[Bestrabacil: a possible target-oriented anticancer agent].

Bestrabucil, the benzoate of an estradiol-chlorambucil conjugate, was initially developed as a target-oriented anticancer agent for breast cancers with positive estrogen receptors, by conjugating a tumoricidal agent, chlorambucil, to a vehicle, estradiol. Further studies, however, revealed that regardless of the presence of estrogen receptors, bestrabucil selectively accumulates in malignant tumor cells. The unique feature of bestrabucil, selective affinity to tumor cells, was demonstrated in in vitro and in vivo studies. Significantly larger amounts of 3H-bestrabucil accumulated in malignant cells (3T3-SV40 transformed) than in normal cells (3T3). In vitro inhibition effects of bestrabucil on cell growth was observed only in malignant cells. Selective accumulation of bestrabucil in malignant tumor tissues was also demonstrated in in vivo experiments. After a single oral administration of 100 mg/kg of bestrabucil to female Wistar rats bearing Walker 256 carcinoma, bestrabucil accumulated significantly occurred in tumor tissues with little or no accumulation in normal tissues and blood. Anti-tumor effects and toxicities of bestrabucil and chlorambucil, were compared using Walker 256 carcinoma. Bestrabucil exerted its antitumor effects with little change in leucocyte counts in the peripheral blood, whereas chlorambucil showed significant side effects. Finally, selective accumulation of bestrabucil in malignant tumor tissues, was demonstrated clinically. Tumor specimens obtained during the operation of patients with various types of cancer, 24 hours after oral administration of 100 mg of bestrabucil, contained significantly larger amounts of bestrabucil compared with adjacent normal tissues. Clinical trials of bestrabucil are being carried out at present. Bestrabucil seems to be a promising target-oriented anticancer agent and deserves further investigation.

Antiviral effects of atropine and caffeine.

The antiviral activities of atropine and caffeine were investigated. Atropine inhibited the multiplication of enveloped viruses and caffeine suppressed th growth of polio, influenza, herpes simplex and vaccinia viruses but not Japanese encephalitis virus, Newcastle disease virus and type 2 adenovirus.

Use of an AR-1 resin column to reduce bilirubin-level in modified ascitic fluid.

Intractable ascites has been accompanied by significant jaundice in some patients. In such cases, a newly developed synthetic resin, AR-1, has been successfully employed to reduce excess bilirubin from the ascitic fluid. This resin has proved to be excellent compared with XAD-2, XAD-7 and activated charcoal, for removal of bilirubin from the plasma and ascitic fluid in our experiments. A column containing 100 ml of AR-1 is inserted in the drip infusion line between the modified ascitic fluid reservoir in the Autoascit device and the patient. This method has been used in 7 cases of intractable ascites associated with advanced gastric, pancreatic, hepatic, rectal and ovarian cancers. The column has sufficient capacity to adsorb excess bilirubin from the modified ascitic fluid. Other biochemical parameters were eseentially unchanged from pre-column values.

Admittance plethysmographic evaluation of undulatory massage for the edematous limb.

A new apparatus, called Hadomer has been developed for the treatment of peripheral lymphedema and venous disorder. It has cuffs with 5 rooms, through which the air pressure moves from periphery to proximal point, just like surging waves. The peripheral stagnant lymph and venous blood are displaced toward the heart by this pneumatic massage. Hadomer has been applied to more than 400 patients with the edematous limbs, with satisfactory results, such as decrease of swelling, pain and induration. These clinical results have been also confirmed by electrical admittance plethysmography which is useful to measure noninvasively the edematous volume and the blood flow. The admittance of the edematous limbs is high and after massaging with Hadomer it approaches normal range. The blood flow in the diseased limbs is less than the healthy one and it is observed better after pneumatically massaging.

Treatment of intractable ascites by continuous reinfusion of the sterilized, cell-free and concentrated ascitic fluid.

The new method for continuous reinfusion of sterilized, cell-free and concentrated ascitic fluid is described, and utilized in 72 patients with intractable ascites in both malignancy as well as liver cirrhosis and is described with satisfactory results. The management by repeated ascites reinfusion of patients with benign massive ascites has been possible. This method is capable of being applied to patients with malignant ascites. Symptomatic relief and prolonged survival time is anticipated. The method described in this study is simple and free of adverse effects.
