High frequency of methylenetetrahydrofolate reductase 677TT genotype in Hungarian HELLP syndrome patients determined by quantitative real-time PCR.

Our aim was to determine the methylenetetrahydrofolate reductase (MTHFR) C677T polymorphism in hypertensive disorders during pregnancy. We conducted our experiments on isolated DNA samples of 73 healthy pregnant, 101 severe pre-eclamptic and 63 HELLP syndrome women in this study. The MTHFR C677T polymorphism was determined by quantitative real-time PCR method. A significantly higher number of the TT genotype (25.4%) was found in the HELLP syndrome group compared to the healthy (8.2%) and severe pre-eclamptics group (8.9%) (P=0.03). The frequency of the mutant T allele was found to be 45.2% of HELLP syndrome, whereas it was 32.2% of the healthy pregnant (P=0.03) and 30.2% (P=0.008) of the severe pre-eclamptic patients. In the HELLP group a high frequency of eclampsia was observed (12.6%) and among them 75% had the MTHFR C677T mutation.

Congenital heart diseases in twin pregnancies.

OBJECTIVE: To find connection between the type of congenital heart malformations and twin pregnancies. METHOD: Retrospective analysis of data of fetal cardiology database between 1 January 1996 and 30 November 2003. RESULTS: In single pregnancies 455 and in twin pregnancies 31 severe congenital heart malformations were diagnosed prenatally. In monozygotic twin pregnancies 36% of heart malformations were pulmonary stenosis and 45% endocardial fibroelastosis, which is significantly higher than in single pregnancies. In dizygotic twin pregnancies Ebstein malformation was significantly more frequent than in single pregnancies. With the exception of Ebstein malformation in dichorionic and dizygotic twin pregnancies the cardiac malformations were similar to the ones in single pregnancies. CONCLUSIONS: The twin pregnancy alone can be considered as indication for fetal echocardiography. The type of congenital heart malformations detected in monochorial twin pregnancies was different from those found in single, dizygotic or dichorionic twin pregnancies. Chorionicity seems to be more important than zygosity.

Correlation between first polar body morphology and further embryo development.

First polar body (PB) morphology of human oocytes can indicate further embryo development and viability. However, controversial data have been published in this topic. Our retrospective study analyses the fertilization and further development of oocytes in relation to different morphological features of the first PB. The morphology of 3387 MII oocytes from 522 in vitro fertilization (IVF) treatments were assessed before intracytoplasmic sperm injection (ICSI). Oocytes were classified according to their first PB morphology. Assessment of fertilization and embryonic development (cell number, embryo grade, amount of anuclear fragmentation and presence of multinucleated blastomeres) was performed 16-20 and 42-48 hours after ICSI. Our results show that fertilization rate and embryo quality is influenced by PB morphology, while speed of development is not affected by the morphology of the first PB. Contrary to previous findings, our results suggest that oocytes with a fragmented PB had a higher developmental ability than those with an intact PB. However, we observed a lower viability of oocytes with a large PB. Since there are contradictions in this and previous observations, an extensive study is needed with standard hormonal stimulation protocol and oocyte evaluation criteria.

Hypogastric artery ligation for intractable pelvic hemorrhage.

OBJECTIVE: To assess the outcomes of bilateral hypogastric (internal iliac) ligation performed to control intractable pelvic hemorrhage and avoid hysterectomy. METHODS: A review of indications and outcomes for 117 cases of bilateral hypogastric artery ligation over 15 years (1990-2004). RESULTS: Apart from a slight lesion to the hypogastric vein, no complications were observed. Hemorrhage was effectively controlled in all 37 obstetric cases. In 13 of these cases, the uterus was preserved even when there was cervical pregnancy, placenta previa, placental abruption, uterine atony, and uterine rupture, and 4 women were delivered of mature infants. Hemorrhage was effectively controlled in 41 of 80 gynecologic cases. Prophylactic reduction of pelvic blood flow was the indication for the procedure in 39 cases, 5 of whom involving Jehovah's Witnesses adverse to blood transfusion. The uterus was preserved in only a few of the 41 controlled cases, but one woman (so far) was delivered of a mature infant. CONCLUSION: Hypogastric artery ligation was found to be indicated if (1) life-threatening pelvic hemorrhage could not be controlled by conservative methods; (2) prophylactic reduction of pelvic blood flow was needed to prevent anticipated hemorrhage; and (3) preservation of reproductive function was desired. The procedure was found to be safe and usually effective and should be taught during obstetric and gynecologic training.

