Ileocolic anastomotic ulcer after surgery in adulthood: case report and review of the literature.

Anastomotic ulcer is a rare complication after ileocolic resection, especially in adults without evidence of inflammatory bowel disease or tumor recurrence. We report the case of a 63-year-old male patient who presented 6 years after ileocolic resection, and also review the data of six similar cases described in the literature. The markedly reduced bile acid absorption found in our case raises the possibility of an etiological role of bile acids in the development of ileocolic anastomotic ulcers.

[Adult-onset Kawasaki syndrome in the differential diagnosis of liver disease].

The authors present a case of an adult with Kawasaki syndrome, who, due to jaundice, enlarged liver and abnormal liver function tests, was admitted the hospital with the suspicion of liver disease. The symptoms of Kawasaki syndrome appeared during the first nine days of the hospital stay. The authors emphasise, that liver function tests are frequently abnormal in adults presenting with this clinical entity, therefore Kawasaki syndrome should be taken into consideration in the differential diagnosis of liver diseases.

[Nipple transposition on superomedial pedicle in reduction mammoplasties].

BACKGROUND: Transposing the nipple-areola complex is a challenge at reduction mammaplasties, particularly when the distance between the sites is big and great amount of breast tissue has to be removed. Many techniques are used, all have to fulfill the following conditions: the blood supply and sensation of the nipple have to be preserved and closure must be tension-free. OBJECTIVE: To evaluate how the superomedial pedicle for nipple transposition fulfills the above requirements. METHOD: Fifteen reduction mammaplasties performed in this way (resection weight: 435 g/breast), postoperative patient examinations with emphasis on sensation. RESULTS: Nipple viability was good without necrosis, retaining sensation was also good with no loss of sensation. CONCLUSION: The technique is safe, giving good cosmetic results.

[Symptoms of fungal esophagitis in alcoholic liver disease].

Symptoms related to fungal esophagitis were studied in patients with alcoholic liver disease who underwent upper gastrointestinal endoscopy. Data of 517 patients were studied retrospectively (group I) and 100 alcoholic liver disease patients, that were successively admitted to hospital, were enrolled in the prospective part (group II). Out of the 41 cases with fungal esophagitis found in group I, data of 38 could be evaluated. In group II 13 of the 93 evaluable patients had fungal esophagitis; according to Kodsi's grading 10 patients had grade 1., one patient grade 2. and two patients grade 2-3. oesophagitis. There was no case with grade 4. esophagitis. The rate of symptoms among the 51 patients with fungal esophagitis was: anorexia 23 (45.0%), abdominal pain 22 (43.1%), vomiting 17 (33.3%), nausea 15 (29.4%), occult gastrointestinal bleeding 12 (23.5%), weight loss 9 (17.6%), melena 7 (13.7%), bloating 6 (11.7%), acidic regurgitation 3 (5.8%), haematemesis 2 (3.9%), thoracic pain 2 (3.9%), singultus 1 (1.9%), odynophagia 0 and dysphagia 0. In 7 patients (13.7%) none of the studied symptoms could be identified. Despite the relatively high frequency of symptom free fungal esophagitis reported in the literature, the total lack of odynophagia and dysphagia in our patient group was remarkable. In the lack of deglutition disorders the other symptoms do not raise the suspicion of esophagitis. The diagnosis in such cases can be established only by endoscopy.

Fungal colonization of the esophagus in alcoholic liver disease.

OBJECTIVES: Little is known about the fungal colonization of the esophagus. Since alcoholic liver disease (ALD) patients are prone to fungal esophagitis, we have investigated the esophageal fungal colonization of this patient group. METHODS: One hundred consecutive ALD patients were enrolled in this prospective study. 22 patients with dyspeptic symptoms acted as controls. After taking an oropharyngeal swab, patients underwent an upper gastrointestinal endoscopy and surface material was obtained from the esophagus for direct smears and culture. RESULTS: In the ALD group pseudohyphae were found in 21.5% and yeast forms in 6.4% of the direct smears. The culture was positive in 40.8% of the ALD patients, the isolated strains were: 30 C. albicans, 2 C. kefyr, 2 C. krusei, 1 C. zeylanoides and in 3 cases the species could not be identified. 41.9% of ALD patients and 13.6% of control patients (p = 0.013) had fungi in their esophagus. Significantly more ALD patients had fungal esophagitis than in the control group (19.3% vs. 0%, p = 0.021), the rate of fungal colonization was also higher, but the difference was not significant (22.5% vs. 13.6%). A significantly higher rate of fungal esophagitis and esophageal colonization was found in patients with fungi in their oropharyngeal swabs (p = 0.00001). CONCLUSIONS: Fungal colonization of the esophagus is frequent in ALD patients. Its presence might have clinical significance in the case of liver transplantation.

