinophils.

[In vitro study on the transduction of mutant human dihydrofolate reductase cDNA into human umbilical cord blood CD34+ cells].

OBJECTIVE: To explore whether human umbilical cord blood hematopoietic progenitor cells transduced with mutant dihydrofolate reductase(mDHFR) gene increase resistance to metrotrexate(MTX). METHODS: Cord blood CD34+ cells were enriched with a high-gradient magnetic cell sorting system(MACS), and then transfected with supernatant of retrovirus containing human mDHFR cDNA. Hematopoietic progenitor clonogenic assay was used to evaluate mDHFR-conferred resistance to MTX. RESULTS: The purity of cord blood CD34+ cells was approximately 90% and recovery rate was 71.1%. The transduced and mock-transduced CD34+ cells were cultured in mediums containing 20 nmol/L MTX for 14 days. The percentages of progenitor colonies of transduced and mock-transduced CD34+ cells were higher than that of non-transduced (P < 0.01), obviously. The resistance of the former to MTX is nearly twofold of the latter. CONCLUSION: Transduction of mDHFR gene could confer the resistance of human hematopoietic progenitor cells to MTX toxicity.

Effects of rhidosin on expression of fibroblast growth factor on olfactory bulb in rats and its relation with senile hyposmia.

OBJECTIVES: To determine the expression of fibroblast growth factor (FGF) on olfactory bulb in rats and its relation with senile hyposmia, and to discuss the effects of rhidosin on expression of FGF on olfactory bulb in senile rats and its significance. METHODS: Four groups of rats, i.e., young group, senile group, rhidosin group, and physiological saline control group were used in this study. After the removal of the heads of these rats, olfactory mucosa and olfactory bulbs were immediately fixed with neutral formalin, followed with paraffin-embedding, serial sectioning, immunohistochemical staining and light microscopic observation. RESULTS: The positive expression rate of FGF on olfactory bulb in young group was significantly higher than that in senile group (P < 0.01). The positive expression rate of FGF on olfactory bulb in rhidosin group was significantly higher than that in physiological saline control group (P < 0.01). No positive expression of FGF on olfactory mucosa was observed in these four groups. CONCLUSION: These results suggest that the expression of FGF on olfactory bulb gradually decline with aging. The decrease of expression of FGF on olfactory bulb may play a key role in the occurrence of senile hyposmia. Rhidosin can increase the expression of FGF on olfactory bulbs in senile rats and this may be one of the mechanisms by which rhidosin promotes the growth of olfactory cells and inhibits aging effects on the olfactory system.

Changes in serum cardiac troponin I levels after percutaneous transluminal coronary angioplasty.

OBJECTIVE: To evaluate the possible effect of percutaneous transluminal coronary (PTCA) on myocardium. METHODS: Serum cTnl and CK-MB were measured in 60 patients with coronary artery disease (CAD) who underwent PTCA before and at the 6th, 12th, 24th, 48th and 72nd hour after the interventional procedure respectively. RESULTS: The serum cTnl levels began to increase at 6 hours (9.65 +/- 6.27 micrograms/L) in 18 patients, reached the peak levels during 12-24 hours (20.43 +/- 11.28 micrograms/L, 18.52 +/- 9.52 micrograms/L), and returned to normal range till 48-72 hours (7.35 +/- 7.62 micrograms/L, 5.51 +/- 3.13 micrograms/L) after PTCA. The serum cTnl and CK-MB levels were kept normal range pre- and post-PTCA in 30 cases. The levels of cTnl in 12 cases were over baseline either before or after the procedure, while for CK-MB, only in 3 cases were over normal range after PTCA. Compared with normal cTnl group, elevated cTnl levels were related to total inflation times and dilated times (P < 0.05). CONCLUSIONS: PTCA may cause some minor damage in myocardium, serum cTnl level was more sensitive and specific for monitoring myocardial injury.

[Correlation of eosinophil apoptosis with interleukin 5 mRNA expression in lung tissues of asthmatic guinea pigs].

OBJECTIVE: To explore the correlation of interleukin 5 mRNA expression with eosinophil apoptosis in lung tissues of asthmatic guinea pigs. METHODS: Guinea pigs were divided into asthma, asthma pretreated with dexamethasone and control groups. Guinea pigs were sensitized by exposure to aerosolized ovalbumin. Twenty four hours after the animals were challenged by aerosolized ovalbumin, the apoptosis percentages of hypodense eosinophils(HEo) and normodense eosinophils(NEo) in bronchoalveolar lavage fluid (BALF), and IL-5 mRNA expression in lung tissues were detected with terminal deoxynucleotidyl transferase mediated dUTP nick end labeling (TUNEL) technique and in situ hybridization. RESULTS: (1) In the asthmatic group, the percentages of HEo and NEo apoptosis were significantly decreased (P < 0.05), and IL-5 mRNA expression was significantly increased as compared with the control (P < 0.01). (2) In the group pretreated with dexamethasone, the apoptosis percentages of HEo and NEo were significantly increased (P < 0.01), and IL-5 mRNA expression was reduced as compared with the asthmatic group (P < 0.01). (3) IL-5 mRNA expression was negatively correlated with apoptosis percentage of HEo (r = -0.491, P < 0.05) and NEo (r = -0.492, P < 0.05). CONCLUSIONS: IL-5 mRNA expression in lung tissues closely correlated with apoptosis percentage of Eos in BALF. Inhibiting the expression of IL-5 mRNA and promoting the apoptosis of Eos in lung tissues may be one of the important mechanisms of glucocorticoids to reduce infiltration of Eos in lung tissues and their therapeutic effect in asthma.

