[Humoral and cellular immunogenecity of genetic vaccine on core gene of hepatitis B virus in mice].

OBJECTIVE: To observe the specific humoral and cellular immunogenecity of genetic vaccine based on core gene of hepatitis B virus in both C57BL/6(H-2b) and BALB/c(H-2d) mice. METHODS: The in vitro expressing product of genetic vaccine of HBV core antigen (HBcAg) was identified by Western-blot. Both methods of gene gun and intramuscular immunization were used. Anti-HBc(IgG) and its isotypes (IgG1, IgG2a) in mice sera were detected by indirect ELISA. HBcAg specific cytotoxic T lymphocyte(CTL) activity was measured by 51Chromium release assay. RESULTS: The expression of HBcAg was confirmed by Western-blot. High titers of serum anti-HBc(1:328,050 by gene gun method and 1:109,350 by intramuscular method) were raised in both H-2b and H-2d mice after immunization with genetic vaccine of HBcAg. The levels of IgG2a isotype were generally higher than those of IgG1 in all groups of mice with both immunization methods. IgG2a was predominant in C57BL/6 mice after intramuscular immunization. These results suggested the Th1 type of immune response in mice after HBcAg genetic vaccination. HBcAg specific CTL activities were more than 50% in both species of mice with both immunization methods. CONCLUSION: This genetic vaccine of HBcAg shows strong antigenecity in both cellular and humoral immunity. The possibility of its clinical application needs to be further investigated.

[Post-operative staging and survival based on the revised TNM staging system for non-small cell lung cancer].

OBJECTIVE: To study the factors affecting post-operative staging and survival in non-small cell lung cancer (NSCLC) patients based on the revised TNM staging system adopted by the UICC in 1997. METHODS: Data were collected from 1757 consecutively operated NSCLC patients, including those receiving complete tumor excision, tumor debulking and exploratory thoracotomy from April 1969 through Dec. 1993. The end point of follow-up was Nov. 30, 1998. Cumulative survival and its influencing factors were analyzed by Kaplan-Meier and Cox model of SPSS software. RESULTS: In this series, 30 patients (1.71%) were lost from follow-up. The 5-year cumulative survival rate was 88.0% for patients in stage IA, 53.9% in stage IB, 33.5% in stage II, 14.7% in stage IIIA, 5.5% in stage IIIB and 7.0% in stage IV. The overall 5-year survival rate was 28.2%. The 5-year survival rate was 39.8%, 14.4% and 4.2% in patients treated with complete tumor resection, tumor debulking and exploratory thoracotomy, respectively. The 10-year survival rate was 31.4%, 9.5% and 0%, respectively. Factors affecting long-term cumulative survival, in the order of decreasing significance, were the type of operation, lymph node status, staging, size and pathologic type of the primary tumor. CONCLUSION: The revised staging system for NSCLC is superior to that used since 1986 as far as the end results of treatment in patients in different stage and the staging specificity are concerned. The T3N1M0 classification and the definition of M1 need to be further studied.

[A study on the reserve capacity of endothelial tissue plasminogen activator in patients with acute coronary syndrome].

OBJECTIVE: To study the changes of the endothelial tissue plasminogen activator (tPA) reserve capacity in patients with acute coronary syndromes (ACS). METHODS: Veinous blood was sampled for determination of markers of the fibrinolytic system and hypercoagulability at admission and on the 10th day in patients with acute coronary syndrome (ACS), including acute myocardial infarction (AMI) in 15 and unstable angina pectoris (UAP) in 20. An occlusion test to determine the maximal endothelial tPA release was performed in order to evaluate the endothelial tPA reserve capacity. 20 patients with stable angina pectoris (SAP) and 12 normal subjects served as control. RESULTS: Patients with ACS had higher levels of tPA mass concentration, PAI-1 activity, D-dimers mass concentration and TAT mass concentration at admission and on the 10th day as compared with those in SAP and normal subjects. Maximal endothelial tPA release was initially reduced to (2.2 +/- 0.8) microg/L (P < 0.01, vs patients with SAP), but the levels recovered on the 10th day of follow-up to (4.5 +/- 1.2) microg/L (P > 0.05, vs patients with SAP). CONCLUSION: Endothelial tPA reserve capacity is reduced only during the acute phase and becomes normalized during follow-up. The alterations of plasma markers of systemic fibrinolysis persist longer in patients with ACS. The results demonstrated that the intravascular thrombus formation may relate more to the disturbance of endothelial fibrinolysis than to markers of the systemic fibrinolytic system. It is very important to correct the disturbances of endothelial fibrinolysis during the acute phase of ACS.

[Pathologic review of 5,050 pulmonary tuberculosis cases].

