[The role of membrane-type 1 matrix metalloproteinase in the invasion and metastasis of hepatocellular carcinoma].

OBJECTIVE: To study the role of membrane-type 1 matrix metalloproteinase (MT(1)-MMP) in the invasion and metastasis of hepatocellular carcinoma (HCC), and find a method to judge the invasion and metastasis of HCC through MT(1)-MMP. METHODS: RT-PCR was used to study the expression of MT(1)-MMP mRNA in the HCC and in HCC nude mice model with higher metastasis potential (LCI-D20) and lower metastasis potential (LCI-35), and statistical methods were used to analyse the association between the expression of MT(1)-MMP mRNA and the pathological indexes of HCC. RESULTS: MT(1)-MMP mRNA was expressed by all the normal liver tissue (1.12 +/- 0.73), HCC (3.51 +/- 0.97), surrounding liver parenchyma (1.19 +/- 0.57), tumor embolus, and HCC nude mice model with LCI-D20, LCI-D35. The overexpression of MT(1)-MMP mRNA in HCC, embolus, LCI-D20 was related to the invasion and metastasis of HCC. The expression of MT(1)-MMP mRNA in HCC being higher than that in surrounding liver parenchyma was an important index to judge the invasion and metastasis of the HCC. CONCLUSION: MT(1)-MMP is related to the invasion and metastasis of HCC. The expression of MT(1)-MMP mRNA in HCC being higher than that in surrounding liver parenchyma could be acted as an important index to judge the invasion and metastasis of HCC.

Gene therapy for murine liver cancer by adenovirus-mediated cytosine deaminase gene.

OBJECTIVE: To investigate anti-tumor effects of AdCD/5FC system on mice bearing MM45T.Li liver cancer. METHODS: IC(50), bystander effect and cell apoptosis were observed in vitro. After treatment of the tumor with AdCD in situ and injection of 5FC peritoneally into the mice, the anti-tumor effects were observed. RESULTS: When MOI=100, AdCD infected MM45T.Li was sensitive to 5FC, and the IC(50) was less than 50 micromol/L. When 10% MM45T.Li cells were infected with AdCD, 72% cells died after 5FC treatment, which showed significant bystander effect. Through TUNEL test and Hoechst 33258 staining, it was demonstrated that there was cell apoptosis after AdCD/5FC treatment of the MM45T.Li cell 21 day after treatment of the mice bearing tumor. The tumor volume was reduced significantly (F=20.33, P<0.05), and the median survival time was prolonged from 41-43 day to 49 day. CONCLUSION: AdCD/5FC system shows significant anti-tumor effects.

Rat hepatocellular apoptosis induced by glycodeoxycholate.

OBJECTIVE: To explore the relationship between glycodeoxycholate (GDC) and rat hepatocellular apoptosis. METHODS: GDC was used to treat rat hepatocytes cultured in vitro and the apoptotic cells were observed and analyzed with light microscope, transmission electron microscope, TUNEL in situ hybridization, DNA agarose gel electrophoresis and flow cytometry. RESULTS: When rat hepatocytes were incubated with GDC (final concentration of 100 micromol/L) for 2h, apoptotic hepatocytes were observed with light and transmission electron microscope, and TUNEL in situ hybridization. When the hepatocytes were incubated with GDC (final concentration of 50, 100, 150 micromol/L, respectively) for 2h, or with GDC (final concentration of 100 micromol/L) for 2, 4, 6, 8h, respectively, agarose gel electrophoresis of the hepatocytes DNA demonstrated the typical ladder patterns. CONCLUSION: GDC with final concentration of 50, 100, 150 micromol/L could induce rat hepatocellular apoptosis.

[Antitumor activities in vivo of interleukin-12 gene modified dendritic cells in murine models].

OBJECTIVE: To study the antitumor activities of the mIL-12 transfected and acid-eluted peptide sensitized dendritic cells (DC) in murine H22 liver cancer model. METHODS: The murine bone marrow derived DC were transfected with the recombinant adenovirus (Ad mIL-12) containing the mIL-12 gene and control virus AdBGFP. The peptides on the surface of the murine H22 cell line were eluted with mild acid buffer and then sensitized the transfected DC. The experimental animals were immunized with the differently disposed DC and the state of tumorgenesis and tumor growth was observed. RESULTS: The tumor growth and weight in the group immunized with mIL-12 transfected and acid-eluted peptide sensitized DC were significantly lower than those of control group (P<0.05). CONCLUSION: The mIL-12 transfected and acid-eluted peptide sensitized DC can induce obvious antitumor activities in the murine liver cancer models and may suggest new strategies for constructing new type of DC vaccine for liver cancer.

