Ecology and prevention of a shellfish-associated hepatitis A epidemic in Shanghai, China.

During a shellfish-borne hepatitis A outbreak in Shanghai during the first quarter of 1988, 300,000 cases were reported in two months. Using cell culture and experimental infection of marmosets, hepatitis A virus (HAV) was isolated from clams collected from the market and the sea bed during the epidemic. A dose-response curve correlating the quantity of clams consumed to the attack rate of hepatitis A was well documented. The occurrence of the epidemic was associated with a good harvest of clams in a new area, serious pollution of this area with sewage and importation of the clams in large quantities into Shanghai where most young adults were susceptible. Clams can apparently be decontaminated by using a continuous water flow. In this way, HAV titres can be reduced by 90% in one day and by 99.9% in two weeks. An attenuated live HAV vaccine which has been developed in China has been shown to be safe and immunogenic and may be used for prevention of such epidemics in the future.

The T-cell repertoire in myasthenia gravis involves multiple cholinergic receptor epitopes.

Antibodies against the alpha-subunit of the acetylcholine receptor (AChR) are found in most patients with myasthenia gravis and are considered to contribute to the receptor damage which leads to the characteristic signs and symptoms of the disease. This B-cell response is T-cell driven. Elevated T-cell reactivities to AChR and its alpha-subunit have been described in myasthenia gravis, and AChR alpha-subunit peptide reactive T-cell lines and clones preferentially recognizing certain defined sequence segments have been reported, thereby disclosing the possibility of specific immunotherapy. We have defined the T-cell repertoire to AChR, its alpha-subunit and the synthetic peptide sequences 100-117, 113-130, 143-163, 161-179, 207-225, 221-240, and 235-255 of the alpha-subunit in an immunospot assay which is based on secretion of interferon-gamma (IFN-gamma) by individual memory T cells upon stimulation with specific antigen in short-term cultures. Most patients with myasthenia gravis displayed T-cell reactivities to 1 to 6 different peptides. The mean numbers of T cells recognizing individual peptides varied in the myasthenia gravis patients between 1 per 77,000 and 1 per 167,000 peripheral blood mononuclear cells. None of the seven peptides evaluated could be identified as an immunodominant T-cell epitope, and any of them was found to dominate in individual patients. The numbers of T cells reacting with AChR and recombinant human AChR alpha-subunit were slightly higher (mean numbers 1 per 26,000 and 1 per 50,000 mononuclear cells, respectively). Such cells, as well as AChR alpha-subunit peptide reactive T cells, were also found in patients with other neurological diseases and in healthy subjects, but at lower frequencies and numbers. In myasthenia gravis, the elevated numbers of memory T cells recognizing multiple AChR alpha-subunit peptides may be crucial for the development of the disease, and the IFN-gamma released by such T cells might be important for its perpetuation.

Construction and implementation of QRST-wave simulation model.

Computer simulation study of the electrical activity of the heart is one of the frontier subjects of electrocardiogram (ECG) theory study. A new algorithm for simulating excitation propagation within the heart is presented in this paper. On the basis of this, computer simulation of QRST-wave is completed on an IBM PC microcomputer. All performance of the model is compared favourably with that of the models implemented on main frames or on high-performance work stations abroad.

A serologically confirmed, case-control study, of a large outbreak of hepatitis A in China, associated with consumption of clams.

A matched and serologically confirmed case-control study was carried out to investigate the source of an outbreak of acute hepatitis involving 290,000 cases in the suburbs of Shanghai, in January 1988. A total of 132 patients with acute hepatitis from six different hospitals were chosen as cases and the same number of control patients without hepatitis were matched for gender, age, admission date and area of residence. Serum specimens from both case and control patients were detected for specific anti-hepatitis A (HA) IgM antibody and a questionnaire was used to investigate probable risk factors related to the outbreak. The positive rate of anti-HA IgM was 98.48% in the case group and only 0.76% in the control, indicating that the infection was caused by HA virus. The results revealed that the source and mode of transmission were due to the consumption of contaminated and inadequately cooked clams (Anadara subcrenata lischke). There was a highly positive dose-response relationship between the odds ratio of contracting HA and the quantity or frequency of clam consumption. The odds ratios of acquiring HA from clams were up to 62.4-63.4 by both group stratification and multiple unconditional logistic regression analyses.

Distribution of hantavirus serotypes Hantaan and Seoul causing hemorrhagic fever with renal syndrome and identification by hemagglutination inhibition assay.

