Effect of rhein on glucose transporter-1 expression and its function in glomerular mesangial cells.

OBJECTIVE: To explore the effect of rhein on the regulation of glucose transporter-1 (GLUT1) overexpression and the possible molecular mechanism that rhein antagonize the effect of transforming growth factor beta 1 (TGF-beta 1) in glomerular mesangial cells. METHODS: Cultured mouse mesangial cells were used. The expression of GLUT1 mRNA was detected by Northern blotting; the ability of glucose uptake was determined by 2-deoxy-[3H]-D-glucose uptake assay. RESULTS: Rhein had no effect on glucose uptake in mesangial cells cultured in normal glucose concentration. TGF-beta 1 could upregulate the expression of GLUT1 mRNA and glucose uptake in mesangial cells. This effect was markedly attenuated by the addition of rhein in a dose dependent manner. CONCLUSIONS: TGF-beta 1 could upregulate the expression of GLUT1 mRNA and glucose uptake in mesangial cells, resulting in excessive glucose consumption and extracellular matrix production in diabetic nephropathy. Rhein antagonized the effect of TGF-beta 1 in mesangial cells, so it might be a hopeful remedy for the treatment of patients with diabetic nephropathy.

Significance of anti-neutrophil cytoplasmic antibodies in patients with pauci-immune crescentic glomerulonephritis.

OBJECTIVE: To evaluate the significance of serum antineutrophil cytoplasmic antibodies (ANCAs) and the effects of immune suppressive treatments on its activity in patients with pauci-immune crescentic glomerulonephritis (PICGN). METHODS: Serum ANCAs and myeloperoxidase (MPO)-ANCA were detected by indirect immunofluorescence and enzyme-linked immunosorbent assay (ELISA) methods respectively, and the renal tissues infiltrating cells including CD4+, CD8+, CD68+ and PCNA+ cells were determined by four-layer peroxidase-antiperoxidase (PAP) method. The clinical manifestations, pathologic features and immune pathologic changes in patients with positive ANCAs were compared with that in patients with negative ANCAs. The effects of immune suppressive therapy on clinic and pathologic changes as well as ANCAs activity were also investigated in ANCAs positive and ANCAs negative patients. RESULTS: Both of clinic active manifestations such as the degree of hematuria, rapidly progressive renal failure and pathologic active features including segmental capillary necrosis and vasculitis were much common in patients with positive ANCAs as compared with that in patients with negative ANCAs. The number of infiltrating cells in renal tissue, especially CD4+ cells, was markedly higher in ANCAs positive patients than that in ANCAs negative patients. The effects of immune suppressive therapy were also much better in patients with positive ANCAs than that in patients with negative ANCAs. CONCLUSION: Serum ANCAs is not only a marker for diagnosis of systemic vasculitis, but also a sensitive predictor for evaluation of diseases' activity and treatment in patients with PICGN. The good effect of immune suppressive treatments on patients with PICGN is partially associated with the degree of ANCAs activity.

[Analysis of clinical characteristics and drug sensitivity tests of lower respiratory tract infection by Xanthomonas maltophilia].

OBJECTIVE: To analyse the clinical characteristics of lower respiratory tract infection caused by Xanthomonas maltophilia and to investigate the antibiotic sensitivity of Xanthomonas maltophilia strains. METHODS: Retrospective study of the clinical data of 54 cases with lower respiratory tract infection by Xanthomonas maltophilia, including risk factors of morbidity, clinical symptoms and signs, X-ray findings, blood routine test, treatment and prognosis. Drug sensitivity against strains of Xanthomonas maltophilia by K-B method was studied. RESULTS: There were 39 males and 15 females, the mean age being 51 +/- 17 years. 87% of the cases had underlying diseases, most of which were COPD complicated by respiratory failure. 57% of the cases were immunocompromised. 39% of the cases were in ICU or CCU. 54% of the cases accepted invasive treatments, and 93% of the cases were given broad-spectrum antibiotics. Clinical manifestations include chill (72%), fever (80%), cough (94%) and expectoration (91%). The chest X-ray revealed infiltration in lower lobes of both lungs. 16 cases had consolidations, and 11 cases were complicated with pleural effusions. The drug sensitivity test in vitro showed that these strains were multiresistant to commonly used antibiotics, and drugs whose sensitive rate were over 50% included SMZco, ceftazidine, and timentin. CONCLUSIONS: The lower respiratory tract infections caused by Xanthomonas maltophilia develop at patients with various underlying diseases, especially in the immunocompromised patients. Risk factors of morbidity were: patients in ICU or CCU, acceptance of invasive treatment and inappropriate use of broad-spectrum antibiotics. Clinical manifestations include severely toxic symptoms and some cases had pulmonary consolidations and pleural effusions.