[The expression of platelet-derived endothelial cell growth factor in liver cancer].

OBJECTIVE: To investigate the expression of platelet-derived endothelial cell growth factor (PD-ECGF) in liver cancer and explore its related intervention with Capecitabine for the growth and metastasis of liver cancer. METHODS: The protein level of PD-ECGF was determined using immunohistochemical method in 61 HCC samples and the mRNA level was detected using Northern blot analysis. Capecitabine was administered orally in 24 nude mice bearing LCI-D20 tumor. All treatments lasted 3 weeks. The tumor size was calculated by the following formula: V = a x b2 x 0.5. Lung metastasis was evaluated by HE staining in lung samples. RESULTS: The PD-ECGF expression rate in 61 HCC and paratumoral liver tissues was 70.5% and 47.5%, respectively. The rate was higher in HCC tissues with advanced TNM stage than in those with early TNM stage (81.1% vs 54.2%, P < 0.05). The mRNA level of PD-ECGF was related well to its protein expression. The tumor size on day 3 after last treatment measured in the control, and low dose (1.05 mmol.kg-1.day-1), moderate dose (2.10 mmol.kg-1.day-1) and high dose (3.15 mmol.kg-1.day-1) of Capecitabine treatment groups was 447 mm3 +/- 159 mm3, 414 mm3 +/- 97 mm3, 240 mm3 +/- 119 mm3 and 209 mm3 +/- 150 mm3, respectively. High doses of Capecitabine increased the inhibitory effect on the growth and lung metastasis of HCC implant. CONCLUSION: PD-ECGF is highly expressed in HCC and correlates with the TNM staging. Capecitabine may inhibit the growth and metastasis of HCC.

Peri-implant mucosal inflammation and bone loss: clinical and radiographic evaluation of 108 dental implants after 1-year loading.

OBJECTIVE: To evaluate the clinical outcomes, and a possible correlation between peri-implant mucosal inflammation and marginal bone loss, 1 year after restoration in a group of Chinese patients with dental implants. METHODS: Observation of the clinical and radiographic parameters of 108 IMZ and Frialit-2 dental implants in 70 patients. RESULTS: One year after loading, there were no signs of mobility and peri-implant radiolucency in any of the implants. The average bone loss adjacent to implants was 0.63 mm (SD 0.78). The prevalence of peri-implant mucosal inflammation, as indicated by sulcus bleeding index (SBI) of 1.0 or more, was 32.9%. Significant positive correlation was found between mucosal inflammation and bone loss (P < 0.05). 92% of the observed sites had plaque index (PLI) scores greater than 1. CONCLUSIONS: The 1-year outcomes from this patient group indicate that IMZ and Frialit-2 implants provide satisfactory clinical results. One of the etiologic factors of alveolar bone loss around implants seems to be plaque-associated marginal inflammation. Improvement of oral hygiene, elimination of soft tissue inflammation, and proper treatment of periodontal disease of natural teeth are critical for maintaining the implants.

Hepatocellular adenoma and focal nodular hyperplasia: a series of 24 patients with clinicopathological and radiological correlation.

OBJECTIVE: To investigate two rare benign lesions, hepatocellular adenoma (HCA) and focal nodular hyperplasia (FNH), and evaluate differential diagnosis. METHODS: Twenty-four consecutive patients with presumed HCA and FNH were studied at the Liver Cancer Institute from January 1996 to May 1999. Preoperative assessment included clinical evaluation, symptoms and laboratory tests. New imaging techniques were prospectively appraised in addition to usual techniques. All had hepatic resections and follow-up. Histologic examination of surgical specimens was obtained in all cases. RESULTS: In every instance, FNH was an incidental finding. FNH consists of nodular aggregates of cytologically normal hepatocytes with foci of intranocular bile duct proliferation. In this series, patients with HCA had larger tumors and more often were symptomatic but the occurrence was unrelated to oral contraceptive steroids (OCS) usage. Intralesional hemorrhage or necrosis is common, and was seen in 75% of cases. The best imaging procedure in the diagnosis of FNH was MRI. Color Doppler US was a useful adjunct, but CT lacked specificity, making histological diagnosis mandatory. All patients underwent tumor resected were tumor--free during the follow-up. CONCLUSIONS: FNH is a distinct histopathologic entity, and is distinguishable from HCA. FNH is a hyperplastic response by the liver parenchyma to a pre-existing arterial malformation. HCA is a liver neoplasia and has the potential of malignant transformation to HCC. Based on these findings, we believe that if the clinical suspicion of HCA or FNH is strong, resection is usually the best approach if technically feasible and histologic diagnosis is mandatory.

