Cerebral hydatid cysts in children.

Twelve children with intracranial cysts of Echinococcus granulosus underwent surgery during a period of 5 years and constituted 19% of all children operated on for intracranial space-occupying lesions. The more common symptoms were raised intracranial pressure (8 cases) and hemiparesis (7 cases). The total number of procedures was 14, with a standard craniotomy approach in 13. In 6 the cysts were removed without puncture or rupture, in 5 puncture and drainage were carried out before removal of capsule, and in 3 the cyst ruptured accidentally. Recurrence of multiple cysts occurred in 1 case and another patient was reoperated on twice for recurrent cysts after an operation in another center. There was no mortality. Non-bacterial meningitis occurred in 2 cases. Although drainage of the cyst contents greatly facilitates removal, it also carries a risk of contamination; thus a traditional approach via a large craniotomy seems to be the safest choice.

[Effect of exercise therapy on the PH, pO2 and pCO2 of capillary blood of centrally paralyzed limbs after stroke].

Gasometric determinations of pO2, pCO2 and pH were carried out in capillary blood obtained from the upper extremities before and after 10 minutes of kinesitherapy in 30 patients with hemiplegia following stroke. The determinations were done with an acid-base equilibrium analyser Radiometer Copenhagen pH M 71 MKZ. Ten minutes of kinesitherapy caused significant changes in the pH of the blood towards alkalinization, less pronounced in the paralytic extremities. However, differences in the determinations in paralytic and non-paralytic extremities were statistically not significant. These results confirm earlier observations of the authors and point out that 10 minutes of kinesitherapy seems to be the shortest times of exercise giving significant biochemical and clinical effect.

[Cerebrospinal fluid glucose, lactate and pyruvate concentration in patients with cerebral infarction after intravenous glucose loading test].

The determinations of the above substances were done in the blood and CSF of 27 patients with cerebral infarction and 10 controls. The glucose level was determined in the blood and CSF and lactate and pyruvate levels in the CSF before and in the 8th and 16th minutes after a glucose load. The 0-toluidine colorimetric method was used for glucose and spectrophotometric methods were applied for determination of lactate and pyruvate. In the patients with recent cerebral infarction as compared with the control group an increase of glucose concentration was found in the blood (statistically significant) and in the CSF (statistically not significant), while the levels of lactate and pyruvate were raised in the CSF before as well as in the 8th and 16th minutes of the test. Absence of a statistically significant difference between lactate and pyruvate concentrations in the CSF in the same patients with recent cerebral infarction before and in the 8th and 16th minutes of the test suggests that glucose in a hypertonic solution failed to exert any significant influence on CSF lactate and pyruvate concentrations. These biochemical findings seem to support the observations of good therapeutic effects of glucose in hypertonic solution in some cases of cerebral infarction.

[Case of cerebral aneurysm, arteriovenous fistula and cerebral hematoma].

A female patient is reported who had subarachnoid haemorrhage and in whom angiography demonstrated presence of an aneurysm and an arteriovenous angioma in the territory of the left middle cerebral artery. During surgical intervention an intracerebral haematoma was found additionally in the posterior part of the left temporal lobe. The postoperative course was uneventful. The patient was discharged without evidence of focal brain injury.
