[Residual findings in retrobulbar neuritis during demyelinization].

Retrobulbar neuritis is a common clinical manifestation of multiple sclerosis. We evaluated and compared the results of visual field and fundus examinations, visual evoked response and nuclear magnetic resonance of the brain in 37 patients with the diagnosis of multiple sclerosis and retrobulbar neuritis. Diffuse, peripheral scotomas seemed to be the most often found scotomas on perimetry in these patients. In contrary, central scotomas formed just a little part of the visual field defects. Each threshold increasing of 0.10 log. units on perimetry was followed by increased latencies of 2.5 ms. Each other central scotoma on perimetry decreased the visual acuity of the patient for 1.6 lines on the Snellen chart. The central visual field examination without periphery in the patients with multiple sclerosis and retrobulbar neuritis seems to give false negative results.

[Masquerading syndromes].

Masquerade syndromes comprise a number of diseases imitating as to their clinical picture inflammation of the eye. These diseases comprise also malignant tumours. The authors present three patients where intraocular tumours (lymphoma, leukaemia, melanoma of the choroid) reminded of chronic uveitis.

[Behcet's disease].

The author evaluates diagnostic and therapeutic results in nine patients with Behcet's disease investigated at the uveological ambulance of the First Ophthalmological Clinic, Medical Faculty Charles University in Prague during 1987-1996. Anterior uveitis with hypopyon was recorded twice as frequently as retinal vasculitis. Chronic CME, secondary glaucoma and occlusive vasculitis were the most serious complications of ophthalmological manifestations of BD. Ulcerations of connective tissue and skin manifestations of BD were found most frequently in the authors patients. In two thirds of the patients also the risk phenotype HLA B5 was recorded.

[Anterior ischemic optic neuropathy and increased intraocular pressure].

PURPOSE: To find out the occurrence of an increased intraocular pressure (IOP) in the anterior ischemic optic neuropathy (AION). METHODS: We studied 17 patients (10 men, 7 women, mean age 62.8 years) with AION. We measured the IOP at the beginning of observation and during the systemic corticosteroid therapy. RESULTS: We found an increased IOP in 4 patients (23.5%). CONCLUSIONS: The occurrence of an increased IOP in AION patients is higher than in a normal population. The increased IOP seems to be one of possible risk factors in the pathogenesis of AION.

[Intermediate uveitis in clinical practice].

OBJECTIVE: Evaluation of experience with the diagnosis and treatment of intermediate uveitis in the Czech population. METHODS: The retrospective study summarizes the results assembled in 21 patients (37 eyes) investigated at the First Ophthalmological Clinic in 1988-1995. RESULTS: A diffuse type of inflammation was revealed in 15 patients (26 eyes), exsudative type in 6 patients (11 eyes). In three patients the inflammation did not call for treatment. Long-term remission was achieved in 3 patients by periocular injections of steroids, in 11 patients by systemic Prednisone treatment, in 4 patients by combined immunosuppression. In one patient cryotherapy was performed, in two patients PPV. In 31 eyes (84%) vision was the end of the observation period 6/12 better. CONCLUSION: From the results of the investigation ensues that intermediate uveitis occurs in our geographical zone more frequently in younger age groups and is manifested as a rule by a milder, diffuse type of inflammation. The prerequisite of successful patient care is early diagnosis and correct indication of treatment.

[Anterior ischemic optic neuropathy].

Anterior ischemic optic neuropathy is based on the lesion of blood supply to the optic nerve head. The disease presents decrease of visual acuity and altitudinal field defects with optic dics oedema in ophthalmoscopic view, which progresses in optic atrophy. Anatomy, pathogenesis, causes, clinical picture and management are discussed. 20% of patients from the group treated with vasodilatants and 50% of those treated with corticosteroids improved their visual acuity.

[Charlin's syndrome--nasociliary nerve neuralgia].

The history, etiopathogenetical factors, clinical symptomatology, differential diagnosis and therapy of neuralgia nervi nasociliaris--Charlin's syndrome are described. The authors give an account of an 18-years-old male patient.

[Biocompatibility of retropupillary intraocular lenses].

The authors give an account on the incidence of two types of reactions in the early postoperative period (within two weeks) after implantation of retropupilar intraocular lenses. In a group of 320 patients they observed them in 51 (16%) incl. 17 diabetics. In the group of diabetic patients the incidence of reactions was twice as frequent. A typical fibrin reaction starting on the third day after operation was recorded in 18 patients (5%), in the remaining 33 patients (11%) there was increased opalescence of the aqueous humour, and exudation of cels and cellular structures to the lens surface was associated with the surgical trauma, in particular in beginner implantologists. The two mentioned types of reactions do not have a substantial influence on the final functional effect.