[Diagnosis and treatment of bile duct tumor thrombus in patients with hepatocellular carcinoma].

OBJECTIVE: Tumor thrombus in the bile duct (BDT) is very rare in patients with hepatocellular carcinoma (HCC). The prognosis of the patients with BDT is very poor. To improve the prognosis, HCC patients with BDT were retrospectively reviewed. METHODS: Retrospective study was performed in 16 cases of HCC with BDT found in authors' institute from July 1987 to Oct. 1998. Factors affecting prognosis were analyzed. RESULTS: The occurrence rate of BDT as 0.76% (16/2100). Removal of BDT and HCC was performed in all but one patients. Fourteen patients were followed-up for over 1 year after operation. The 1-year survival and recurrence rate was 71.4% (10/14) and 57.1% (8/14), respectively. Three female patients survived over 4, 6, and 12 years, respectively. CONCLUSIONS: Early detection and diagnosis, surgical removal of primary tumors and BDT are the key points to prolong the survival time of patients.

[Nitric oxide synthase and vascular endothelial growth factor expression in hepatocellular carcinoma and their relation to angiogenesis].

OBJECTIVE: To analyse inducible nitric oxide synthase (iNOS), endothelial nitric oxide synthase (eNOS), vascular endothelial growth factor (VEGF) protein expression in hepatocellular carcinoma (HCC) and their relation to angiogenesis. METHODS: Tissue sections from 71 HCC patients were examined immunohistochemically for protein expression of iNOS, eNOS, and VEGF. Microvessal density (MVD) was counted by endothelial cells immunostained by anti-CD34 antibody. RESULTS: Positive iNOS and eNOS immunostaining was detected in 83.1% and 85.9% of HCC respectively. iNOS and eNOS were not detected in normal hepatic tissue. MVD was 34.3 +/- 1.5/HP and 38.6 +/- 1.6/HP in HCC positive for iNOS and VEGF while it was 31.2 +/- 2.8/HP, and 22.4 +/- 2.0/HP in HCC negative for iNOS and VEGF respectively (P < 0.01). A correlation between NOS expression and VEGF in HCC was not observed. CONCLUSION: iNOS and eNOS may play a role in malignant transformation of post-hepatic cirrhosis. The expression of iNOS and VEGF favors angiogenesis of HCC.

[Effect of chang'ai kangfu decoction on immunity in postoperational patients with large intestine cancer].

OBJECTIVE: To explore the effect of Chinese drug Chang'ai Kangfu decoction (CAKF) on immunity in post-operational patients with large intestine cancer (LIC). METHODS: Forty-eight patients with LIC in Dukes' B, C stage after operation were randomly assigned to 3 groups, the CAKF group (16 cases), chemotherapy group (17 cases) and combination therapy (CAKF plus chemotherapy) group (15 cases). 5-FU and mitomycin C were given to the chemotherapy group. The dynamic changes of T-lymphocyte subsets, NK cells and immunoglobulins were investigated. RESULTS: Before operation, the CD3+, CD4+, CD4+/CD8+ and the activity of NK cells in LIC patients were lower, but CD8+ was higher than that of normal level (P < 0.01), which indicated that cellular immunity in LIC was in immunosuppressive state, they all further reduced 1 week after operation, particularly CD3+ cell counts, but CD3+, CD4+ and the activity of NK cells normalized 1 month after operation in CAKF group, and 2 months were needed to normalize in combination therapy group. Both groups recovered to a certain extent in comparing with before treatment, but the chemotherapy group recovered slower. The similar results appeared in humoral immunity. CONCLUSION: CAKF could obviously increase the immunity in LIC patients after operation.

[The regulation of calcium in the movement of colonic smooth muscle in wrap restraint stress rats].

OBJECTIVE: To investigate the influence of contraction of isolated colonic smooth muscle and role of guanylin in colonic movement in wrap restraint stress rats. METHODS: By using wrap restraint stress induced defecation rats, the tone of isolated longitudinal and circular colonic muscle strips and guanylin contents in colon and blood were measured by transducer and radioligand binding assay respectively. RESULTS: There were similarities between the intestinal effects of wrap restraint stress in rats and irritable bowel syndrome (IBS) in human. Therefore, wrap restraint stress rat is an appropriate animal model to study stress related intestinal dysfunction. In wrap restraint stress rats, both the tone of smooth muscle and the reaction to K(+), Ca(2+) and acetycholine increased significantly; meanwhile guanylin contents in blood and colon increased also. Trimebutine can decrease the basic tone of smooth muscle and reaction to Ca(2+), K(+) and acetycholine. CONCLUSIONS: The abnormality of colonic smooth muscle contraction is one of the mechanisms in IBS and regulation of colonic smooth muscle contraction is a new method to treat IBS.