[Cloning of differentiation-related genes induced by all-trans retinoic acid(ATRA) from human lung cancer GLC-82 cell line].

OBJECTIVE: Cloning of differentiation-related genes induced by all-trans retinoic acid(ATRA) from human lung cancer GLC-82 cell line. METHODS: The difference in gene expression between human lung cancer cell GLC-82 before and 8, 24 hours and 4 days after ATRA treatment was investigated by an improved protocol of mRNA differential display with SYBR Green I, a highly sensitive fluorescent stain. RESULTS: There were obvious differences in gene expression between human lung cancer cells before and after ATRA treatment. The expression of some genes was decreased or inhibited while that of other genes was activated temporarily or persistently. Three cDNA fragments of the differentially expressed genes were cloned and analyzed. CONCLUSION: ATRA plays an important role in regulating the differentiation-related gene expression. ATRA-induced differentiation is a complex process with multiple genes involved.

[Collagen-binding proteins of human oral spirochetes].

OBJECTIVE: To evaluate pathogenic effect of the collagen-binding proteins(CBP) of human oral spirochetes. METHODS: Immunoblot analysis, immunoelectron microscopy and other experiments had been used. RESULTS: It was showed that: type I, IV and V CBP were detected in heated and unheated preparations from Treponema denticola(Td) ATCC 33520 and Treponema socranskii (Ts) ATCC 35534. Few CBP, however, were detected in heated and unheated preparations from a recently characterized isloate, Treponema medium(Tm) strain G7201. 27,000 type V CBP of Td, 110,000 type I of Ts and 95,000 type IV of Tm were located on the envelopes of the individual cells. The adherence of Td to the collagen-coated surface was significantly greater than that of Tm. CONCLUSION: The CBP on the oral spirochetal cells play an important role in their adherence to collagen-rich connective tissues of the host.

[A study on the effect of house dust mite on interleukin-10 secretion of peripheral blood lymphocytes in patients with allergic asthma].

OBJECTIVE: To investigate the relationship between interleukin (IL)-10 and allergic asthma, and the effect of antigen on IL-10. METHODS: The supernatant of cultured peripheral blood lymphocytes (PBLC) from 20 patients with allergic asthma and 20 normal subjects with different stimuli [home-dust-mite (HDM) or dexamethasone] was determined for IL-10, by means f enzyme-linked immunosorbent assay. RESULTS: (1) IL-10 was significantly decreased in asthma group, as compared with the control group (P < 0.01); (2) Less IL-10 was released in HDM-stimulated PBLC in asthma (P < 0.01); (3) IL-10 was obviously increased in dexamethason-stimulated whether in the asthma or in the control group. CONCLUSION: The diminished IL-10 production from PBLC could be related to asthmatic attack in patients with allergic asthma. Dexamethason might exert, at least, a part of anti-inflammatory activity by inducing IL-10 secretion of PBLC.

[Clinical effect of captopril in diabetic patients with complicated acute myocardial infarction].

