Quantitative three dimensional echocardiography in patients with pulmonary hypertension and compressed left ventricles: comparison with cross sectional echocardiography and magnetic resonance imaging.

OBJECTIVE: To evaluate the accuracy of quantitative three dimensional echocardiography in patients with deformed left ventricles. DESIGN: Three dimensional and cross sectional echocardiographic estimates of left ventricular volume and ejection fraction were prospectively compared to those obtained from magnetic resonance imaging. SETTING: Echocardiography laboratory of a university hospital. PATIENTS: 26 patients (9 months to 42 years, median age 11 years) with pulmonary hypertension and fixed reversal of normal interventricular septal curvature. MAIN OUTCOME MEASURES: Left ventricular end diastolic and end systolic volumes and ejection fraction. RESULTS: Three dimensional echocardiographic comparison to magnetic resonance imaging (MRI) yielded r values of 0.94 and 0.87 with a bias of -6.9 (SD 6.9) ml and -16 (11.2) ml for systolic and diastolic volumes respectively. Inter-observer variability was minimal (8.3% and 7.6% respectively). Cross sectional echocardiography gave correlation coefficients of 0.62 and 0.80 and bias of 3.1 (14.1) ml and 16.3 (18.3) ml for systolic and diastolic volumes respectively. Ejection fraction by three dimensional echocardiography also had closer agreement with MRI (bias = 1.1 (7.7)%) than cross sectional echocardiography (bias = 4.4 (13.9)%). CONCLUSIONS: Three dimensional echocardiography provides reliable estimates of left ventricular volumes and ejection fraction, comparable to magnetic resonance imaging in pulmonary hypertension patients with compressed ventricular geometry. Because it eliminates the need for geometric assumptions it shows closer agreement with magnetic resonance imaging in that setting than cross sectional echocardiography.

Different immune effects on cardiac allografts and xenografts induced by neonatal intrathymic inoculation with allogeneic and xenogeneic antigens.

BACKGROUND: In a previous study we demonstrated that intrathymic exposure of neonatal rats to both alloantigens and xenoantigens produced tolerance only to subsequent cardiac allografts and not to xenografts implanted when the animals were 6 weeks old. Interestingly, graft recipients were not sensitized to the xenografts as observed in the adult model. This study was designed to investigate whether earlier grafting would produce tolerance to cardiac xenografts in animals pretreated by neonatal intrathymic inoculation with allogeneic and xenogeneic cells. METHODS: All recipient animals were Lewis rats. Donors were either Lewis Brown Norway rats or Golden Syrian hamsters. Lewis Brown Norway rat and hamster splenocytes (25 x 10(6) cells in a volume of 0.01 ml) were inoculated percutaneously into the thymus of neonatal recipients (n = 22). At age 4 weeks, five pretreated recipients underwent cervical heterotopic heart transplantation with rat hearts, and 2 weeks later abdominal heterotopic transplantation was done with hamster donors. A second group ( n = 6) received hamster hearts as the first graft and then grafts from rat donors. The third group underwent rat followed by hamster heart transplantation at age 6 to 7 weeks. RESULTS: Mean rat allograft survival time for groups 1,2, and 3, respectively was 49.8 days with 4 of 5 surviving indefinitely, 4.3 days with 2 of 3 surviving indefinitely, and 42 days with 7 of 11 surviving indefinitely (p < 0.01 versus untreated control animals, 7.8 days, n = 5). In rats undergoing transplantation at 4 weeks, cardiac xenografts were rejected in 2.5 days 2.3 days, which was significantly shorter than xenograft survival (3.3 days) in rats that underwent transplantation at an older age (p < 0.02), in naive rats (p < 0.05), and in rats treated with hamster cells alone (p < 0.05). Mixed lymphocyte reaction showed a diminished proliferative response to Lewis Brown Norway rat cells in pretreated rats, which retained the ability to respond in culture to hamster cells (p < 0.05). CONCLUSIONS: Earlier grafting in rats pretreated as neonates produces allograft tolerance but may accelerate rejection of xenografts. Preliminary mixed lymphocyte reaction results suggest that only the alloimmune cellular proliferative response is abrogated by this pretreatment.

Purification of phosphoglycerate kinase and glyceraldehyde phosphate dehydrogenase by DEAE-sepharose fast flow chromatography.