[Retrospective cohort study on the prognosis of chronic myeloid leukemia].

OBJECTIVE: To evaluate the prognostic factors of chronic myeloid leukemia (CML). METHODS: Survival curves were plotted according to the Kaplan-Meier method. Variables associated with prognosis were analyzed by Logrank test. A multivariate analysis of prognostic factors was performed using stepwise Cox model. RESULTS: Of the total 158 patients, the median survival time was 1,480 days. 5 years survival rate was 47.1% (95% CI, 36.7% - 56.9%). 10 years was 24.8% (95% CI, 15.0% - 35.8%). The predominant blastic crisis type is myeloblastic, myelomonocytic or monocytic(77.5%). The period from chronic phase to blastic phase was equal in myelosuppressive therapy group, non- myelosuppressive therapy group or altretamine (HMM) group. After evaluated by stepwise Cox's regression model, age>or=50 years, bone marrow blasts>or=0.05, bone marrow basophilic>or=0.05, Ph chromosome negative and HMM treatment were associated with poor prognosis. Hydroxyurea was always associated with longer survival. CONCLUSION: Age, Ph chromosome, bone marrow blasts and basophilic, and various treatment affected the survival of CML.

[Treatment of non iatrogenic injury of retrohepatic inferior vena cava].

OBJECTIVE: To study a repair method for injured retrohepatic inferior vena cava (IVC) associated with liver trauma. METHODS: Laceration of IVC was sutured bloodlessly after the injured retrohepatic IVC was occluded by compression combined with occlusion of the first hepatic hilus. RESULTS: From April 1996 to January 1999, the injured IVC was successfully repaired in 6 patients. The length of laceration was 2.9 cm (1.5 - 4 cm). The occlusion time was 9.2 min (6 - 15 min) for the IVC and 20.5 min (15 - 29 min) for the first hepatic hilus, respectively. All patients recovered uneventfully, and the IVC was patency without stricture as showed by B-mode ultrasonography and CT. CONCLUSION: The injured retrohepatic IVC can be repaired by direct compression when the laceration of the IVC is caused by liver trauma.

[The recognition of the epidemic area of tsutsugamushi disease on Nan Peng Lie Island in China and the strategy of prevention].

OBJECTIVE: To investigate the region for the epidemic area of tsutsugamushi disease. METHODS: Epidemiological studies, Orientia tsutsugamushi isolation, and preventive measures were used. RESULTS: The region belonged to epidemic area of south subtropical zone. The main host was found Rattus norvegicu. The main biological vector was Leptotrombidium deliens. The seasonal trends of the quantity of Rattus norvegicu and Leptotrombidium deliens were consistent with the incidence change of the disease in the region. Orientia tsutsugamushi has been isolated from Rattus norvegicu and Leptotrombidium deliens. Data showed that Orientia tsutsugamushi isolated strains mainly belongs to Karp. Sero-epidemiological data showed that antibodies of Orientia tsutsugamushi are 100.0% positive in the residents of the region and 4.0% in army men. After taking comprehensive preventive measures, the incidence was descending. CONCLUSION: Nan Peng Lie island was proved an epidemic area of tsutsugamushi disease.

[A clinicopathological study of hepatic focal nodular hyperplasia].

OBJECTIVE: To study the clinicopathological features of focal nodular hyperplasia (FNH). METHODS: The clinicopathological characteristics of 25 cases of FNH were studied retrospectively. 20 cases followed for 6 to 47 months. All were evaluated by use of paraffin-embedded sections, special and immunohistochemical staining (EnVision method) and electron microscope. RESULTS: 17 female and 8 male FNH patients aged 14 to 58 (median 38) years of age, all alpha-fetoprotein negative, asymptomatic and normal biochemical liver tests in most cases. The macroscopic hallmark is a central stellate fibrotic scar, composed of fibrous connective tissue and tortuous blood vessels, the fibrous tissue radiated peripherally, dividing the mass into multiple, variably sized nodules, simulating the pattern of cirrhosis. Microscopically, multinodular proliferation of benign-appearing hepatocytes separated by bile-duct-containing fibrous septae that radiate from the central scar. Internodular bile duct proliferation is abundant and merge imperceptibly with the hepatocyte elements near the fibrous septa. Immunohistochemically, the keratin expessed by hepatocytes and by the proliferated bile ducts were similar. Electron microscopic examination found that the ultrastructure of tumor cells were similar to normal hepatocytes. 20 cases were followed for 6-47 months, all survived with no recurrence. CONCLUSION: FNH is a benign hepatocytic lesion, i.e. a reactive proliferation of hepatic cells to local blood vessel anomalies.