OBJECTIVE: To observe the clinical character of the diabetic patients with complicated acute myocardial infarction and to evaluate the earlyterm effect of captopril on these patients. METHODS: In a multi-centre, randomized, double -blind and placebo-controlled clinical trial of ACEI for AMI in China. In a total of 14 666 AMI patients 1 376 had diabetes mellitus. The 1 376 diabetic patients with complicated AMI were randomized into captopril (669) and placebo (707). All the patients were treated with captopril (12.5mg three times daily) or placebo for four weeks. RESULTS: The results of the entire study showed that the four-week mortality of AMI showed significant difference between patients with and without diabetes (14.75% vs 8.89%, P < 0.0001). The four-week mortality of AMI in patients with diabetes showed no significant difference between those treated with captopril and placebo (14.95% vs. 14.57% P > 0.05). Rates of incidence of main complications within four weeks also showed no significant difference between the two groups. CONCLUSION: After AMI, for the patients with diabetes, treatment with captopril not show a beneficial effect on the early-term prognosis.

[A clinical study of bacterial infection in patients with chronic obstructive pulmonary disease].

OBJECTIVE: To investigate the association of bacteriologic characteristics and bacterial infection in small airway disease. METHODS: Fiberoptic bronchoscopy and ultrathin fiberoptic bronchoscopy were performed on outpatients with COPD and chronic bronchitis both during stable stage and acute exacerbation. Samples were obtained from both large and small airways by protective specimen brush, and bacteria were cultured quantitatively. RESULTS: The positive rates for pathogens in patients with COPD and chronic bronchitis during acute exacerbation were 26.6% (8/30) and 23.5% (5/17) respectively. Major pathogens in acute COPD include Staphylococcus aureus, Streptococcus pneumoniae, Hemophilus influenzae, Moraxella catarrhalis, and Pseudomonas aeruginosa. All of the pathogens were present in large airways, with 2 strains present in small airways concurrently. The positive rates for pathogens in stable COPD and chronic bronchitis were 24.0% (6/24) and 11.7% (2/17) respectively. Major pathogens found in stable COPD include Staphylococcus epidermis, Micrococcus and Streptococcus pneumoniae. The bacteria were less than 10 x 10(3) cfu/ml, present mainly in large airways. There was no significant difference in the extent of small airway diseases between COPD with bacterial infection and that without bacterial infection (P > 0.05). CONCLUSIONS: Bacterial infection is not a major pathogenic factor in stable COPD. There is no direct cause-and-effect relationship between bacterial infection and the extent of small airway diseases in patients with COPD.

[Cervical spinal cord injury treated by expanded hemilaminectomy].

OBJECTIVE: To treat cervical spinal cord injury (SCI) accompanied with narrowing spinal canal by expanded hemilaminectomy. METHODS: From 1995 January to 1998 April 51 patients of cervical SCI were treated by expanded hemilaminectomy. Spinal injury classified in to 3 types: no fracture-dislocation (39 patients) fracture dislocation at the lower cervical spine (11), and burst fracture (1). The types of SCI included central cord injury (18 patients) incomplete cord injury (19), and complete cord injury (14). MR imaging in 23 patients showed degenerative changes with normal intensity of the cord in 14 patients, multiple level hyperintensity in 3, cystic changes in 3, myelomalasia in 3, and cord brocken in 1. Expanded hemilaminectomy was performed in 24 hours in 3 patients, in 48 hours in 9, in one week in 2, after one week in 35, and after one year in 2. The left or right laminae were removed from C(7) to C(3) in 42 patients, C(3) - T(1) in 3, C(2) - C(7) in 2, C(3) - C(6) in 3 and C(4) - C(7) in 3. Hemilaminectomy was expanded lateral to the inner of apophyseal joint and medial to the inner lamina beneath the spinal process. RESULTS: Follow-up lasted for 1 year and 7 months. Six patients with complete cord injury had of the no recovery lower extremity but recovery of the brachialis and extensor radial longus. 12 patients of central cord injury had full recovery except intrinsic muscles of the hand (5). They operated were on 2 weeks after injury. 17 patients of incomplete cord injury recovered to Frankel IV. CONCLUSIONS: Expanded hemilaminectomy is indicated for patients of cervical SCI with narrowing spinal canal or without fracture dislocation. Best results can be obtained in patients of central cord injury, and incomplete cord injury. Even in complete cord injury, 1 - 2 forearm muscle may recover (24.8%), securing a pinch grip reconstruction.