Organochlorine contamination and physiological responses in nestling tree swallows in Point Pelee National Park, Canada.

Point Pelee National Park in southwestern Ontario, Canada--a major migratory route and vital breeding area for many birds--has localized areas of organochlorine (OC) contamination from the 1950s and 1960s. During 2002, we investigated the effect of tissue OC contaminant levels on the physiology and growth of nestling tree swallows (Tachycineta bicolor). Polychlorinated biphenyl (PCB) contamination in the nestlings positively correlated with hepatic ethoxy-, benzyloxy-, and pentoxyresorufin-o-dealkylase enzyme activities and liver size. Despite detectable physiologic changes associated with body burdens of PCBs, reproductive success of breeding pairs was not affected. Hatching date was negatively correlated with PCB levels, alkoxyresorufin-O-dealkylase (alkROD) activities, liver size, and serum protein levels. alkROD activities were largely dependent on hatching date because insect prey contaminated with PCBs were only abundant during a limited period of time during the breeding season.

Accumulation of three-body resonances above two-body thresholds.

We calculate resonances in the three-body system with attractive Coulomb potential by solving homogeneous Faddeev-Merkuriev integral equations for complex energies. The equations are solved using the Coulomb-Sturmian separable expansion method. This approach allows us to study the exact behavior of the three-body Coulomb systems near the threshold. A negatively charged positronium ion is used as a test case. In addition to locating all previously known S-wave resonances of the positronium ion, we also find a large number of new resonant states that accumulate just slightly above the two-body thresholds. The pattern of accumulation of resonant states above the two-body thresholds suggests that probably they are infinite in number. We conjecture that this may be a general property of the three-body system with an attractive Coulomb potential.

Extremely high maternal alkaline phosphatase serum concentration with syncytiotrophoblastic origin.

An extremely high alkaline phosphatase (AP) concentration (3609 IU/litre) was found in a 20 year old primigravida at 37 week's gestation, prompting an examination of its histological and cellular origin. Immunohistochemistry and western blots using antibodies against AP, Ki-67, phospho-protein kinase B (Akt), phospho-p44/42 mitogen activated protein kinase/extracellular signal regulated kinase 1/2 (MAPK/Erk1/2), phospho-glycogen synthase kinase-3beta (GSK-3beta), phospho-stress activated protein kinase/c-Jun N-terminal kinase, total-Akt, total-GSK-3beta, and phospho-p38-MAPK were carried out on index and control placental samples of the same gestational age. Compared with controls, staining of the index placenta showed minimal AP labelling of the brush border and remarkable positivity of the intervillous space. Cytotrophoblastic proliferation was 8-10% in the index placenta compared with 1-2% in controls. The index placenta also had raised concentrations of protein kinases with important roles in cell differentiation. The proliferation and differentiation rates of the cytotrophoblasts were found to be five times higher in index samples than in controls. It is hypothesised that loss of syncytial membranes in immature villi led to increased AP concentrations in the maternal circulation and decreased AP staining of the placenta. Loss of the syncytium might also stimulate increased proliferation of villous cytotrophoblasts, which would then fuse and maintain the syncytium.

Effects of Ca2+ -sensitizers in permeabilized cardiac myocytes from donor and end-stage failing human hearts.

