[Influence of cryotherapy on the concentration of malondialdehyde in rats serum].

Malondialdehyde (MDA) consistency in rats serum was measured when performing cryotherapy by means of fluorescence method. The rsult showed MDA consistency in rats serum increased after cryotherapy, and the highest MDA consistency appeared respectively in the 2nd week, 4th week and 7th week (P < 0.01). The variation of MDA consistency in mice serum is identical to that when carrying out cryotherapy, which demonstrates that lipid peroxidation involves the whole course of injury and recovery of cryotherapy.

[Intracerebral transplantation of genetically modified cells for Parkinson's disease].

The clinical application of brain transplantation for treating Parkinson's disease (PD) is limited because it is difficult to obtain ideal donor cells. In this study, the tyrosine hydroxylase (TH) gene was introduced into NIH-3T3 cell line and the genetically modified cells reacted positively with TH antiserum and released L-dopa. These cells were implanted into the striatum of PD rats. 3,6,9,12 days after implantation, the average rate of the abnormal rotational behavior recovery was 52.6%, 68.4%, 63.2%, 44.7%, respectively. This result is considered to be potential for further clinical application.

[Clinical and pathological characteristics in Chinese patients with pauci-immune crescentic glomerulonephritis].

We investigated pauci-immune crescentic glomerulonephritis (PICGN) in Chinese patients, During 11 years (1985-1996), 5,800 patients underwent renal biopsy in Nanjing Jinling Hospital. Eighteen patients were diagnosed as having PICGN. They were 11 women and 7 men with median age of 29 (range 10-71 years). Polyarteritis (33.3%) and polyarteritis nodosa (11.1%) were the secondary diseases. The incidence of PICGN was 0.31% in renal biopsies and 23.10% in crescentic glomerulonephritis. Clinically, most patients (83.3%) showed rapidly progressive nephritis with enlarged kidneys. Gross hemoturia was noted in 61.1% patients, hypertension 44.4%, nephrotic syndrome 44.4%, and oligria 33.3%. However, systemic symptoms were rare except anemia. Pathologically, we observed necrosis of glomerular capillaries (61.1%), infiltration of monocytes and neutrophil cells in glomeruli (44.4%), and vasculitis in interstitium (55.5%), in addition to glomerulosclerosis (44.4%), severe tubular atrophy (83.3%) and interstitial fibrosis (72.2%). Antineutrophil cytoplasmic antibodies (ANCA) was positive in 41.7%. All patients except one received intensively immunosuppressive therapy. 12 patients were subjected to long-term follow up (median 25.9, range 8-72 months). Only three patients were dialysis-dependent, while none died.

[Detection of types I, III and IV collagen in human cementum, periodontal ligament and alveolar bone].

The aim of the present study was to investigate distribution and composition of collagen Types I, III and IV in human cementum, periodontal ligament and alveolar bone with immunohistochemical localization and image analysis technique. The results showed that collagen Type I was 78.06% in periodontal ligament, 73.09% in cementum and 30.50% in alveolar bone; Type III was 11.73% in periodontal ligament and was also observed in apical cementum and bone tissue of alveolar socket; Type IV was restricted to the basement membrane of the blood vessles; Sharpey's fibers was stained by Type I and its periphery was demonstrated stronger staining by Type I and Type III as a sheath. These results provided necessary informations on healthy periodontal tissues that will be required for future studies on the effects of pathological and regenerative processes.

[Auxiliary partial orthotopic liver transplantation in rats].

A rat model of auxiliary partial orthotopic liver transplantation (APOLT) was developed in this study. Under hemihepatic inflow and outflow occlusions, the recipient underwent 75% partial hepatectomy before the liver graft comprising 30% of the donor liver was implanted on the orthotopic site. Forty rat APOLT were successfully performed. The 5-day survival rate of the recipient was 87.5% and that of the graft was 75%. On the fifth postoperative day, the living graft weight increased by 100%, the liver cell showed an active proliferation with a diploid of deoxyribonuclease (DNA), and the liver cells in DNA-synthetic phase accounted for 22.6 +/- 2.75%, significantly exceeding 12.22% +/- 1.48% of the normal rat hepatocytes (P < 0.001). It may be concluded that the rat APOLT is a more ideal animal model.

