Mechanism of overproduction of plasma prostacyclin in portal hypertensive rats.

OBJECTIVE: To evaluate the role of increased portal pressure and portosystemic shunting in elevated level of prostacyclin (PGI2) in portal hypertension. METHODS: Thirty-six male Sprague-Dawley rats were divided into four groups: prehepatic portal hypertension (PHPH, 8 rats), intrahepatic portal hypertension (IHPH, 9), end-to-side portacaval shunt (PCS, 8), and sham-operated controls (SO, 11). Two weeks after surgery, free portal pressure (FPP) was measured; systemic and splanchnic hemodynamics was studied by radioactive microsphere technique and blood sample from the femoral artery was obtained to measure the level of plasma 6-keto-PGF1 alpha with radioimmunoassay. RESULTS: The FPP (mmHg) in IHPH, PHPH, PCS and SO rats was 13.10 +/- 1.02, 12.10 +/- 1.52, 3.0 +/- 0.82 and 6.86 +/- 0.69, respectively. The value of FPP was significantly increased in IHPH, PHPH rats and significantly decreased in PCS rats when compared to SO rats. Cardiac index (CI) and portal venous inflow (PVI) were in the order of PCS > PHPH > IHPH > SO rats. Portosystemic shunting (PSS) in PCS, PHPH, IHPH was 99.7 +/- 0.29%, 76.02 +/- 20.62% and 30.34 +/- 10.18%, respectively. The concentrations of plasma 6-keto-PGF1 alpha (ng/ml) in PHPH, IHPH, PCS and SO rats were 6.93 +/- 2.43, 5.09 +/- 2.27, 2.36 +/- 1.01 and 1.56 +/- 0.61, respectively. The concentrations of plasma PGI2 in PHPH, IHPH and PCS rats were significantly higher than those in SO rats. Furthermore, the concentrations of plasma PGI2 in PHPH and IHPH rats were also significantly higher than those in PCS rats. Moreover, a closed positive correlation existed between plasma PGI2 and FPP (r = 0.67, P < 0.001). CONCLUSIONS: The results of the present study suggest that the elevated PGI2 in portal hypertension is mainly due to the overproduction of PGI2 in vascular epithelium cells induced by increased portal pressure, whereas portosystemic shunting and liver dysfunction play a secondary role. In addition, the results of this study do not support that PGI2 mediated the hyperhemodynamics in portal hypertension.

[Periodontal regeneration enhanced by BMP and GTR technique].

OBJECTIVE: To evaluate the effects of BMP on the repair of animal periodontal defects by using GTR technique. METHODS: 30 experimental mandibular buccal alveolar periodontal defects were produced surgically around premolar and molar teeth in 8 young mongrel dogs. The exposed root surfaces were curetted to remove the periodontal ligament and the cementum. The defects were devided into 3 equal groups (collagen, collagen + BMP, control). The results were examined histologically 1 month and 3 months respectively. RESULTS: In experimental groups the JE downgrowth was inhibital in the early healing, and the new attachment formation was promoted while comparing with the controls. Compound membrane (with BMP) showed the capacity to support more new bone and cementum formation than the other two groups. CONCLUSION: It is suggested that exogenous BMP can be used in GTR technique to promote periodontal regeneration.

[Timing of surgical treatment for gallstone pancreatitis].

OBJECTIVE: To evaluate the timing of surgery in the treatment of gallstone pancreatitis (GP). METHOD: The records of 53 patients with GP were retrospectively analysed. RESULT: The morbidity and mortality of early surgical treatment (less than 48 hours after admission) were 29.20% and 9.30%, respectively, in contrast to 3.50% and 0 in patients who underwent delayed surgical treatment (more than 48 hours after admission) (P < 0.05). In mild gallstone pancreatitis (APACHE-II 8), the timing of surgery was related to the morbidity and mortality, namely, they were higher in early surgery than in delayed surgery. CONCLUSION: We suggest delayed surgical treatment for mild gallstone pancreatitis, and delayed and selective surgery for severe gallstone pancreatitis.