[Data on the epidemiology of head and neck neoplasms in Hungary].

Oral and pharyngeal cancer mortality of the male population increases in the fastest rate in Hungary. Both oral and pharyngeal as well as laryngeal cancer mortality is the highest in Europe for both sexes. The leading risk factors of them are smoking and drinking together. The per capitam cigarette and alcohol consumption of Hungarian population belong also among the highest in Europe. The authors have examined these damaging habits taking in account the collected data of Hungarian patient's group, the presence of them as risk factors have been compared to the smoking and alcohol consuming habits of the average population collected by microcensus data. Only 4 patients were nonsmokers and nondrinkers among the 145 patients who were interviewed. On the other hand from the same number of the population with the same sex and age distribution 73 did not smoke and drink while 123 patients were smokers and drinkers against the 48 from the population mentioned before. They have found that the two groups have diverged at a level 0.1% significance. Taking in account the fact that the number of patients is relatively low, further data collection is carried on in order to analyse in a more detailed way, but they regard it as proved, according to the recent tests the serious risk role of the two factors for the Hungarian population.

[Cancer screening in Hungary: World Bank supported model programs].

Since 1995, a model cancer screening program has been in operation in Hungary, the overall purpose of which is to promote the establishment of effective and efficient screening programs by means of adapting the internationally agreed principles of organized screening to the needs and opportunities in Hungary. The establishment and operation of a national population-based cancer registration system is an other aim of the Program. The model program--financed partly from a loan from the World Bank, partly from local funds provided by the Government of Hungary--is to develop standard procedure for cervical, breast and colorectal screening and to end up with tested recommendations for introduction of organized screening of proved effectiveness, integrated into the health care system, on country-wide service bases in Hungary.

[Experience with endoscopic diagnosis and treatment of acute biliary pancreatitis].

Acute biliary pancreatitis may be a grave illness with a fatal outcome in some instances. Its etiology is unclear so far. The diagnosis is difficult even with the help of conventional diagnostic methods and the most up to date imaging techniques. The most sensitive method among laboratory investigations is the elevation of ALT (serum alanine transferase) while endoscopic cholangiopancreatography has the highest accuracy rate of invasive diagnostic procedures. There is a chance of spontaneous remission in cases with mild symptoms while invasive measures-endoscopy or surgery-are needed in cases with grave symptoms. In the present study we evaluate 23 of our cases.

Comparison of single-dose and 7-day fluconazole treatment of fungal esophagitis in alcoholic liver disease.

The aim of the study was to assess the effectiveness of a single-dose fluconazole treatment of fungal esophagitis in patients with alcoholic liver disease. Twenty-two alcoholic liver disease patients with fungal esophagitis were randomly assigned to receive either a single-dose of 150 mg fluconazole or a 7-day treatment of daily 50 mg fluconazole. Control esophagoscopy was performed in both groups on days 9-11. Direct smears and cultures on Sabouraud's medium were performed at both endoscopies. Patients' sera were tested for Candida antigens and for antibodies against Candida albicans on days 1, 8 and 15. Twenty patients (18 C. albicans, 1 C. tropicalis, 1 C. pseudotropicalis) completed the study, there were two drop-outs from the single-dose group. Antibodies against C. albicans were found in four cases, Candida antigens in five. There were no significant differences in the treatment outcome between the two groups, clinical cure was recorded in eight out of nine patients in the single-dose group and nine out of eleven patients in the 7-day group, mycological eradication in four out of nine, and in three out of eleven, respectively. Single-dose fluconazole treatment seems to be an effective therapy of fungal esophagitis in alcoholic liver disease patients.

[Diagnostic value of abdominal ultrasonography and endoscopic retrograde cholangiopancreatography in obstructive jaundice].