[Predictive chemotherapy of breast cancer directed by MTT assay in vitro].

OBJECTIVE: The predictive value of in vitro MTT assay for chemotherapy to breast cancer patients was studied. METHODS: Of the 156 advanced breast cancer patients, 83 had had MTT assay before chemotherapy. According to the result of the MTT assay, 73 patients in MTT sensitive group received chemotherapy. The other 10 patients in MTT resistant group and 73 patients in the control group were given chemotherapy according to clinician' experience. RESULTS: The response rate in MTT sensitive group was 76.7% (56/73). There was statistically significant difference as compared with 0(0/10) in the resistant group and 43.8% (32/73) in the control group. Between MTT sensitive group and control group, similar results were obtained at different therapeutic phases, in the subgroups of most lesions and chemotherapeutic schemes. Both the relapse rate and mortality rate were reduced remarkably in the MTT sensitive group. But there was no difference in the median remission and survival of relapsed patients in the two groups. Between in vitro and in vivo, the overall coincident rate was 79.5% [(56 + 10)/83]. CONCLUSION: The in vitro MTT assay can predict chemotherapy for breast cancer giving satisfactory results.

Study on small hepatocellular carcinoma and its extension.

This paper summarizes the "study on small hepatocellular carcinoma and its extension" in Liver Cancer Institute, Zhongshan Hospital of Shanghai Medical University during the past 25 years. The results indicated that it was an important approach to obtain long-term HCC survivors, of the 239 patients with 5-year survival, small HCC resection accounted for 51.4%. It was an effective approach to improve the prognosis of HCC in the entire series, the 5-year survival of inpatients treated in authors' institution was 4.8% in 1958-1970, 12.2% in 1971-1983, and 46.7% in 1984-1995; which were correlated to the increase proportion of small HCC resection in the series; it was more effective as compared to large HCC resection, the 5 year survival was 61.3% (n = 645) versus 33.6% (n = 950). Extensions of small HCC study included early detection and treatment of small recurrent HCC, of the 147 patients with re-resection, the 5-year survival was 48.9% calculated from the time of first resection. Another extension was conversion of large HCC into small HCC, using multimodality combination treatment, 72 out of the 663 patients with surgically verified unresectable HCCs have been converted to resectable, 5-year survival being 62.1%, which was comparable to that of small HCC resection. Studies on related basic aspect of small HCC such as cell origin of recurrence, and molecular aspect of small HCC, indicated that biological characteristics, particularly the tumor invasiveness, remained the key link for further prolong survival after small HCC resection. Recently, a "patient-like" human HCC metastatic model in nude mice has been established. Experimental interventions have also been tried. Clinical trials for prevention of recurrence after small HCC resection have shown preliminary encouraging results. However, the "cost-effectiveness" of screening, the invasiveness of HCC, the multicentric origin, the coexisted Child C cirrhosis, etc., remained great challenge.

[A research on rat osteoblasts culture in vitro].

Osteoblasts were isolated from Sprague Dawley rats of 1-3 days on the frontal parietal bones and were confirmed by testing for multiple biological features. The results showed that rat osteoblasts in vitro had the same morphological feature, ALP activity, the function of synthetic BMP as in vivo and it had the ability of calcification in vitro. The results proved that these rat osteoblasts in vitro had the same function and features as in vivo and it could be used to study cytocompatibility on replacement materials of human body's hard tissues and biomedical to make experiments related to bone tissues.

[Effect of noise in submarine compartment on signal discrimination and arithmetic performance].

To study the effects of noise level in submarine compartment on signal discrimination and arithmetic performance, experiments were carried out on 13 subjects. The results showed that as noise level raised from moderate (73 dBA) to higher levels (85-92 dBA), work efficiency showed a progressive decrease, but when noise level reached 96 dBA the decrease in efficiency reached a steady state. The results indicated that impairment of efficiency apparently occurred at a level of 85 dBA and that the interference effect of noise was more pronounced on task performance with mental strain.

