Evaluation of a bromocresol purple method for the determination of albumin adapted to the DuPont aca discrete clinical analyzer.

We evaluated a new method for the determination of albumin on the DuPont aca discrete clinical analyzer that utilized an albumin selective dye, bromocresol purple (BCP). This dye minimizes globulin interference that occurs with bromocresol green (BCG) methods that have long (greater than 30 s) incubation times. Good correlation was observed between the new BCP method and two methods which do not show significant globulin interference: an immunological method and a rapid BCG method. Comparisons with alternate BCG methods (SMA 12/60, SMAC, and the aca) were generally not as good. The albumin results by the BCP method on specimens with elevated globulins were similar to results from the immunological and rapid BCG methods. The reference interval was 34-50 g/L. The between-day coefficient of variation ranged from 0.9 to 3.5% at three evaluation sites on 2 levels of 8 different control materials. The method was linear between 6-70 g/L. No interference was observed from hemoglobin at levels less than or equal to 5.00 g/L or from bilirubin at levels less than 342 mumol/L. Also, no interference was observed from nine common drugs that are known to bind to albumin. These studies show that accuracy, reproducibility and linearity of the BCP albumin method on the aca are acceptable for clinical use.

Studies of adsorbents for hemoperfusion in artificial liver support. I. Preparation and in vitro studies of cross-linked agarose beads entrapped activated charcoal (CAAC).

A new method for the preparation of the cross-linked agarose beads entrapped activated charcoal (CAAC) is reported. Since the agarose-encapsulated adsorbents reported elsewhere cannot stand high temperature for sterilization, the CAAC has the advantage of thermal stability to withstand autoclave at 121 degrees C. for 1/2 hour without breaking up or melting. A further advantage of CAAC is that the adsorbent has a much better consistency with good mechanical strength and elasticity, so that it can be formed into beads of a diameter less than 1 mm. This will not only give a better adsorption capacity than larger beads, but can also assure a better blood flow than soft beads which usually interfere in hemoperfusion due to compacting and sludging. Preliminary investigations indicate that the CAAC is relatively hemocompatible.

Three-dimensional quantitative structure-activity relationship analysis of the new potent sulfonylureas using comparative molecular similarity indices analysis.

The present study describes the implementation of a new three-dimensional quantitative structure-activity relationship (3D-QSAR) technique: comparative molecular similarity indices analysis (CoMSIA) to a set of novel herbicidal sulfonylureas targeted acetolactate synthase. Field expressions in terms of similarity indices in CoMSIA were applied instead of the usually used Lennard-Jones and Coulomb-type potentials in CoMFA. Two different kinds of alignment techniques including field-fit alignment and atom-by-atom fits were used to produce the molecular aggregate. The results indicated that those two alignment rules generated comparative 3D-QSAR models with similar statistical significance. However, from the predictive ability of the test set, the models from the alignment after maximal steric and electrostatic optimization were slightly better than those from the simple atom-by-atom fits. Moreover, systematic variations of some parameters in CoMSIA were performed to search the best 3D-QSAR model. A significant cross-validated q2 was obtained, indicating the predictive potential of the model for the untested compounds; meanwhile the predicted biological activities of the five compounds in the test set were in good agreement with the experimental values. The CoMSIA coefficient contour plots identified several key features explaining the wide range of activities, which were very valuable for us in tracing the properties that really matter and getting insight into the potential mechanisms of the intermolecular interactions between inhibitor and receptor, especially with respect to the design of new compounds.

The rating form of IBD patient concerns: a new measure of health status.

Health status assessment for persons with chronic illness includes not only symptoms, but also an appraisal of the psychosocial concomitants of illness. In this national study of persons with inflammatory bowel disease (IBD), we standardized a disease-specific 25-item measure of perceived health status: the Rating Form of IBD Patient Concerns (RFIPC). Factor analysis yielded four indices: a) impact of disease (e.g., being a burden, loss of energy, loss of bowel control); b) sexual intimacy; c) complications of disease (e.g., developing cancer, having surgery, dying early); and d) body stigma (e.g., feeling dirty or smelly). A higher level of IBD concerns was associated with greater disease severity, female gender, and lower educational status. When controlling for these factors, as well as disease type and age, we found that concerns about: a) impact of disease was positively associated with poorer perception of health and well-being, greater psychological distress (SCL-90), and poorer daily function (Sickness Impact Profile) (p less than 0.0001); b) sexual intimacy was related to poorer psychologic function (p less than 0.01); and c) complications of disease was related to several measures of poorer daily function (p less than 0.0001 to 0.01). This standardized measure of the worries and concerns of persons with IBD may be used in clinical care and research to evaluate the effects of interventions on IBD patient outcomes.

Preparation and performance of cross-linked agar encapsulated activated charcoal (CAAC-III).

A technique is described to encapsulate activated charcoal for hemoperfusion to be used in an artificial liver support. Spherical activated charcoal was coated with agar and shaped in organic solvent, as well as cross-linked by epichlorohydrin (40-50 degrees C). The product withstood autoclaving at 121 degrees C for 0.5 hours, and had a good adsorption ability for compounds with molecular weight ranging from 113-5200. The adsorption kinetic showed pore diffusion limitation to compounds of small molecular weight, and film diffusion limitation to compounds of large molecular weight. Microparticles of charcoal release were greatly reduced, and within the permissible range according to the requirements in the U.S. Pharmacopeia (1985).