An epidemiologic evaluation of patients with hemorrhagic fever with renal syndrome from different locations in the People's Republic of China was conducted to define the prevalence of two Hantavirus serotypes, Seoul (SEO) and Hantaan (HTN). Serum specimens were collected between 5 and 14 days after the onset of illness and were tested for antibodies by both hemagglutination inhibition (HI) and plaque reduction neutralization (PRN). By the HI test, the geometric mean titer (GMT) of antibodies to SEO in the sera from individuals from Kaifeng City of Henan Province was five times higher than that to HTN. In contrast, by the HI test, the sera from individuals from Jiande County of Zhejiang Province had a GMT of antibodies to HTN that was seven times higher than that to SEO. In the sera from individuals from Shanghai, only a twofold difference was observed in HI antibody titers to the two hemagglutinins by the HI test, with that to HTN being higher than that to SEO. By the PRN test, the GMT ratios of antibody between HTN and SEO strains from individuals in Kaifeng, Jiande, and Shanghai were found to be 1:13, 14:1, and 2:1 respectively. A close correlation (r = 0.8219) and concordance rate (78.3%) were observed between the PRN and HI tests for the identification of the serotypes of individual cases of hemorrhagic fever with renal syndrome. The hantavirus serotypes from individuals in Kaifeng and Jiande were identified as predominantly SEO and HTN, respectively, and those from individuals in Shanghai had an indeterminant serotype defined by these two techniques. The HI test appears to be a simple and reliable way of determining the predominant hantavirus that causes HFRS in a given geographic area.

Comparison of the therapeutic efficacy and side effects of a single dose of levo-praziquantel with mixed isomer praziquantel in 278 cases of schistosomiasis japonica.

A randomized double-blind study comparing the therapeutic efficacy of single dose of levo-praziquantel and mixed isomer praziquantel was carried out on 139 matched pairs of patients with schistosomiasis japonica. A single dose of either levo-praziquantel (20 mg/kg) or praziquantel (40 mg/kg) was given to each patient. Four and six months after treatment, the stool ova negative conversion rates were 94.85% and 96.27% for the levo-praziquantel group, and 97.06% and 94.03% for the praziquantel group, respectively; there was no statistically significant difference between the two treatments (P greater than 0.05). For lightly and moderately infected patients, a single 20 mg/kg dose of levo-praziquantel was as efficacious as 40 mg/kg of praziquantel. Moreover, levo-praziquantel produced fewer side effects than praziquantel. These results suggest that levo-praziquantel is the component of the mixed isomer preparation that is antihelminthic. Levo-praziquantel could be used therapeutically at half the current dose of the mixed isomer drug.

A serological study of hepatitis C infection in plasmapheresis donors.

An epidemic of parenterally transmitted non-A, non-B hepatitis (PT-NANBH) occurred in plasmapheresis donors in Guan County, Hebei Province, China, in 1985. PT-NANBH was diagnosed by epidemiological studies and serological exclusion of HAV, HBV, CMV and EBV infections. Recently, 163 sera samples of 108 patients with PT-NANBH and 65 sera samples of 49 cases with elevated alanine aminotransferase (ALT) levels collected during the epidemic were tested by anti-HCV EIA (Chiron C100). The positive rates of anti-HCV in these two groups were 89.8% (97/108) and 93.9% (46/49), averaging 90.8%. The figures increased with the course of illness and persistance of ALT elevation, i.e., 17.6% and 55.6% within 1 month, 88.9% and 87.5% at 6 months and 100% and 100% after 2 years. Five patients with PT-NANBH and 1 with elevated ALT levels were followed up for 3 to 4 years. We demonstrated that anti-HCV remained positive after the disease had resolved and ALT levels had normalized.

Environmental determinants of lung cancer in Shenyang, China.

To investigate determinants of the high rates of lung cancer in Shenyang, an industrial city in north-eastern China, a case-control study was conducted. Interviews with 1249 lung cancer patients and 1345 population-based controls revealed that cigarette smoking was the main cause of lung cancer. Smoking accounted for 55% of the lung tumours in men and 37% in women. In addition, air pollution from coal-burning heating and cooking devices was significantly linked to lung cancer, with risks rising in proportion to duration of exposure to indoor pollutants. Measurement of benzo[a]pyrene revealed average wintertime levels in air that were nearly 60 times the recommended upper limit for US cities, with even higher concentrations indoors in traditional single-storey homes using coal-burning kang (stoves). Occupational factors were also involved, the risk being elevated by three fold among smelter workers. Soil levels of arsenic and other metals rose with increasing proximity to the Shenyang copper smelter, and elevated risks of lung cancer were found among men, but not women, living within 1 km of its central stacks. Prior nonmalignant lung disease was common and was reported more often among the lung cancer patients than among controls. The findings suggest that cigarette smoking and environmental pollutants combine to account for most of the excess risk of lung cancer in this population.