[Motor-evoked potential in investigating neurologic function during spinal cord ischemia and reperfusion injury].

OBJECTIVE: To investigate the method monitoring the neurologic function of spinal cord ischemia and reperfusion injury. METHODS: Motor-evoked potentials (MEP)were monitored at different time points during ischemia and reperfusion of the lumbar spinal cord of the rat. RESULTS: MEP latency increased 15 minutes after ischemia (P < 0.01) and MEP amplitude decreased 25 minutes after ischemia (P < 0.01). MEP disappeared 40 minutes after ischemia and reappeared 5 minutes after reperfusion, but the latency was still longer and the amplitude was lower than the normal value (P < 0.01). The amplitude returned to normal 2 hours after reperfusion (P > 0.05) in contrast to the latency. 24 hours after reperfusion, the amplitude decreased again, and motor function of rat hind limbs was more decreased than that at early time of reperfusion (P < 0.05). CONCLUSION: MEP is a sensitive index for spinal cord ischemia and reperfusion injury.

[Analysis and prevention of dislocation after total hip replacement].

OBJECTIVE: To analyse and prevent postoperative dislocation after total hip replacement (THR). METHODS: Of 850 cases operated on by THR 15 (1.7%) had postoperative dislocation. Eleven were revision cases while the other 4 were operated on their primary procedures. RESULTS: Postoperative dislocation after THR were found in 15 cases. Eleven cases had posterior dislocation by posterolateral approach after revision of THR. Anterior dislocation occurred in 2 cases who received lateral approach. Another two cases of dislocation were related to the design of prosthesis. Separation of polyethylene liner and metal shell in acetabular prosthesis was noted in the two cases. All the cases but 1 were reduced operatively. The cases with malpositioned prosthesis were revised operatively. Augmentation procedure was carried out to increase the strength of abductor. In one case, relocation of the great trochantor increased the strength of abductor. No redislocation was observed during follow-up. CONCLUSIONS: Postoperative dislocation after THR are due to malpositioning of prosthesis, unbalanced bilateral soft tissues, especially the loosening of abductor. Evaluating carefully the development of the pelvis and the deviation of normal bone markers is important in addition to accurate insertion and installment of the prosthesis intraoperatively. Complete clearance of osteophytes and bone cements around the acetabulumis reqaired apart from the repairment of adjacent soft tissues. Double check of hip stability should be carried out before the closure of incision.

[Significance of apoptosis status and apoptosis-associated antigen expression in human colorectal adenocarcinoma sequence].

To investigate the effects of apoptosis on colorectal tumorigenesis and its possible biological significance, the apoptotic frequency in primary cultural cell of 9 normal mucosa, 4 adenomas and 9 adenocarcinomas in time period of 2, 12, 24, 48 hour was measured by flow cytometry. The apoptotic cells index (AI) in situ for 15 colorectal normal mucosa, 7 hyperplastic epithelial, 25 adenomas and 77 adenocarcinomas was identified by the terminal deoxynucleotidyl transferasemediated dUTP-biotin nick end labeling technique (TUNEL). Ki-67 proliferate index (KI), wafl and p53 genes were immunostained with ABC method. The results showed that culture related apoptotic incidence was obviously decreased in cultural tumor cell when compared with mucosa cell after 24-48 hour in vitro. There was a directly positive relationship between the spontaneous apoptosis and Ki-67-index in vivo. The well differentiated or early stage lesions with intensive bcl-2 expression were significantly more likely to have low AI and KI. Both mp53 accumulation and wafl depression which mainly related to KI, had no apparent correlation with AI, bcl-2/bax expression and clinicopathological features statistically; elevatory bax/bcl-2 and pervasive wafl depression led to an increasing AI/KI both in adenoma with atypia and in advanced cancer with distant metastasis or embolus, comparatively. The data indicated that the reduction of susceptibility to inductive apoptosis may contribute to the early phase of tumorigenesis, that AI in vivo may reflect proliferative activity, and that bcl family was closely associated with spontaneous apoptosis and biological behavior of human colorectal cancer.