During heart failure, alterations occur in contractile protein expression and phosphorylation, which may influence the effects of Ca2+ -sensitizers. To quantify the magnitude of these effects, isometric force was studied in mechanically isolated Triton-skinned myocytes from end-stage failing and non-failing donor hearts under control conditions (pH 7.2; no added inorganic phosphate (Pi)) and under mimicked ischemic conditions (pH 6.5; 10 mM Pi). Two different Ca2+ -sensitizers were used: EMD 53998 (10 microM), which exerts its influence through the actin-myosin interaction, and OR-1896 (10 microM) (the active metabolite of levosimendan), which affects the Ca2+ -sensory function of the thin filaments. The maximal force (Po) measured at saturating Ca2+ concentration and the resting force (Prest) determined in the virtual absence of Ca2+ (pCa 9) did not differ between the failing and non-failing myocytes, but the Ca2+ concentration required to induce the half-maximal force under control conditions was significantly lower in the failing than in the non-failing myocytes (DeltapCa50=0.15). This difference in Ca2+ -sensitivity, however, was abolished during mimicked ischemia. EMD 53998 increased Po and Prest by approximately 15% of Po and greatly enhanced the Ca2+ -sensitivity (DeltapCa50 > 0.25) of force production. OR-1896 did not affect Po and Prest, and provoked a small, but significant Ca2+ -sensitization (DeltapCa50 approximately 0.1). All of these effects were comparable in the donor and failing myocytes, but, in contrast with OR-1896, EMD 53998 considerably diminished the difference in the Ca2+ -sensitivities between the failing and non-failing myocytes. The action of Ca2+ -sensitizers under mimicked ischemic conditions was impaired to a similar degree in the donor and the failing myocytes. Our results indicate that the Ca2+ -activation of the myofibrillar system is altered in end-stage human heart failure. This modulates the effects of Ca2+ -sensitizers both under control and under mimicked ischemic conditions.

Diurnal and monthly variation of indoor radon and thoron progeny concentrations at a hillside place of northern India.

The natural radioactivity levels all over the world can create health problems due to the inhalation of radioactive aerosols. Radon and thoron progeny content of indoor air have major contribution to natural radiation dose. In the present study, simultaneous indoor measurements of radon and thoron progeny concentrations have been carried out over a period of four months at a low activity hilly area of northern Punjab, India to see their diurnal and monthly variations. The method used for this purpose is based on the defined solid angle absolute beta counting of radioactive aerosols sampled on a filter. The average values of equilibrium equivalent radon and thoron concentrations were 5.20 Bq m(-3) and 0.235 Bq m(-3), respectively, over the period August to November 2003. Both, EECRn and EECTh show a negative correlation with temperature.

Myosin light chain composition in non-failing donor and end-stage failing human ventricular myocardium.

The increased Ca(2+)-responsiveness in end-stage human heart failure cannot be attributed to contractile protein isoform changes, but rather is the complex resultant of changes in degree of phosphorylation of VLC-2 and TnI. Despite the decreased basal level of VLC-2 phosphorylation the response to VLC-2 dephosphorylation is enhanced in failing myocytes, which might result from differences in endogenous phosphorylation of thin and thick filament proteins between donor and failing hearts. Taken together decreased VLC-2 phosphorylation in end-stage human heart failure might represent a compensatory process leading to an improvement of myocardial contractility by opposing the detrimental effects of increased Ca(2+)-responsiveness of force and impaired Ca(2+)-handling on diastolic function.

Increased Ca2+-sensitivity of the contractile apparatus in end-stage human heart failure results from altered phosphorylation of contractile proteins.

OBJECTIVE: The alterations in contractile proteins underlying enhanced Ca(2+)-sensitivity of the contractile apparatus in end-stage failing human myocardium are still not resolved. In the present study an attempt was made to reveal to what extent protein alterations contribute to the increased Ca(2+)-responsiveness in human heart failure. METHODS: Isometric force and its Ca(2+)-sensitivity were studied in single left ventricular myocytes from non-failing donor (n=6) and end-stage failing (n=10) hearts. To elucidate which protein alterations contribute to the increased Ca(2+)-responsiveness isoform composition and phosphorylation status of contractile proteins were analysed by one- and two-dimensional gel electrophoresis and Western immunoblotting. RESULTS: Maximal tension did not differ between myocytes obtained from donor and failing hearts, while Ca(2+)-sensitivity of the contractile apparatus (pCa(50)) was significantly higher in failing myocardium (deltapCa(50)=0.17). Protein analysis indicated that neither re-expression of atrial light chain 1 and fetal troponin T (TnT) nor degradation of myosin light chains and troponin I (TnI) are responsible for the observed increase in Ca(2+)-responsiveness. An inverse correlation was found between pCa(50) and percentage of phosphorylated myosin light chain 2 (MLC-2), while phosphorylation of MLC-1 and TnT did not differ between donor and failing hearts. Incubation of myocytes with protein kinase A decreased Ca(2+)-sensitivity to a larger extent in failing (deltapCa(50)=0.20) than in donor (deltapCa(50)=0.03) myocytes, abolishing the difference in Ca(2+)-responsiveness. An increased percentage of dephosphorylated TnI was found in failing hearts, which significantly correlated with the enhanced Ca(2+)-responsiveness. CONCLUSIONS: The increased Ca(2+)-responsiveness of the contractile apparatus in end-stage failing human hearts cannot be explained by a shift in contractile protein isoforms, but results from the complex interplay between changes in the phosphorylation status of MLC-2 and TnI.

