[Primary multiple cancer of both lungs].

The study summarizes the experience gained in the treatment of 46 cases of bilateral primary multiple cancer of the lung (synchronous cancer--17 and metachronous--29). Criteria for establishing primary multiple cancer and choice of treatment procedure are suggested. Surgery for bilateral cancer was carried out in 15 cases (metachronous tumor--11; synchronous--4). Radiation treatment was given to 8 patients (synchronous tumor--4, metachronous--4) and chemotherapy--to 7 patients (synchronous tumor--2, metachronous--5). The advantages offered by active surgical intervention in management of primary multiple bilateral cancer of the lung are discussed.

[Lentiginous melanocytic dysplasia as a precursor of malignant melanoma].

Precursors of malignant melanoma are special pigment spots characterized by proliferation of atypical melanocytes accompanied by reactive lymphoplasmocytic infiltration of the derma designated as lentiginous melanocytic dysplasia (LMD), a variant of Dubreuilh melanosis. The material of the study of 53 cases of LMD and the time course of its progress suggest LMD to be not a distinct nosological entity but the beginning of the development of malignant melanoma.

[Early and "latent" breast cancer].

The results of microscopic study of the material obtained from 71 cases of clinically-occult cancer of the breast are reported. Generally, metastases into axillary lymph nodes are primary clinical manifestations. A histological description of dysplasia and preinvasive cancer of the breast is presented. Metastatic spreading in regional lymph nodes unaccompanied by microscopically-detectable invasive tumor growth is discussed.

[Early thyroid gland cancer].

Among 338 patients operated upon for "nodes" in the thyroid in 31.4% of cases there was found small cancer (up to 1 cm in diameter), determined microscopically and visually. A clinico-morphological definition of early thyroid cancer is suggested. It is emphasized that for the recognition of early thyroid cancer being successful a complex diagnostic method is essential, which comprises clinical, roentgenological, thyroidolymphographic, radioisotope, cytological and histological studies. The method concerned makes it possible to increase considerably (up to 77.4%) the rate of recognition of early thyroid cancer, while the operations, performed after the extrafascial technic, would provide for an adequate therapy in due course and a favourable prognosis.

[Clinical aspects and treatment results in stage T1N0M0 lung cancer].

Late results of surgical treatment for lung cancer in stage TINOMO have been studied. A 5-year survival was 83.3%, 10-year survival--67%. Primary multiplicity of malignant tumors was revealed in 16% of observations. Peripheral cancer has more frequently arisen against the background of scars, central- on the background of proliferation and metaplasia of bronchial epithelium. Roentgenological methods for recognizing small size tumors proved to be effective in peripheral cancer, repeat cytological assays of the sputum--in central one. Lobectomy is considered to be the operation of choice in this stage of cancer. The diagnosis of cancer established in stage T1N0M0 should be considered as being timely, but it requires further improvement in the organization of active recognition of such patients.

[Morphogenesis of dysplasia and lobular cancer of the breast].

During the recent ten-year period lobular cancer of the mammary gland arrests special interest. It is characterized by a specific clinical course, frequent bilateral involvement of the glands, multicentric and manifest invasive growth without any noticeable signs of destruction of the pre-existing glandular tissue. This cancer is preceded by carcinoma in situ. In the paper, the necessity is substantiated to differentiate this kind of cancer as a special nosological unit, its peculiar structure being described. Besides the classical form of the growth from small homologous cells like chains, also there was found a special form of the growth as alveolar structures consisting of light pagetoid cells. The initial stages of lobular cancer growth are described both against the background of carcinoma in situ and avoiding it from dysplasia. Attention is given to a tendency of this cancer to mucicarminophilia, a frequent association with mucous cancer, the data speaking in favour of its myoepithelial origin are reported. It is emphasized that carcinoma in situ may give rise both to lobular (myoepithelial origin) and other forms of cancer.

[Lobular carcinoma in situ and lobular cancer (lobular carcinoma) of the breast].

The histological characterization of mammary cancer (MC) is of leading importance for prognosis and choice of therapeutic methods (directed hormonal and chemotherapy). In this respect of interest is the morphological picture of a special form of MC, lobular cancer which is characterized by the most malignant course, marked infiltrative and less manifest destructive growth, multicentricity and multiplicity, as well as bilateral foci of lesions. It probably originates from the myoepithelium. In the national literature, no information on lobular MC is available. Little information is available on the precursor of lobular MC, lobular carcinoma in situ. This problem is insufficiently and inaccurately dealth with in the International classification of mammary gland tumors (WHO). A review of literature on lobular MC, as well as photomicrographs of its structure and that of its precursor are presented.

[Spontaneous regression of malignant melanoma].

Malignant melanomas are known to be prone to undergo spontaneous regression (spontaneous resorption). The morphological picture of this process similar to that in spontaneous regressions of nevuses (halo-nevus), pigmentous spots (Dubreille melanosis) and superficially spreading melanoma is described. Six observations of spontaneously regressing melanomas are presented. Resorption of pigmented malignant tumors is similar to radiation resorption of malignant melanomas. It is noted that spontaneous resorption of malignant melanomas does not improve their prognosis.