[The relationship between hypoxic pulmonary hypertension and changes of HO-1 and its mRNA].

OBJECTIVE: To study the relationship of HO-1 and HPH. METHODS: The expression of HO-1 and HO-1 mRNA were observed in pulmonary vessles of rats using the technique of immunohistochemistry and hybridization in situ. The average value of integral light density of HO-1 in three different diameters of arteries was detected by means of the image analysor and considered as the relative content in pulmonary arteries. RESULTS: Hypoxia induced the expression of HO-1 and HO-1 mRNA and the relative content of HO-1 was significantly increased in pulmonary arteries corresponding with the period of hypoxia. CONCLUSIONS: In the condition of HPH, relative content of HO-1 increase obviously. It suggests that HO-1 plays certain role in development of HPH. The change of HO-1 mRNA expression is the basis of molecular change of HO-1.

[Construction of anti-human VEGF165 chimeric antibodies and expression in eukaryotic cells].

OBJECTIVE: To express chimeric anti-VEGF165 antibody genes in eukaryotic cells. METHODS: The variable region genes of light and heavy chains from mouse anti-human VEGF165 monoclonal antibody VmD11 were cloned into eukaryotic expression vectors and transfected into dihydrofolate reductase-deficient Chinese hamster ovary(CHO-dhfr-) cells to express chimeric antibody. The antibody expressed was examined for the presence of human constant regions and specificity against human VEGF165. RESULTS: The chimeric antibody with human constant regions and specificity against human VEGF was detected in the culture supernatant of transfected CHO cells by ELISA and Western blot. The chimeric antibody gene was also detected at mRNA level by RT-PCR. CONCLUSION: The mouse anti-human VEGF165 monoclonal antibody is successfully humanized and expressed in eukaryotic cells.

[AFP mRNA expression in peripheral blood correlates with metastasis of hepatocellular carcinoma].

OBJECTIVE: To investigate the status of hematogenous spread of hepatocellular carcinoma (HCC) and the significance of peripheral blood AFP mRNA expression as a predictive marker for metastasis of HCC. METHODS: Peripheral blood samples were collected from 54 patients with HCC, 20 of whom had received interventional therapy before samples were collected, and from 30 subjects as control(10 cases with benign liver disease, 20 healthy donors). AFP cDNA was amplified from total RNA extracted from 5 ml whole blood by nested reverse transcription polymerase chain reaction(RT-PCR). RESULTS: Of the 54 HCC patients, 26(48.1%) patients had positive AFP mRNA, while AFP mRNA was detected in only 1 of the 10 patients with benign liver disorder and 1 of the 20 healthy subjects. In 16 HCC patients with intra- and extra-hepatic metastases, 13 (81.3%) were positive for AFP mRNA. In patients who did not yet have metastasis, 34.2% of them (13/38) gave positive AFP mRNA. The presence of AFP mRNA correlated with the stage of HCC and the presence of intrahepatic and/or extrahepatic metastasis, but did not correlate with the tumor size and the serum AFP level. There was no significant difference in AFP mRNA expression before and after surgical treatment or transcatheter arterial chemoembolization. CONCLUSION: AFP mRNA is a valuable marker for detecting hematogenous dissemination and metastasis of HCC. Systemic chemotherapy or immunotherapy is needed to prevent clinically overt metastasis and recurrence.

[Protective effect of mutant dihydrofolate reductase gene on human peripheral blood CD34+ cells against methotrexate].

OBJECTIVE: To examine if human peripheral blood hematopoietic progenitor cells transduced with mutant dihydrofolate reductase(mDHFR) gene may increase their resistance to metrotrexate(MTX). METHODS: After enrichment with a high-gradient magnetic cell sorting system(MACS), the peripheral blood CD34+ cells were transfected with supernatant of retrovirus containing human mDHFR cDNA. Colony formation of hematopoietic progenitor cells was utilized to evaluate mDHFR-conferred resistance to MTX. The number of colonies formed from every 500 cells seeded was counted and expressed as the percentage of colony formation. RESULTS: After the mD-HFR-transduced CD34+ cells had been screened in vitro in medium containing 20 nmol.L-1 MTX for 14 days, the percentage of colony formation of mDHFR cDNA-transduced and of mock-transduced CD34+ cells was 29.4% and 16.3%, respectively (P < 0.01). The resistance to MTX was increased twofold. CONCLUSION: Transduction of mDHFR gene could confer the resistance of human hematopoietic progenitor cells to MTX toxicity.