The effect of myosin light chain 2 dephosphorylation on Ca2+ -sensitivity of force is enhanced in failing human hearts.

OBJECTIVE: Phosphorylation of the myosin light chain 2 (MLC-2) isoform expressed as a percentage of total MLC-2 was decreased in failing (21.1+/-2.0%) compared to donor (31.9+/-4.8%) hearts. To assess the functional implications of this change, we compared the effects of MLC-2 dephosphorylation on force development in failing and non-failing (donor) human hearts. METHODS: Cooperative effects in isometric force and rate of force redevelopment (K(tr)) were studied in single Triton-skinned human cardiomyocytes at various [Ca(2+)] before and after protein phosphatase-1 (PP-1) incubation. RESULTS: Maximum force and K(tr) values did not differ between failing and donor hearts, but Ca(2+)-sensitivity of force (pCa(50)) was significantly higher in failing myocardium (Deltap Ca(50)=0.17). K(tr) decreased with decreasing [Ca(2+)], although this decrease was less in failing than in donor hearts. Incubation of the myocytes with PP-1 (0.5 U/ml; 60 min) decreased pCa(50) to a larger extent in failing (0.20 pCa units) than in donor cardiomyocytes (0.10 pCa units). A decrease in absolute K(tr) values was found after PP-1 in failing and donor myocytes, while the shape of the K(tr)-Ca(2+) relationships remained unaltered. CONCLUSIONS: Surprisingly, the contractile response to MLC-2 dephosphorylation is enhanced in failing hearts, despite the reduced level of basal MLC-2 phosphorylation. The enhanced response to MLC-2 dephosphorylation in failing myocytes might result from differences in basal phosphorylation of other thin and thick filament proteins between donor and failing hearts. Regulation of Ca(2+)-sensitivity via MLC-2 phosphorylation may be a potential compensatory mechanism to reverse the detrimental effects of increased Ca(2+)-sensitivity and impaired Ca(2+)-handling on diastolic function in human heart failure.

The evolving role of ultrasound in obstetrics/gynecology practice.

The value of ultrasound examinations depends heavily on the preparation of the personnel carrying out the examination and the technical capabilities of the equipment they use. Only well-organized regional or national programs are able to provide high level, cost-effective care based on certification of quality. Such certification must include the training of professionals, the definition of competence levels, accreditation of laboratories and the establishment of professional protocols. Together, these factors can guarantee the standard of care and provide legal protection for practitioners. It is worth carrying out routine screening in each pregnancy because the majority of abnormalities occur in pregnancies with low risk. Abnormalities detected on screening cases and the examination of high risk groups should be referred to higher level centers. Here, appropriate technical background and qualified personnel are present to provide cost-effective care. At the same time, necessary invasive interventions can also be performed. A minimum of three screening tests should be performed during pregnancy. The first should be performed at the fetal age of 10-14 weeks to detect abnormalities and pathological conditions in early pregnancy. The second one has to be performed between the fetal ages of 18 and 22 weeks to assess detailed fetal anatomy and rate of development. The third should be performed between the fetal age of 30 and 34 weeks to assess fetal anatomy, rate of development, placentation and circulation. It is worth considering a fourth screening at approximately 36-38 weeks to assess the intrauterine condition of the fetus and determine the appropriate method of delivery. Finally, besides improving the standard of living, education, and hygienic conditions in developing countries, developed countries also have to help improve the standard of pregnancy care. Within this context, the dissemination of diagnostic ultrasound must be given special emphasis.