The purification of phosphoglycerate kinase (PGK) and glyceraldehyde phosphate dehydrogenase (GAPDH) from soy bean was studied by a DEAE-sepharose fast flow chromatography. When scaling up, the diameters of columns increase but the heights of columns were maintained in the range of 10 to 20 cm to avoid the problem of the high pressure drop. The recovery of GAPDH and PGK was 57% and 41%, respectively. The purification factor of GPADH and PGK was 144 and 44, respectively. The economical evaluation indicated that the cost of the process was 2.09$/ku GAPDH, which has some commercial values.

[Treatment and prognosis of soft tissue sarcoma].

The treatment and prognosis of 121 patients with primary and locally recurrent soft tissue sarcomas of the extremities and trunk were analyzed. The most common histologic type was malignant fibrous histocytoma (32 patients, 26.4%). Ninety-three patients had high grade sarcomas (76.9%) and 28 had low grade sarcomas (23.1%). Adjuvant radiotherapy and chemotherapy were applied to 37 and 41 patients respectively. In 28 patients who had local recurrence after surgery, the 5-year local control rate was 72.3%. The overall 5-year survival rate was 59.9% for all patients, and 59.1% for 96 patients with locally recurrent lesions. The independent risk factors affecting the overall survival included high grade, tumor of upper extremity, perioperative blood transfusion, and tumor size of > or = 10 cm. We conclude that (1) wide excision is the most accepted operative procedure for treatment of soft tissue sarcoma of the extremity and trunk unless the lesions are locally advanced (In the advanced cases amputation should be performed); (2) in patients with local recurrence only, there is an almost same rate of long-term salvage as that in patients with primary tumors; (3) avoiding unnecessary perioperative blood transfusion may improve survival; and (4) further investigation of more effective regimen of adjuvant radiotherapy and chemotherapy needs to be taken.

[Functional motor innervation of brachial plexus roots: an intraoperative electrophysiological study].

The electrophysiological properties of normal brachial plexus and functional motor innervation during the operation of contralateral healthy side C7 transfer were studied different roots of brachial plexus were stimulated and maximum amplitudes were recorded. The results showed functional motor innervation of brachial plexus roots are (1)C5 mainly forms the axillary nerve which innervates deltoid muscle; (2)C5 constructs most of the musculocutaneous nerve fibers which innervate biceps muscle; (3)main component of the radial nerve comes from C7, which innervates triceps muscle; (4)Medial nerve mainly comes from C8, which innervates flexor digitorum muscle; (5)T1 forms most of the ulnar nerve which innervates intrinsic muscle. Based on the relationship between brachial plexus roots and their functional distribution, the particular aspects of functional innervation of C7 as well as the possibility of utilization of other cervical roots were discussed.

[Retinoic acid nuclear receptor beta (RAR beta) inhibits breast carcinoma growth].

Retinoids are capable of modulating cellular differentiation and proliferation. Retinoids mediate gene function through a series of nuclear receptors. The retinoic acid receptor beta (RAR beta) has been shown to play an important role in the differentiation of a number of cell types. RAR beta is either absent or expressed at extremely low levels in a number of tumor types including breast carcinoma. It was demonstrated that transfection of RAR beta gene in breast carcinoma cell with its subsequent expression resulted in inhibition of cell growth. Retinoic acid significantly inhibited monolayer growth of the breast carcinoma cells expressing RAR beta, while it had no effect on the growth of the control cells. The RAR beta expressing cells formed much smaller and fewer colonies in soft agar and were significantly less tumorigenic in nude mice than the controls. These results suggest that RAR beta may function as a tumor suppressor in breast carcinoma cells.

Inhibition of spontaneous apoptosis in human breast cancer.

Breast tumorigenesis proceeds through an accumulation of specific genetic alteration. Breast malignant transformation is dependent on not only the rate of cell production but also on apoptosis, a genetically programed process of autonomous cell death. We investigated whether breast tumorigenesis involved an altered susceptibility to apoptosis and proliferation by examining normal breast epithelium and breast cancer samples. We found there is a great inhibition of spontaneous apoptosis in breast cancer cells compared with normal breast epithelium. The inhibition of apoptosis in breast cancer may contribute to neoplastic transformation.

Retinoic acid nuclear receptor alpha(RAR alpha) plays a major role in retinoid-mediated inhibition of growth in human breast carcinoma cells.