[The role of nitric oxide in hyperdynamic circulation in portal hypertensive rats].

OBJECTIVE: To evaluate the role of nitric oxide (NO) in hyper-hemodynamics in portal hypertension, we investigated the effects of nitric oxide synthase inhibitor L-NMMA with or without L-arginine, the substrate for enzyme NO synthase on the systemic and splanchnic hemodynamics in portal hypertensive rats. METHOD: Ninety-seven male Sprague-Dawley rats were divided into four groups: intrahepatic portal hypertension (IHPH, n = 23) by injection of CCl4, prehepatic portal hypertension (PHPH, n = 26) by stenosis of the portal vein, end-to-side portacaval shunt (PCS, n = 23), and sham-operated controls (SO, n = 25). Animals of each group were divided into three subgroups: systemic administration of L-NMMA (50 mg/kg BW, 1 ml), L-NMMA (50 mg/kg BW, 0.5 ml) plus L-arginine (30 mg/kg BW, 0.5 ml), and normal saline solution (1 ml). The radioactive microsphere method was used for hemodynamic study. RESULT: There was the characteristic of hyperdynamic circulatory state including increased cardiac output and splanchnic blood flow, decreased mean arterial blood pressure, total peripheral vascular resistance and splanchnic vascular resistance in IHPH, PHPH and PCS rats. The hyperdynamic circulatory state in IHPH, PHPH and PCS rats could be effectively reversed by L-NMMA to the resting values of hemodynamics in SO rats, but not the levels after administration of L-NMMA in SO rats, indicating that there is excessive production of NO in portal hypertensive and PCS rats. Administration of L-arginine counterbalanced the effects of L-NMMA on hyperdynamic circulatory state in portal hypertensive and PCS rats. CONCLUSION: NO is an important mediator of hyperdynamic circulation in portal hypertension.

[Inhibitor of plasminogen activator 1 (PAI-1) in hepatocellular carcinoma].

OBJECTIVE: To study the expression of the type-1 inhibitor of plasminogen activator (PAI-1) on protein and mRNA in hepatocellular carcinoma (HCC), and the relation between PAI-1 and biological behaviour of HCC. METHOD: The tumor specimens of HCC (n = 48) and benign liver tumor (contorl n = 12) were tested by immunohistochemistry for the expression of PAI-1 protein. The tumor specimens of HCC (n = 20) and normal liver tissue (contorl n = 5) were evaluated by Northern blot analysis for the expression of PAI-1 mRNA. RESULT: PAI-1 protein and PAI-1 mRNA were higher in tumor cell than in the marginal tissue of tumor and in control (P < 0.01 and P < 0.05). In deaths 2 years after surgery, PAI-1 was increased as compared to the survival group (P < 0.05). When negative PAI-1 conupared with positive urokinase-type plasminogen activator (uPA) and positive uPA receptor (uPAR). The nambeigs invasion case increased, the difference was more significant than that in the group with negative PAI-1, uPA and uPAR (P = 0.039). CONCLUSION: The expression of PAI-1 protein and PAI-1 mRNA is increased in HCC. PAI-1 contributes to the invasion, metastasis and prognosis of HCC.

[Retrograde partial hepatectomy: report of 11 patients].

OBJECTIVE: To study a new approach to hepatectomy for liver tumors resected difficulty by typical method. METHOD: The operative order is retrograde as compared with typical hepatectomy, namely, transection of the liver is performed first, isolating adhesions between the tumor and the diaphragm or partial invaded phrenectomy is performed second, and then after cutting corresponding ligaments, liver tumor is removed. If the surrounding organs were invaded or adhered by tumor too tightly to be separated. They were resected with the tumor. RESULT: The approach was used in 11 patients with malignant tumors of liver from June 1994 to January 1996. Operation time was 192 min (150 - 250 min) and estimated blood loss during operation was 1460 ml (800 - 4, 200 ml). No operative mortality was found and all of the patients recovered uneventfully. CONCLUSION: The retrograde partial hepatectomy is a good method for resection of liver tumors resected difficulty by typical hepatectomy.