Estimate of the dose-increment due to outdoor exposure to gamma rays from uranium progeny deposited on the soil around a coal-fired power plant in Ajka Town, Hungary.

Brown coal unusually rich in uranium is burnt in a coal-fired power plant that lies inside the confines of a small industrial town named Ajka, Hungary, and has been operational since 1943. The 238U (226Ra) activity discharged to the atmosphere per unit electrical energy produced was about 330-400 GBq (GW y)(-1), which is 66-80 times more than that was estimated by UNSCEAR (1988) as a characteristic value for old type coal-fired power plants [5 GBq (GW y)(-1)]. The objective of this study was the experimentally established assessment of the artificial increment in the dose from external exposure to gamma rays of terrestrial radionuclides outdoors. Soil samples were collected in and near Ajka from 81 locations. The samples were investigated by Ge(Li) gamma spectrometry. Considerably elevated concentrations of uranium and its progeny have been measured in most of the samples that were collected near to the plant. Concentrations of 238U and 226Ra in the top (0-5 cm depth) layer of undisturbed soil at public areas inside town were 4.7 times higher, on average, than those in the uncontaminated deeper layers. Dose rate in air (air kerma) from external exposure to terrestrial gamma rays outdoors at a height of 1 m and effective doses were estimated from the measured activity concentrations using some relevant literature data. The estimated artificial increment in the dose rate in air was, on average, 32.8, 10.3, and 102.1 nGy h(-1) at public areas, vegetable gardens, and backyards, respectively. The mean artificial increment in the annual per caput effective dose from external exposure to terrestrial radionuclides outdoors is 21.8 microSv y(-1). The collective dose commitment per unit energy generated from outdoor exposure to the deposited uranium progeny is about 8.0-9.1 person Sv (GW y)(-1), which is 67-76 times more than that evaluated by UNSCEAR (1988) for a typical "old" coal-fired power plant [0.12 person Sv (GW y)(-1)]. Ajka is a suitable place for studying the dosimetric consequences of the utilization of coal for energy production experimentally.

Adjuvant chemotherapy following surgery in the management of uterine sarcomas.

OBJECTIVE: The aim of this study was to investigate the use of imaging tools in the diagnosis of uterine sarcomas, and to evaluate the effect of the adjuvant chemotherapy for uterine sarcomas. PATIENTS AND METHODS: The data of 29 patients with uterine sarcomas who received cytostatic polychemotherapy between 1990 and 2000 at the Oncological Division of the Ist Department of Obstetrics and Gynecology, Semmelweis University were evaluated by the authors. Symptoms leading to diagnosis and methods of diagnosis were examined. Vascular changes shown by two-dimensional, color and pulsed Doppler ultrasonography were observed. For staging the currently accepted FIGO method was adopted. Most of the patients underwent total abdominal hysterectomy and bilateral salpingo-oophorectomy (TAH & BSO). In each case we administered adjuvant combination chemotherapy according to the CYVADIC-protocol. The effect of adjuvant chemotherapy was evaluated. RESULTS: Six patients had Stage I, ten had Stage II, 11 had Stage III, and two had Stage IV disease. The mean age of the patients was 53.6 years with a range of 22 to 77 years. Histopathologic distribution included nine leiomyosarcomas (LMS), 13 mixed mesodermal sarcomas (MMS), and seven endometrial stromal sarcomas (ESS). Although most patients experienced neutropenia following cytotoxic chemotherapy, other non-hematologic adverse effects were easy to control. The average progression-free interval was 22.14 months, in which no significant difference was found between the histologic types. Different stages showed highly varied responses: surprisingly, patients in Stage IV with lung metastases were documented to have the longest progression-free survival. The three-year survival rate for all stages was demonstrated in 34.4% of cases. Patients with progressive disease had an average survival period of 4.4 months. CONCLUSIONS: These findings suggest that adjuvant cytostatic therapy for patients with distant metastasis confined to a single organ may produce better results than expected.