The diagnostic value of ultrasonography (US) and endoscopic retrograde cholangio-pancreatography (ERCP) was compared, in 66 patients with bile duct obstruction, who underwent subsequent biliary surgery. The level of the blockage was diagnosed by US with a 80%, by ERCP with a 95.4% sensitivity, while the cause of the obstruction was determined with a 50% and a 89% sensitivity, respectively. Predictive value of these examinations is over 90%. Based on the clinical results and on the high diagnostic value of the above mentioned examinations, it is essential to fill up the bile ducts with direct contrast material.

[Experimental studies relative to the persistence of several facultative pathogenic fungi in river water].

Under laboratory conditions survival time of 3 fungi species (Candida albicans, Geotrichum candidum, Rhodotorula rubra) in river water of various quality and temperature has been studied. The mean (and the maximum) survival time was as follows: 82 (240) d for C. albicans, 210 (578) d for G. candidum and 333 (606) d for R. rubra. Differences concerning survival time were recorded in dependence on temperature and quality of water. All the 3 strains were multiplied periodically. Variants of culture character could be noticed during the experiments. The flora of association seems to have a considerable influence on the survival time of the fungi studied. The very long persistence of fungi enables them to become widespread by means of water, and in such a way that they pollute environment.

Problems of staging colorectal cancer.

A total of 2994 cases of previously untreated colorectal carcinomas taken from the database of the International Cancer Patient Data Exchange System of the UICC were analyzed. The objective was to compare pretherapeutic and postsurgical data and to investigate the impact of missing information on the quality of tumor staging under routine conditions. In clinical staging, a higher percentage of items were marked "unknown" in the questionnaires than in histopathological staging. The clinical and postoperative assessment of TNM stages Ib, II, and III frequently diverged, indicating a low precision for clinical staging. Fistulae were rarely observed in either colon or rectum carcinoma.

["Rare localisation" of bone metastases by patients with primary mamma carcinoma (author's transl)].

Authors report on bone metastases by 8 female patients with mamma carcinoma rarely occurring according to the literature. By the first patient a metastasis to the zygomatic bone and to the maxilla, by a second to the corpus mandibuale, by a third to the ascending branch of mandibula was encountered. The fourth patient showed metastasis at the distal end of radius and in the carpal bones, the fifth in the midthird part of tibial diaphysis, the sixth in the I, and II, cuneiform bone, the seventh in the carpal and metacarpal, tarsal and metatarsal bones respectively, the eighth in the calcaneus. In all the above cases previously metastases to other bones (as spine, pelvis etc.) have been noted. This means that in course of the disease the metastases of "rare localisation" appear in a later period.

Experimental studies on the persistence in distilled water of certain conditional pathogenic fungi.

The authors followed up the behaviour in distilled water of 8 fungus strains belonging to four species (Candida albicans, C. krusei, Geotrichum candidum and Rhodotorula) kept at various temperatures (i.e. +4 degrees, +18 degrees and 25 degrees C). The average and the maximum survival periods were found to be as follows: C. albicans--476/1138 days, C. krusei--411/1138 days, G. candidum--127/219 days, and R. rubra--139/226 days. All strains revealed periodically multiplication bursts of variable intensity depending on the kind of species and the temperature at which the cultures were maintained. These findings plead for the existence of cryptic periods of growth in fungi, too. Although, during the experiments, the variants occurred as concerns the morphological and cultural characteristics, sugar fermentation, sugar and nitrate assimilation, germ tube formation did not undergo any modifications. The pathogenic characteristics of C. albicans strains for rabbit were retained even after 1096 days maintenance in distilled water at 18 degrees C.

[A simple method for testing basic cytotoxicity in vitro].

The authors have recommended a simple method for further trial, of in vitro cytotoxicity test, for screening examinations, in order to complete in vivo methods or use it as an alternative technique. Briefly, it is as follows: the substance studied is treated in bidistilled water with the suspension of a well-known living cells of Candida strain, and then the changing of the number of surviving cells is followed up daily. The result is expressed as a percentage relation of the initial number of living cells, in survival curves in comparison with the controls. The number of surviving cells is inversely proportional to the toxicity of the substance studied. The method is very simple and cheap.