[Pharmacological study of cholagogic capsule].

Cholagogic capsule is mainly composed of Fiuctus Cardui, Mariae, Oleum Menthae Piperilae, Radix Currmae, olia taraxaci, Oleum Hyperici. It is shown that Cholagogic Capsule increase the rat's bile secretion significantly, but bile component is not influenced effectively, the retention jaundice models of which are created by ANIT isn't influenced effectively. It can reduce the mouse pinna swelling caused by dimethythenzene and depress the metatarsal swelling induced by carragenin, the antiinflammatory effect keeps for more than 4 h. It inhibits the contraction of gallbladder smooth musele of Guinea Pig caused by Ach. The results show that cholagogic Capsule increase bile secretion, antispasmodic and antiinflammatory effect. It is suggested the therapeutic effects of Cholagogic Capsule in sand-like calcul and acute chronic cholecystitis are related to the above pharmaological effects.

[Virological and clinical features of patients with sporadic hepatitis C].

In this study, the transmission route in 16 sporadic hepatitis C (SHC) patients was investigated. Three of them were surgeons who had often had occupational needlestick accidents, another 3 had close household contact with their spouses who had been diagnosed as chronic posttransfusion viral hepatitis C (PTHC), and the remaining 5 had potential parenteral exposure such as tooth extraction, injection or inoculation and so on. Five patients with SHC didn't have such history, their transmission route was not determined. Our result showed a lower viremia level in patients with SHC when compared to PTHC patients (the serum dilutions for HCV RNA detection was 10-100 times in the former and 100-10000 times in the latter. P<0.01). Only 1 patient with SHC was anti-HCV positive. Comparing to PTHC, the patients with SHC in our study had milder liver demage and lower ALT levels, and most of them (10/16) were symptomless.

The prognosis of biopsy-proven lupus nephritis in chinese patients: long term follow-up of 86 cases.

OBJECTIVE: To investigate the prognosis of lupus nephritis (LN) and its influencing factors in patients with lupus nephritis in China. METHODS: A retrospective study was carried out in 86 patients followed up for at least 5 years. Clinical features, serological and immunological tests were investigated. Renal biopsies were performed at the beginning of the study and repeated in some cases during the follow-up period. All the 86 patients had serum creatinine (Scr) level less than 132.6 mumol/L at the initial presentation and were divided into three groups according to the level of Scr at the end of the study. Group I: the patients with normal renal function (Scr < 132.6 mumol/L); Group II: the patients with mild-moderate renal insufficiency (132.6 mumol/L < or = SCR < or = 530.4 mumol/L); Group III: the patients with end stage renal failure (ESRF) (Scr > or = 530.4 mumol/L). RESULTS: Forty-seven patients reached clinical remission with normal renal function, 11 had with stabilization of renal function, although the systemic lupus erythematosis (SLE) activity relapsed repeatedly, while 28 subjects developed renal insufficiency after 60-170 (mean 88.12 +/- 28.23) months of observation. ESRF rate was 11.6% in this group of patients. Eight patients died (2 died of infections and 6 died of ESRF) during the follow-up period. At the beginning of the study the rates of hypertension, persistent anemia and hematuria in Group II were 50%, 70%, and 70% respectively, being much higher than those in Group I. The calculation of AI and CI in 60 patients revealed that there were 65% of patients with AI > or = 7 and 70% of patients with CI > or = 3 in Groups II and III, while in Group I there were only 32% of patients with AI > or = 7 and 19% of patients with CI > or = 3. Sixteen cases had pathologic class changed in 48 repeated biopsies. Seven cases changed to Class IV, 5 to Class II, 3 to Class V from other classes and one to class III from Class II. CONCLUSIONS: Factors associated with the development of renal insufficiency in these lupus patients included hypertension, anemia and hematuria. Renal biopsy evaluation offered additional prognostic information and showed that patients with severe active and chronic histologic changes were at risk for developing renal insufficiency. During the clinical course, the renal classification of LN changed in certain patients, thus the histologic classification of renal morphology at initial presentation did not fully predict the outcome. Renal involvement is very common in systemic lupus erythematosis (SLE) patients. Lupus nephritis (LN) is the most common form of secondary renal disease in China. It has various and unpredictable clinical features. The risks associated with its treatment have challenged investigators to detect the factors which may influence the survival rate of patients with LN and to develop the rational approaches to therapy. Decades of intensive investigation at many centers around the world had underscored the predictive value of demographic, clinical and laboratory data prior to treatment. Controversies still existed due to the difference in environment, race and the selection criteria of patients as well as the method used to evaluate the outcome. There are still some factors which are thought to have an impact on the prognosis of LN. Most of the articles on the prognosis of patients with LN were reported from Europe and America either about Caucasian or African-American patients. The prognosis of LN and its influence in Chinese patients need more elucidation.