The cardiotonic effects of levosimendan in guinea pig hearts are modulated by beta-adrenergic stimulation.

The effects of the Ca2+-sensitiser levosimendan alone or in combination with beta-adrenergic stimulation on the contractile function were studied in various guinea pig cardiac preparations. Echocardiography in narcotised animals indicated that a maximal dose of levosimendan (50 microg x kg(-1)) increased the left ventricular posterior wall movement velocity during systoles and diastoles by 25 +/- 3% (mean +/- S.E.M.) and 17 +/- 2%, respectively. In Langendorff hearts, a saturating concentration of levosimendan (0.3 micromol x l(-1) for 5 min) increased +dP/dt(max) and dP/dt(max) by 28 +/- 3% and 14 +/- 2%, respectively. Further, the Ca2+-sensitising potential of levosimendan in Triton-skinned cardiomyocytes (EC50: 5 +/- 3 nmol x l(-1)) was illustrated by a maximal increase in the isometric force production by 51 +/- 5% (at pCa 6.2). However, following stimulation by isoproterenol, when the level of troponin I phosphorylation was elevated, no significant additional increase in the contractile parameters could be demonstrated upon levosimendan administration. Moreover, the levosimendan-induced increase in force production in isolated skinned myocytes could be prevented by incubation with the catalytic subunit of protein kinase A (100 U x ml(-1) for 40 min). These data indicate that thin filament-targeted Ca2+-sensitisation by levosimendan is modulated by phosphorylation of the contractile filaments, an effect that should be considered during combination therapy with levosimendan.

Calcium sensitivity of force in human ventricular cardiomyocytes from donor and failing hearts.

In failing human myocardium changes occur, in particular, in isoform composition and phosphorylation level of the troponin T (TnT) and troponin I (TnI) subunits of the actin filament and the myosin light chains (MLC-1 and -2), but it is unclear to what extent they influence cardiac performance. This overview concentrates on the relation between contractile function, contractile protein composition and phosphorylation levels in small biopsies from control (donor) hearts, from biopsies obtained during open heart surgery (NYHA Class I-IV) and from end-stage failing (explanted, NYHA class IV) hearts. Furthermore, attention is paid to the effect of the catalytic subunit of protein kinase A on isometric force development in single Triton-skinned human cardiomyocytes isolated from donor and end-stage failing left ventricular myocardium at different resting sarcomere lengths. A reduction in sarcomere length from 2.2 to 1.8 microm caused reductions in maximum isometric force by approximately 35% both in donor and in failing cardiomyocytes. The midpoints of the calcium sensitivity curves (pCa50) of donor and end-stage failing hearts differed markedly at all sarcomere lengths (mean delta pCa50 = 0.22). Our findings indicate that 1) TnI phosphorylation contributes to the differences in calcium sensitivity between donor and end-stage failing hearts, 2) human ventricular myocardium is heterogeneous with respect of the phosphorylation of TnT, MLC-2 and the isoform distribution of MLC-1 and MLC-2, and 3) the Frank-Starling mechanism is preserved in end-stage failing myocardium.

Significant radioactive contamination of soil around a coal-fired thermal power plant.

Soil samples were collected around a coal-fired power plant from 81 different locations. Brown coal, unusually rich in uranium, is burnt in this plant that lies inside the confines of a small industrial town and has been operational since 1943. Activity concentrations of the radionuclides 238U, 226Ra, 232Th, 137Cs and 40K were determined in the samples. Considerably elevated concentrations of 238U and 226Ra have been found in most samples collected within the inhabited area. Concentrations of 235U and 226Ra in soil decreased regularly with increasing depth at many locations, which can be explained by fly-ash fallout. Concentrations of 235U and 226Ra in the top (0-5 cm depth) layer of soil in public areas inside the town are 4.7 times higher, on average, than those in the uncontaminated deeper layers, which means there is about 108 Bq kg(-1) surplus activity concentration above the geological background. A high emanation rate of 222Rn from the contaminated soil layers and significant disequilibrium between 238U and 226Ra activities in some kinds of samples have been found.