[Comparison of nuclear accumulation of p53 protein with mutations in the p53 gene on the tissues of human breast cancer].

OBJECTIVE: To compare nuclear accumulation of p53 protein with mutations in the p53 gene on the tissues of human breast cancer. METHOD: Fifty-four invasive ductal carcinomas of breast were analyzed by polymerase chain reaction-single strand conformational polymorphism (PCR-SSCP) silver stain and strep-avidin-biotin-peroxidase complex (SABC) immunohistochemistry. RESULT: A highly significant association between the presence of p53 gene mutation and nuclear accumulation of p53 protein was found (r = 0.714, P < 0.01). All 22 tumors with p53 gene mutations showed nuclear accumulation of p53 protein. Of the 32 tumors with gene mutations undetected, 9(28%) showed nuclear accumulation of p53 protein. Both p53 mutation protein and p53 gene mutations were prevalent in steroid and progesterone receptors negative tumors (P < 0.05). There was a statistically significant association between the nuclear accumulation of p53 protein and lymph node invasion (P < 0.05). So did p53 gene mutations (P < 0.05). Therefore, p53 abnormalities might be associated with an aggressive phenotype in breast cancer. CONCLUSION: The immunohistochemical detection of nuclear p53 protein accumulation is highly associated with p53 gene mutations in breast cancer tissues. This method is useful for rapid screening of p53 abnormalities. In some cases of slightly positive for p53 nuclear protein we must observe p53 genemutations to avoid the false positive reaction.

[Nontypical limited resection of the lung].

OBJECTIVE: To evaluate the role of limited resection in the treatment of lung disease. METHOD: From 1989 to 1996, 46 limited resections were performed on 35 patients with lung diseases. All patients were followed up for 0.5 to 7 years (mean 29.3 months). RESULT: There were no operative deaths and operative complications. In 16 patients with benign lung diseases, no recurrences and late complications were found. In 5 patients with primary lung cancers, 2 died 1.5 and 2 years after operation, respectively, and 3 are still living well for 2.5, 4 and 5 years postoperatively. In 14 patients with metastatic lung diseases (10 with bilateral multiple metastases), 216 metastatic lesions were excised. 1-, 3-, and 5-year survival rates were 78.5%, 50% and 21.4% respectively. CONCLUSION: Limited lung resection could be an additional option and a surgical alternative to conventional lung resection for some compromised patients.

[The effects of different instrumentation on scoliosis].

OBJECTIVE: To investigate the effectiveness of the treatment of scoliosis with different surgical methods. METHOD: 125 cases of scoliosis less than 100 degrees treated during 14 years (1984 - 1997) were analyzed. Different operative methods were compared for their advantages and disadvantages, i.e. results and complications 10 months after operation and follow-up. RESULT: Harrington instrumentation was less effective in scoliotic curve correction and had more complications than other procedures. Luque technique needed longer operation time for wire passage at each level and had risk of neurological damage. CD and Zielke method showed 10% - 20% curve correction. No dislodgment of hook and fractured rods was found in the CD group. Anterior release plus posterior instrumentation was superior to other techniques. CONCLUSION: Treatment of scoliosis with anterior release plus posterior CD is practical for children before a new technique without spinal fusion is developed.

[The determination of neuron-specific enolase of serum in the diagnosis and supervision of retinoblastoma].