[Study on osteocompatibility of hydroxyl polycalcium sodium phosphate in vitro].

Hydroxyl polycalcium sodium phosphate(HP) is a new substitute material for human hard tissues. The isolated rat osteoblasts from the parietal and frontal bones of 1-3d SpragueDawley rats were used to estimate the osteocompatibility of HP in this study. The effects of the materials on cell morphology, growth and proliferation were determined. The result showed that rat osteoblasts could attach, spread and proliferate on HP as normal cells. The proliferation of cells on HP was more favour able to the adhesion and proliferation of rat osteoblasts and could form osteointegration early.

The bi-directional regulation of filamin on the ATPase activity of smooth muscle myosin.

OBJECTIVE: The aim of this study is to investigate the functional relationship between filamin, a known actin binding protein, and myosin and the effects of filamin on the interaction between myosin and actin. METHODS: Ultra-centrifugation method was used to investigate the binding of filamin to both phosphorylated and unphosphorylated myosins. Mg-ATPase activities of both phosphorylated and unphosphorylated myosins in the presence and absence of actin were measured to observe the effects resulted from filamin-actin and filamin-myosin interactions. RESULTS: It was found that filamin is also a myosin binding protein. Filamin inhibited the actin activated Mg-ATPase activity of phosphorylated myosin and stimulated Mg-ATPase of phosphorylated myosin in the absence of actin; in addition, filamin stimulated Mg-ATPase activity of unphosphorylated myosin in both the presence or absence of actin. CONCLUSION: The result suggest that the effects of filamin on the myosin Mg-ATPase activities are bi-directional, i.e., stimulatory via binding to myosin and inhibitory via binding to actin.

Validation of oxygen saturation measurements in a canine model of hemoglobin based oxygen carrier (HBOC) infusion.

OBJECTIVE: The study was designed to validate oxygen saturation measurements from the NOVA CO-Oximeter, the i-STAT System, and the Corning 170 blood gas analyzer under conditions similar to the clinical application of a hemoglobin-based oxygen carrier. DESIGN: A canine model was used for both in-vitro and in-vivo experiments. SETTING: Canine laboratory for in-vivo, and tonometry laboratory for in-vitro sets. PARTICIPANTS: 6 mixed-breed canines, 30 kg; canine remainder sample blood. INTERVENTIONS: In the first set of experiments, the target blood pO2 levels were reached by tonometry. In the second set of experiments, quantitative measurements of total oxygen content with the LEXO2Con-K, were performed immediately followed by measurements with the NOVA CO-Oximeter and the i-STAT system. HBOC was added in concentrations of 16.2, 32.5, 65.0, and 97.5 g/L. To analyze the clinical significance of the differences in the results obtained with the different investigated instruments; blood samples from dogs treated with HBOC after acute hemorrhagic shock were used. RESULTS: There was a strong correlation between the oxygen saturation values measured with the investigated instruments in samples after tonometry and known oxygen partial pressure. The total calculated O2 content varied by 5% based on results generated by calculations based on the investigated instruments. The results did not change with different oxygenation of the sample. The differences among methods were not significant when HBOC concentration was 16.2 g/L. Higher concentrations of HBOC increased the difference between calculated and measured oxygen content; i-STAT system demonstrated a greater deviation from the results of the other two instruments. Systemic oxygen uptake based on investigated instruments showed high correlation with values based on LEXO2CON-K measurements (r=0.97 for CO-Oximeter, 0.96 for gas analyzer, and 0.79 for i-STAT). Systemic oxygen uptake values based on CO-Oximeter and blood gas analyzer data showed a 75% accuracy; i-STAT accuracy was 63% for control samples and 50% for samples after HBOC infusions. CONCLUSION: The NOVA CO-OXimeter is an accurate analyzer for measurement of oxygen saturation after HBOC infusion in the canine model. Use of the i-STAT system should be limited to arterial samples or for samples with a low HBOC concentration.