Survey on use of antimicrobial agents and bacterial resistance in Huashan Hospital.

A cross-section study was carried out to assess the general patterns in use of antimicrobial agents and the trends of bacterial resistance in Huashan Hospital. Of 2,400 patients whose charts were reviewed, 61% were given such drugs. 3,596 antibiotic courses were prescribed. Gentamicin was most frequently used. Results of the susceptibility test of 320 bacterial strains showed a high percentage of resistance against gentamicin, ampicillin, and chloramphenicol. Our findings suggest that antibiotic policies in the hospital need reappraising.

Comparative efficacy of levo-praziquantel and praziquantel in treatment of schistosomiasis japonica at a single dose.

The result of a randomized double blind comparison of therapeutic efficacy of single doses of levo-praziquantel (L-PZQ) and praziquantel (PZQ) in the treatment of 139 matched pairs of proved cases of schistosomiasis japonica was reported, 268 were chronic early cases and 10 were late cases. The dosage of L-PQZ was 20 mg/kg, and that of PZQ was 40 mg/kg. Four and 6 months after treatment the stool-ova negative conversion rates were 94.8% and 96.3% for the L-PZQ group, and 97.1% and 94.0% for the PZQ group respectively (P greater than 0.05).

Indoor radon and lung cancer in China.

Radon has long been known to contribute to risk of lung cancer, especially in undergound miners who are exposed to large amounts of the carcinogen. Recently, however, lower amounts of radon present in living areas have been suggested as an important cause of lung cancer. In an effort to clarify the relationship of low amounts of radon with lung cancer risk, we placed alpha-track radon detectors in the homes of 308 women with newly diagnosed lung cancer and 356 randomly selected female control subjects of similar age. Measurements were taken after 1 year. All study participants were part of the general population of Shenyang, People's Republic of China, an industrial city in the northeast part of the country that has one of the world's highest rates of lung cancer in women. The median time of residence in the homes was 24 years. The median household radon level was 2.3 pCi/L of air; 20% of the levels were greater than 4 pCi/L. Radon levels tended to be higher in single-story houses or on the first floor of multiple-story dwellings, and they were also higher in houses with increased levels of indoor air pollution from coal-burning stoves. However, the levels were not higher in homes of women who developed lung cancer than in homes of controls, nor did lung cancer risk increase with increasing radon level. No association between radon and lung cancer was observed regardless of cigarette-smoking status, except for a nonsignificant trend among heavy smokers. No positive associations of lung cancer cell type with radon were observed, except for a nonsignificant excess risk of small cell cancers among the more heavily exposed residents. Our data suggest that projections from surveys of miners exposed to high radon levels may have overestimated the overall risks of lung cancer associated with levels typically seen in homes in this Chinese city. However, further studies in other population groups are needed to clarify the carcinogenic potential of indoor radon.

Hantavirus strains isolated from rodentia and insectivora in rural China differentiated by polymerase chain reaction assay.

Polymerase chain reaction (PCR) assay and other techniques were applied to differentiate Hantavirus strains isolated from different animal hosts and geographic regions in China. Two groups of related strains, Hantaan and Seoul, have been classified by cross-neutralization, radioimmunoprecipitation (RIP), and sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) assays. The molecular weights of glycoprotein 1 (G1) of Hantaan and Seoul viruses were 72k and 80k, whereas those of the nucleocapsid (N) and glycoprotein 2 (G2) remained the same, respectively. The PCR assay was used to differentiate these isolates using synthetic oligonucleotide primers selected from various regions of the M genome of 76118 and R22 strains. 76118-specific primers amplified only the RNAs extracted from Hantaan strains while R22-specific primers, the RNAs from Seoul strains. The PCR results for classification are consistent with those obtained by cross-neutralization, RIP and SDS-PAGE assays.

Lung cancer among women in north-east China.

