Differential reduction in the volume of leiomyoma and uterus during buserelin treatment.

Twenty-four women (mean age 42.0 +/- 2.4 years) with leiomyoma underwent treatment with a gonadotropin releasing hormone (GnRH) agonist (buserelin) for 6 months at a dose of 1200 micrograms daily intranasally for the first 2 months, followed by 900 micrograms daily for 4 months. The volume of the uterus and of individual leiomyomas were measured by echosonography before treatment, every 2 months during treatment, and every 2 months for 6 months after cessation of therapy (a total of 7 evaluations). A control group, comprising 14 women with untreated leiomyoma, underwent ultrasound scanning every 2 months for 6 months. A significant reduction in the volume of the uterus and leiomyomas was observed after the first 2 months of treatment in all but one patient. Leiomyomas showed a greater reduction in volume (54.6 +/- 2.2%) than did the uterus (41.8 +/- 2.3%; p = 0.0003). They also showed a more rapid increase in volume after cessation of therapy (p = 0.03). The control group showed an insignificant increase in the size of the uterus and individual leiomyomas over the 6 months' monitoring. The differences observed in reduction of the volumes of the leiomyomas and the uterus during the state of hypoestrogenism induced by buserelin, as well as in their regrowth after cessation of treatment, can be explained by the greater number of estrogen receptors in leiomyoma tissue and a possible lower rate of conversion of estradiol into estrone.

[Retroperitoneal hematoma].

Diagnosis and management of retroperitoneal haematoma is the problem of controversy in actual moment. It appears most frequently in the range of polytrauma or various traumas of abdomen and retroperitoneal organs. Here we report our experience in management of retroperitoneal haematoma. During the ten year period (from 1979 to 1986) we treated surgically, at the department for surgery, 58 injured patients with retroperitoneal haematoma. In 5 cases explorative laparotomy was done, and in other 53 cases there were injuries of intraabdominal and retroperitoneal organs. The haematomas were caused by the ruptures of spleen, liver, kidneys, pancreas, duodenum, small bowel with mesenterium, large bowel, bladder, retroperitoneal large blood vessels and pelvic fractures. In 17 cases retroperitoneal haematoma was associated with the injury of one organ. In 36 cases there were injuries of two or more organs. Retroperitoneal haematoma was caused by blunt trauma in most cases. During the management there were some diagnostic difficulties. In diagnosis we use: clinical status, of patients, radiography, angiography, ultrasonography, but the most secure was laparotomy. There are two treatment approaches, operative and conservative. Retroperitoneal haematoma was a consequence of ruptured solid organs and retroperitoneal blood vessels, and associated with injuries of intraperitoneal organs. All this, mentioned above, is the reasons for detailed exploration of abdominal cavity.

[Duplex ultrasonography in the diagnosis of compression of vascular structures at the level of the upper thoracic outlet].

In order to determinate place of compression and degree of haemodynamic disturbance we examined 34 patient and 30 asymptomatic volunteers. By duplex scan we studied retroscalenal part of subclavian artery. Using real-time (B-mode) imaging we founded significant reduction of lumen diameter of subclavian artery during Adsonś, manoeuvre in 51% of cases. We examined haemodynamic changes during Adsonś, costoclavicular and hyper-abduction manoeuvre and found significant changes in linear blood velocity, reduction of blood flow and turbulence in 91% of cases. Adsonś manoeuvre was positive in 52%, costoclavicular in 29% and hyper-abduction (to 90 degrees) in 19% of cases. Place of compression we founded in 82% of cases by duplex ultrasonography prospectively, but using other methods respectively we located compression in 91% of cases.

[Mixed mesodermal tumors of the uterus. Clinico-pathologic characteristics of 23 patients].

The clinical and pathological features, histogenesis and treatment of 23 cases of mixed mesodermal tumours of the uterus (15 benign and 8 malignant) in woman aged from 27 to 78 years (mean 51.25) are reviewed. The tumours manifested by non-specific symptoms, most often uterine bleeding, arising suspicion of malignancy in only two cases. Unlike malignant mixed mesodermal tumours, differentiation of adenofibromas from low-grade adenosarcomas was often very difficult, and needed careful pathohistological analysis of extirpated tissues, including all available histochemical, immunohistochemical and EM procedures. Two cases of adenofibromas recurred as adenosarcomas after four months and five years respectively. Poor prognosis of malignant variants of mixed mesodermal tumours and their difficult differentiation from adenofibromas require close collaboration of pathologists and gynaecologists, implicating hysterectomy as proposed treatment and permanent clinical monitoring.
