A group of 223 insulin-dependent diabetic patients, aged 7-24 yr, who had been under the regular care of our clinic up to 15 yr, were rated by two independent judges on a two-level scale of adjustment and maladjustment. The patients were divided into two groups. Group A (N = 107) comprised those who had been under care from diagnosis of the disease and had been subjected to the special crisis intervention program offered to every family upon referral of a newly diagnosed patient. Group B (N = 116) comprised patients who were diagnosed and treated initially in a clinic that had no crisis intervention program. Significant differences between the two groups were found in respect to three of the four aspects studied, i.e., compliance, familial relationships, and sociability, with group A showing a better adjustment than group B. There was no significant difference in the fourth aspect studied, i.e., school achievement and work performance. It was found that it took three times the effort, i.e., the time invested in counseling and psychotherapeutic measures, to bring group B to a good level of adjustment than it did to achieve similar results with group A. It is suggested that the initial period after diagnosis of diabetes in a child should be considered a period of crisis, requiring special multidisciplinary services to reduce future psychosocial maladjustments and improve compliance.
Two siblings suffering from keratopathy, one with hypoparathyroidism associated with Addison's disease and the other with Addison's disease alone, are described. The hypothesis that there was an autoimmune etiology with variant manifestations in each subject is suggested and discussed.
There is evidence that atherosclerotic vascular disease is observed already in childhood. The data on epidemiology, geographical variation and incidence is incomplete, and several risk factors have been identified and described. It is hoped that international collaborative research studies such as that on the effect of differences in nutrition or diabetes control in children, between our clinic and the Valle Hebron Children Hospital in Barcelona (sponsored by the Child Health Foundation) will generate knowledge on how to prevent premature atherosclerosis in childhood diabetes.
Eleven patients with GH deficiency (GHD) underwent i.v. GH-RH tests (1 mu/kg); 7 had idiopathic GH deficiency (IGHD) and 4 had multiple pituitary hormone deficiencies (MPHD)--diagnosed on the basis of the insulin hypoglycemia, clonidine and sleep tests. One test was done before and the other after five s.c. injections of 1 microgram/kg GH-RH given in the evening. There were three types of response: four patients in whom there was an increase of human GH (hGH) to twice the former level, from 11.6 +/- 4.9 to 22.8 +/- 7.3 ng/ml following priming (a partial hypothalamic lesion); five patients in whom there was no response to the pharmacological tests but the same rise of approximately 13 ng/ml in hGH in response in both acute GH-RH tests (partial pituitary lesion); and two patients with no response to the pharmacological and acute GH-RH tests (pituitary lesion). In all except one patient there was no change in the serum IGF-I values after 5 days priming with GH-RH. It is suggested that patients with a partial GHD due to a hypothalamic lesion might benefit from long-term therapy with GH-RH.

The experience gained in the treatment of 700 diabetic children and adolescents over 20 years at the Beilinson Medical Center, Israel, is reviewed in the light of prevailing concepts. To deal with the psycho-social problems arising in this chronic disease with acute episodes, a multidisciplinary team approach has been used since 1966. The program of this team and the results obtained in relation to the control of the diabetes and to the habilitation and rehabilitation of patients within the family, at school and in adulthood (work and marriage) are described.