Effects of methylprednisolone and cyclophosphamide pulse therapy on renal infiltrating cells in patients with crescentic glomerulonephritis.

OBJECTIVE: To investigate the effects of pulse methylprednisolone (MP) and monthly intravenous cyclophosphamide (CTX) therapy (MP + CTX) on renal infiltrating cells in patients with rapid progressive glomerular nephritis (RPGN). METHODS: Twelve patients with RPGN (> 50% crescents) were given MP + CTX therapy and received repeated renal biopsies within 4 to 12 weeks after MP + CTX treatment. Seven were diagnosed as type II RPGN, including one case of IgA nephropathy, 2 cases of idiopathic RPGN and 4 cases of lupus nephritis and five were diagnosed as type III RPGN, including 2 cases of idiopathic RPGN and 3 cases of vasculitis. The changes of infiltrating CD4+, CD8+, CD68+ and proliferating cell nuclear antigen-PCNA+ cell levels were determined by four PAP method in glomeruli and interstitium. RESULTS: In the patients before MP + CTX therapy, there were higher levels of infiltrating CD4+ and CD8+ cells (306 +/- 118 and 223 +/- 98.4Num/mm2) in renal interstitium, CD68+ cells (17.2 +/- 9.95 Num/G) in glomeruli and (1120 +/- 229 Num/mm2) in interstitium, and PCNA+ cells (7.56 +/- 3.57 Num/G) in glomeruli and (17.6 +/- 6.85 Num/mm2) in interstitium as compared with those in the patients after MP + CTX therapy (CD4+/CD8+ cells were 171 +/- 87.5/121 +/- 38.4 Num/mm2, CD68+ cells were 9.04 +/- 4.33 Num/G and 600 +/- 107 Num/mm2, and PCNA+ cells were 2.04 +/- 1.43 Num/G and 9.40 +/- 4.45 Num/mm2). These changes were associated with improving renal dysfunctions (the levels of serum creatinine and proteinuria decreased gradually from 766 +/- 356 to 284 +/- 192 mumol/L and 2.60 +/- 1.46 to 1.29 +/- 0.85 g/day). CONCLUSIONS: Our data indicate that the renal infiltrating cells may play an important role in renal injury in patients with RPGN. The effects of MP + CTX therapy on improving renal dysfunctions may partially contribute to its amelioration of infiltrating cells in renal tissues. The degrees of CD4+, CD68+, and PCNA+ cells in the kidney may be useful indicators of MP + CTX therapy for RPGN.

Effects of cyclosporine A on serum and urinary soluble interleukin-2 receptor in patients with lupus nephritis.