A case-control study of lung cancer involving interviews with 965 female patients and 959 controls in Shenyang and Harbin, two industrial cities which have among the highest rates of lung cancer in China, revealed that cigarette smoking is the main causal factor and accounted for about 35% of the tumours among women. Although the amount smoked was low (the cases averaged eight cigarettes per day), the percentage of smokers among women over age 50 in these cities was nearly double the national average. Air pollution from coal burning stoves was implicated, as risks of lung cancer increased in proportion to years of exposure to 'Kang' and other heating devices indigenous to the region. In addition, the number of meals cooked by deep frying and the frequency of smokiness during cooking were associated with risk of lung cancer. More cases than controls reported workplace exposures to coal dust and to smoke from burning fuel. Elevated risks were observed for smelter workers and decreased risks for textile workers. Prior chronic bronchitis/emphysema, pneumonia, and recent tuberculosis contributed significantly to lung cancer risk, as did a history of tuberculosis and lung cancer in family members. Higher intake of carotene-rich vegetables was not protective against lung cancer in this population. The findings were qualitatively similar across the major cell types of lung cancer, except that the associations with smoking and previous lung diseases were stronger for squamous/oat cell cancers than for adenocarcinoma of the lung.

[Morphological observation of human LAK cell killing activity on lung adenocarcinoma cells].

LAK activity could be induced by IL-2 from human PBL. After co-culture of LAK cells with Anip973 human lung adenocarcinoma cells, LAK-tumor conjugates were microscopically observed. Formation of the conjugates paralleled well with the expression of LAK activity (P less than 0.001), suggesting that LAK cells' killing of Anip973 cells depended on their close contact. Under electron microscope, LAK cells had various shapes and certain degrees of motility. Ten minutes after co-culture, close binding between microvilli of LAK cells and tumor cells could be seen. Four hours after co-culture, Anip973 cells bound with LAK cells developed numerous membrane blebs with cytoplasm and nucleus very condensed, which was followed by destruction of cell structure and appearance of cell debris. These features are consistent with apoptosis rather than a cytolytic mechanism of cell death. These findings suggest that LAK cells share certain common features with CTL and NK cells on killing tumor cells.

[Ultrastructural studies on effects of colchicine in treating hepatic fibrosis of schistosomiasis rabbits].

8 rabbits were infected percutaneously with the cercariae of Schistosoma japonicum. After hepatic fibrosis had developed in these rabbits 4 months after infection, 4 out of the 8 infected rabbits were given colchicine orally at a dosage of 40 micrograms/kg per day for 7 weeks. Another uninfected rabbits were used as controls. The therapeutic effects of colchicine on hepatic fibrosis were studied by transmission electron microscopy and morphometry, in which the area of collagen microfibrils in the space of disse and liver cells was measured. The results showed that colchicine relieved ultrastructural injury of liver cells and reduced the number of active fibroblasts and collagen microfibrils. The area of collagen microfibrils measured in the liver of the infected rabbits and colchicine-treated rabbits accounted for 42.5% and 0.2%, respectively (P less than 0.01), suggesting that colchicine has therapeutic effect against schistosomal liver fibrosis.

[Experimental study and case control study of nosocomial infection caused by Pseudomonas aeruginosa].

144 strains of Pseudomonas aeruginosa (Ps.a) were serotyped and phage-typed. Antibiotic sensitivity test and case control study of nosocomial infection caused by Ps.a were also done for these strains. Three epidemic strains were collected from the ICU in a neurosurgical ward. The etiology of a cross infection among tracheotomy patients was two epidemic strains isolated from the hands of a nurse and an attendant. Hospitalization days longer than 56 days, tracheotomy and indwelling catheterization were 3 risk factors for Ps.a infection shown by logistic analysis. The sensitivity rate of Ps.a to antibiotics was highest with ceftazidine, followed by amikacin and piperacillin. The most common resistant antibiogram was Gentamycin and tobramycin Resistance to gentamicin increased obviously.

[Effect of praziquantel treatment on the levels of circulating SJ70 antigen in murine schistosomiasis japonica].

A circulating schistosome 70kDa antigen (SJ 70) has been detected in sera of mice infected with Schistosoma japonicum. using a double antibody sandwich enzyme-linked immunoabsorbent assay (ELISA). SJ 70 antigen was first detected in the sera of S. japonicum-infected mice at the 4th week after infection and increased rapidly, reaching their peak by the 7th week after infection, and remaining relatively unchanged for additional 9 weeks. In contrast, in mice infected with S. japonicum for 7 weeks and then treated with praziquantel (100 mg/kg body weight) there was a significant decrease in serum antigen levels within two weeks after treatment. An almost complete disappearance of the antigen from the sera occurred by the 5th week after treatment. The results indicated that the detection of SJ 70 antigen might be used as early diagnosis of schistosomiasis and evaluation of drug efficacy.