[Polylactic acid combined with recombinant human bone morphogenetic protein and basic fibroblast growth factor to repair the mandibular defects in rabbits].

OBJECTIVE: To study the effect of basic fibroblast growth factor(bFGF) on bone formation induced by recombinant human bone morphogenetic protein-2(rhBMP-2) in repair of mandibular defects in rabbits. METHODS: 56 adult domestic rabbits were classified into five groups. The mandiblar three-wall defects(1.5 cm x 0.5 cm, full thick) were created in right sides. The defects were implanted with various grafts: PLA/rhBMP-2/bFGF, PLA/rhBMP-2, PLA/bFGF, PLA and no implant as control group. At 2, 4 and 8 weeks after operation, the effectivenesses of defect repaire were observed by histological and roentgenographic analysis. RESULTS: The composite consisting of three materials stimulated more bone formation than of two materials, and the latter was more than single PLA(at 4 weeks postoperation, P < 0.05). All experimental groups, which had one or two growth factors, were significantly higher compared with control group in every stage of postimplantation. CONCLUSION: PLA has good biocompatibility, absorbability and osteoconductibility, rhBMP-2 and bFGF are cooperated in bone healing process, and PLA/rhBMP-2/bFGF is a promising bone substitute in clinical uses.

[Portal hypertensive vasculopathy of splenic artery].

OBJECTIVE: To study the pathological changes of splenic artery wall in portal hypertensive patients. METHODS: The pathological changes and iNOS activity of splenic artery wall were studied in 21 patients with liver cirrhosis and 15 normal subjects, using light and electron microscope and immunohistochemistry. RESULTS: Compared with normal individuals, the splenic artery of cirrhotic patients showed that the intima was thickened, split and broken, and the smooth muscle layer of middle membrane was thicker and migrated to underintima space. In cirrhotic patients, regression, atrophy or hypertrophy and hyperplasia were seen in smooth muscle cells whose contractile phenotype changed to synthetic phenotype as well. iNOS activated obviously. Extracellular matrix increased in the wall, resulting in collagenic and fibrous changes. CONCLUSIONS: Hemodynamic alteration due to liver cirrhosis induced pathological changes of splenic artery, including the damage to contractile structure and increase of iNOS activity. It is one of the reasons of decrease of artery reaction to contractile vasoactive materials.

[Microelectrode-guided posteroventral medial pallidotomy for Parkinson's disease].

OBJECTIVE: To introduce the methods and effects of microelectrode-guided posteroventral pallidotomy (PVP) for Parkinson's disease. METHODS: 109 patients underwent PVP by microelectrode electrophysiological recording for intraoperative target localization. 45 patients underwent unilateral PVP, 21 patients simultaneous bilateral PVP, 43 patients combined thalamotomy and PVP. Modified Webster Scale was used for objective assessments before and after operation. Postoperative CT scan or MRI was performed to localise lesions. RESULTS: Microelectrode recording usually led to a final pallidotomy lesion position that deviated from the CT stereotactically defined target point. The change rate of targets was 84.7%. PVP significantly and immediately improved all Parkinsonian motor signs and reduced drug-induced motor fluctuations and dyskinesia. The mean Webster Scale was improved by (72.7 +/- 11.3)% in the "on"" state, and (89.3 +/- 8.1)% in the "off" state. No patient showed permanent complications. CONCLUSIONS: Microelectrode-guided PVP was proved to be safe and effective. Bilateral PVP and combined PVP can significantly and immediately abolish all Parkinsonian motor signs. Physiological methods of microelectrode recording can significantly improve the safety and efficacy of PVP, and decrease the rate of complication.

[An epidemiological survey and treatment of blindness in Zhangqiu city of Shandong province].

OBJECTIVE: An epidemiological survey of blindness was carried out in Zhangqiu city of Shandong province. METHODS: According to stratified random sampling rule, 11,884 persons were chosen as the sample (the sampling fraction is 1.27%) in the city. The WHO grading system for blindness was followed in the survey. RESULTS: The prevalence of blindness was found to be 0.31%, resulting chiefly from cataract (45.95%), glaucoma (24.32%) and vitreous retinal disease (13.51%). 1 016 cases of blindness caused by cataract were operated for restoration of vision, that were accounted for 73.89% of all cataract blindness estimated from the result of the survey. The rate of intraocular lens implantation in the restorative operations was 84.15% (855/1,016). After operation, the corrected vision in 64.47% of the blindness was >or= 1.0, in 21.26% between 0.6 - 0.9, in 10.24% between 0.3 - 0.5, and in 4.03% between 0.05 - 0.2. CONCLUSION: The epidemiological results show that the main cause of blindness is cataract. So it is the key objective for surgical restoration of vision in the prevention of the blindness in later days.