Retinoids mediate their actions via retinoic acid receptors (RARs) and retinoid X receptors (RXRs). Each of class of these nuclear retinoid receptor is further subdivided into three species namely alpha, beta and gamma. Recently studies demonstrated that estrogen receptor (ER)-positive human breast cancer (HBC) cell lines are sensitive and ER-negative cell lines are resistant to growth inhibitory effects of retinoic acid (RA). In this study, we found that only RAR alpha mRNA levels was strongly correlated with ER-status. To further investigate the major role of RAR alpha in retinoid-mediated inhibition of growth, we transfected RAR alpha cDNA into two RA-resistant ER-negative HBC cell lines. Analysis of different clonal populations of RAR alpha transfectants from each cell line revealed growth inhibition by retinoids. Our results demonstrated that RAR alpha plays a major role in mediating retinoids inhibition of growth in HBC cells and adequate levels of RAR alpha are required for such an effect.

[Study of skeletal and occlusal pattern].

The aims of this study were to analyze the characteristics of craniofacial structure in normal occlusion and to assess the relationship between the skeletal and occlusal pattern. Fifty-seven subjects with normal occlusion were divided into three skeletal groups according to Hauston measurement. One hundred and sixty-six subjects with Angle's malocclusion were also classified according to ANB values. The result showed that there were significant differences in the values of NA-APg, AB-NPg, SNA, SNB, U1-NA, L1-NB among the three groups. These data indicate that skeletal pattern does not coincide with occlusion. Different skeletal pattern exists even in normal occlusion. In diagnosis and treatment planning, the data of skeletal pattern should be referred.

Lateral mobility of Fc gamma RIIa is reduced by protein kinase C activation.

The lateral mobility of membrane proteins can reflect the extent of various protein-protein interactions. Using the fluorescence recovery after photobleaching technique, we have studied the lateral mobility of human Fc gamma RIIa and some Fc gamma RIIa mutants expressed in either P388D1 cells, a mouse macrophage-like cell line, or in Chinese hamster ovary (CHO) cells [1]. After treatment with phorbol myristate acetate (PMA), only the Fc gamma RIIa molecules capable of mediating rapid endocytosis of immune complexes exhibited a reduced lateral diffusion coefficient with respect to untreated controls. Wild type Fc gamma RIIa expressed in CHO cells, and nonfunctional Fc gamma RIIa mutants expressed in P388D1 cells did not show any differences upon PMA treatment. This finding suggests that protein kinase C activation evokes additional protein-protein interactions with the cytoplasmic domain of functional Fc gamma RIIa, which reduced receptor lateral mobility. The identity of these putative interacting proteins and the nature of the interactions remain to be elucidated.

Assessment of cardiac function by three-dimensional echocardiography compared with conventional noninvasive methods.

BACKGROUND: Reliable, serial, noninvasive quantitative estimation of left ventricular ejection fraction is essential for selecting and timing therapeutic interventions in patients with heart disease. Equilibrium radionuclide angiography is widely used for this purpose but has well-recognized limitations. Advantages of echocardiography over equilibrium radionuclide angiography include assessment of wall motion, valvular pathology, and cardiac hemodynamics, in addition to portability, lack of radiation exposure, and substantially lower cost. However, conventional echocardiographic techniques are limited by geometric assumptions, image positioning errors, and use of subjective visual methods. To overcome these limitations, a three-dimensional echocardiographic method was developed. This study compares ejection fraction by three-dimensional echocardiography, quantitative two-dimensional echocardiography, and subjective two-dimensional echocardiographic visual estimation with that by equilibrium radionuclide angiography. METHODS AND RESULTS: Fifty-one unselected patients with suspected heart disease underwent left ventricular ejection fraction determination by equilibrium radionuclide angiography and three-dimensional echocardiography using an interactive line-of-intersection display and a new algorithm, ventricular surface reconstruction, for volume computation. In 44 patients, ejection fractions were also estimated visually by experienced observers from two-dimensional echocardiography and by quantitative two-dimensional echocardiography using an apical biplane summation-of-disks algorithm. An excellent correlation was obtained between three-dimensional echocardiography and equilibrium radionuclide angiography (r = .94 to .97, SEE = 3.64% to 5.35%; limits of agreement, 10.3% to 13.3%) without significant underestimation or overestimation. SEE values and limits of agreement were twofold to threefold lower than corresponding values for all two-dimensional echocardiographic techniques. In addition, interobserver variability was significantly lower for the three-dimensional echocardiographic method (10.2%) than for the apical biplane summation-of-disks method (26.1%) and subjective visual estimation (33.3%). CONCLUSIONS: Determination of ejection fraction by three-dimensional echocardiography yields results comparable to those obtained by equilibrium radionuclide angiography and is substantially superior to all two-dimensional echocardiographic methods. Therefore, three-dimensional echocardiography may be used for accurate serial quantification of left ventricular function as an alternative to equilibrium radionuclide angiography.