OBJECTIVE: To investigate the value of determining quantity of neuron-specific enolase (NSE) in the diagnosis and supervision in patients with retinoblastoma (Rb). METHOD: We used rapid electrophoretic and the fluorometer method in the 35 serum specimens from 35 patients with Rb during surgery, after enucleation, respectively, at 3, 6, 12, >or= 18 months and 31 serum specimens from 31 control cases to determine the quantity of NSE and enolase isoenzymes. RESULTS: There were 207 +/- 65.9 u/L of all isoenzymes and 98.4 +/- 40.3 u/L of NSE from 35 patients with Rb. In comparison with the levels of control cases (respectively, 140.7 +/- 46.0 U//L of all isoenzymes, 41 +/- 10.9 u/L of NSE), both serum levels in patients with Rb increased obviously, the difference being significant. At 6, 12, and >or= 18 months after surgery, the determinations of all isoenzymes and NSE from 35 patients with Rb decreased step by step, the difference being significant in comparison with the values before the surgery. After surgery >or= 18 months, the levels of NSE and all enolase decreased to that of NSE and all enolase from control cases. CONCLUSION: The determination of enolase level, especially NSE of serum, is valuable for the diagnosis of patients with Rb.

[Cytotoxic effect of mitomycin C on the nonpigmented epithelium of ciliary body in rabbit eyes].

OBJECTIVE: To observe pathomorphological changes of nonpigmented epithelial cells (NECs) of ciliary body after local single application of mitomycin C (MMC) during sclerectomy in rabbit eyes, and to explore cytotoxic effect of MMC on ciliary body. METHODS: A single five minutes intraoperative exposure to the agent MMC (0.2 mg/ml, 0.1 ml) was used during sclerectomy in rabbit eyes. The treated area was rinsed with 20 ml normal saline. The NECs of ciliary body at the operated area were observed under light microscope and transmission electron microscope on the 7th and 28th day postoperatively. RESULTS: On the 7th day postoperatively, the NECs were swollen, and the intracellular abundant mitochondria were swollen and vacuolized. String pearls-like membrane-encapsulated vacuoles were present in the intervals of beta-cell membrane of the NECs. On the 28th day postoperatively, these damages were gradually recovered. CONCLUSIONS: These findings suggest a toxic effect of MMC on the NECs of ciliary body, possibly that can decrease the aqueous production and intraocular pressure, even causing persistent hypotony.

[Light sensitivity in central visual field].

OBJECTIVE: To examine the difference in the light sensitivity in the central visual field between the normal subjects and glaucoma patients with or without the early visual field damages and propose the criteria to identify the possible early disturbance in the visual field. METHODS: The light sensitivity at the 76 locations in the central field was measured in 108 normal eyes of 75 normal volunteers aged from 20 to 78 years and 82 eyes of 53 patients with early open-angle or angle-closure glaucoma aged from 30 to 70 years on the Humphrey Field Analyzer (Model 610) by using the Central-30-2 program. RESULTS: No sex and eye differences in the light sensitivity were found between the two groups. The significant difference in the light sensitivity existed between the age of 20 - 45 years and the age of 46 - 78 years in the normal subjects. The lower limit of 80% normal range for each location in the central field of the two age groups was calculated. The light sensitivity decreased by 2 - 3 dB at every 10 degree from the eccentricity. The light sensitivity in the inferior field was 1 - 2 dB higher than that in the superior. There was no statistical difference between the nasal and the temporal field. For the patients with the field defects the light sensitivity of many locations decreased by 4 - 8 dB, and by 1 or 2 dB in the patients without the field defects as compared with the normal control. CONCLUSIONS: Based on the comparison with the normal control and the lower limit of 80% normal range, we propose the following criteria for classification of damages in the glaucoma visual field. Grade I: light sensitivity decreases by 1 - 3 dB at a cluster of locations in the field; Grade II: light sensitivity reduces by 4 - 6 dB at a cluster of locations in the field; Grade III: light sensitivity reduces by more than 6 dB at a cluster of locations in the filed.

[The diagnostic significance of "contraction bands" of striated muscle in forensic pathology].

The contraction bands of cardiac and skeletal muscle probes were studied. They were taken from autopsy cases with different death causes. The morphologic characteristics of the contraction bands that were closely related to diagnosis were carefully observed. It was found that the morphological patterns of the contraction bands were various and some were regularly seen for the death with certain causes. The forensic significance of the contraction bands was then discussed and it was suggested that the morphology of contraction bands of the striated muscle correlate to the type and structure of tissues as well as to the property of injuries.