OBJECTIVE: To evaluate the association of the level of urine and serum soluble interleukin-2 receptor (sIL-2R) with disease activity and response to cyclosporine A (CsA) therapy in patients with lupus nephritis (LN). METHODS: Sixteen hospitalized patients with LN were studied. At admission, fifteen patients had type IV-LN and one had type V-LN. All patients received CsA 6 mg/kg per day for 6-8 weeks, then tapered off gradually to 2 mg/kg per day. The levels of urinary and serum sIL-2R were determined by enzyme-linked immunosorbent assay (ELISA). Serum antinuclear antibody (ANA), anti-dsDNA antibody (A-ds-DNA), complement C3 and C4, total IgG, creatinine, urinary red blood cells and protein excretion, and lymphocyte subpopulations in the peripheral blood were also measured before and after CsA treatment. RESULTS: In LN patients, both urinary (534 +/- 101 U/ml) and serum SIL-2R levels (326 +/- 148 U/ml) were higher than those in normal controls. These findings were associated with higher levels of peripheral blood CD4 + and CD8 + lymphocytes (29.3 +/- 4.24 and 28.6 +/- 9.12%), higher titer of serum anti-ds-DNA, lower levels of serum complement C2 and C4 (0.98 +/- 0.23 and 0.24 +/- 0.12 g/L), as well as more proteinuria (Upro 2.99 +/- 0.76 g/24 hrs) and hematuria (URBC 83.9 +/- 95.2 10(4)/ml). These abnormalities were gradually ameliorated by CSA therapy. The changes in the levels of both serum (116 +/- 58.6 U/ml) and urine (136 +/- 43.2 U/ml) SIL-2R induced by CsA (at 8 weeks) were correlated with the changes in the levels of CD4 + and CD8 + cells (23.2 +/- 3.30 and 26.7 +/- 3.54%), degrees of immune abnormalities (serum C3 and C4 1.28 +/- 0.14 and 0.42 +/- 0.06 g/L), and renal injuries (Upro 1.07 +/- 0.46 g/24 hrs, URBC 5.82 +/- 3.15 10(4)/ml). CONCLUSIONS: These results suggest that serum and urinary sIL-2R are sensitive markers for the disease activity in patients with LN. CsA, a powerful immunosuppressive agent, significantly improves both immunologic disorders and renal functional impairments, the mechanism of which on patients with LN appears to inhibit the lymphocyte activation in the peripheral blood and renal tissues as indicated by the decrease in sIL-2R levels.

[The role of MMP-2 in the invasion and metastasis of hepatocellular carcinoma (HCC)].

OBJECTIVES: To get insights into the role of MMP-2 in the invasion and metastasis of hepatocellular carcinoma (HCC), and to find a method to judge the invasion and metastasis of HCC through MMP-2. METHODS: Zymograph and immunohistochemistry were used to study the content and types of positive cells of MMP-2 in the HCC, and statistical methods were used to analyse the association between the content of MMP-2 and the pathological indexes of HCC. RESULTS: MMP-2 was expressed by all the normal liver, HCC and surrounding liver parenchyma. The increase of MMP-2 and the presence of the active type of MMP-2 were related to the invasion and metastasis of HCC. The content of MMP-2 in HCC being higher than that in surrounding liver parenchyma was an important index to judge the invasion and metastasis of the HCC. The positive cells of MMP-2 found in immunohistochemistry were normal hepatocytes, cholangioepithelial cells, Ito cells, regenerated hepatocytes, new generated cholangioepithelial cells, and HCC cells. CONCLUSION: MMP-2 was related to the invasion and me astasis of HCC. The content of MMP-2 in HCC being higher than that in surrounding liver parenchyma could be bused as an important index to judge the invasion and metastasis of HCC.

[The investigation of risk factors of cardiovascular diseases in elderly people in Beijing].

The health status of 623 elderly people aged 60 years and over in different residential areas of Beijing was investigated. Among them 289 lived in the Xuanwu district, an urban area, 194 lived in a suburb area of Beijing, the Daxing county and 140 lived in a mountain area of the Huairou county. It was found that the prevalence rates of overweight (BMI 24-27) and obesity (BMI > or = 28), hyperlipidemia and diabetes in urban residents were much higher than that in the suburb and mountain residents (P < 0.01 and 0.05, respectively). The rate of obesity is higher in female than in male (P < 0.05). The rates of overweight and obesity decreased with increase of age (P < 0.001). In subjects with overweight and obesity the rates of hyperlipidemia, hypertension and diabetes were higher than those in the subjects without (P < 0.001). The detected rates of coronary heart disease and calcified valvular heart disease between the four groups with different body weight did not differ significantly.