OBJECTIVE: To summarize the pathomorphologic changes of pulmonary tuberculosis and the relationship between those changes and the combined chemotherapy regimens. METHODS: Observing the pathological changes in 5,050 cases with pulmonary tuberculosis, and the relationship between three different combined chemotherapy regimens HSP, HRS, HRZS and the pathological changes of pulmonary and bronchial tuberculosis. RESULTS: The rate of surgical excision of caseous cavities and fibrous-cavities declined markedly in cases treated by HRS and HRZS regimens (P < 0.01). Whereas, after treated by HSP, HRS and HRZS regimens, the rate of surgical excision of tuberculoma were 26.82% (897/3,345), 41.24% (464/1,125) and 47.8% (277/580) respectively which showed a rising tendency (P < 0.01). Open healing cavities were found in 42 patients. CONCLUSIONS: The effects of combined rifampicin and pyrazinamide chemotherapy caused the changing of pathologic types of pulmonary tuberculosis, that is: the increased proportion of tuberculoma and the decreased proportion of caseous and fibrous cavities. Tuberculoma ought to be taken into account in the classification of tuberculosis.

[Multivariate analysis of prognostic factors after hepatectomy for primary liver cancer].

OBJECTIVE: To determine the prognostic factors for primary liver cancer (PLC) after hepatectomy. METHODS: 182 patients who underwent hepatectomy for PLC from 1988 to 1996 were included for univariate and Cox multivariate analysis. 20 factors contributed to the long-term survival rate (SR) and the disease-free SR were analysed. RESULTS: The cumulative SR (n = 182) in 5 years was 43.3%. The 5-year SR and disease-free SR in the curative resection (CR) group (n = 156) were 48.0%, 24.9%, respectively. Multivariate analysis revealed that classification of CR and differentiation grade were significant factors for the long-term SR, and classification of CR and Child-Pugh's classification were significant factors for the disease-free SR. The 5-year SR and disease-free SR for pathological CR, clinical CR and non-CR (palliative resection) groups were 61.1%, 20.4%, 0 and 28.8%, 15.5%, 0, respectively. CONCLUSIONS: The classification of CR is the determining prognostic factor. Pathological CR is the first option for patients with requirements. To control liver disease and improve adjuvant therapy are essential to decreasing the recurrence and metastasis rates.

[Clinical protocols for the treatment of severe acute pancreatitis].

OBJECTIVE: To set up principles for the treatment of severe acute pancreatitis (SAP). METHODS: The therapeutic principles and results of SAP in three periods were analyzed. Period I (1980 - 1990) was characterized by early surgical intervention supplemented with integrated traditional and western medical therapy, period II (1991 - 1993) by integrated traditional and western medical therapy supplemented with surgery for early complications, and period III (1994 - 1997) by integrated traditional and western medical therapy supplemented with surgery for local infectious complications only. RESULTS: Because of the improvement of intensive care, the operation rates kept declining in the three periods. They were 77.59%, 54.55% and 29.55% with a mortality rate of 40.52%, 17.17% and 11.36%, respectively. CONCLUSIONS: Integrated traditional and western medicine has been effective in most SAP patients, and surgery is indicated only for late complications such as necrotizing infection. Early surgery should be restricted.

[Early results of 1,110 cases of coronary artery bypass].

OBJECTIVE: To introduce our experience with Coronary artery bypass. METHODS: From January 1996 to March 1999, coronary artery bypass was in 1110 patients at our hospital. They were 985 men and 125 women, aged 27 - 78 years (57 +/- 9). 500 patients were aged over 60 years. 670 patients had a history of myocardial infarction. 450 patients suffered from instable, and 460 from stable angina. 169 patients had stenosis of the left main trunk (>or= 50%). The heart function of 240 patients was over NYHA class III, the EF of 123 patients was less than 40%. 454 patients were associated with hypertension, 133 with diabetes, 131 with ventricular aneurysm, 48 with valvular disease, and 5 with rupture of ventricular septal. 1048 procedures were performed on hypothermia cardiopulmonary bypass (CPB) and 60 on beating heart. Myocardial preservation was carried out by cold blood cardioplegia with potassium. The average time of CPB was 115 +/- 35 minutes, and that of aortic cross clamp was 72 +/- 24 minutes 110 cases received one graft, 145 cases two grafts, 415 cases three grafts, and 439 cases four and more than four grafts. Left internal mammary artery, great saphenous vein, and radial artery were used as coronary artery bypass conduits on 751, 877 and 101 patients, respectively. Total arterial myocardial revascularization was performed on 72 patients. Concomitant operation included ventricular aneurysmetomy (112 cases), plication (14), left ventriculoplasty (5), valvular procedures (48), repair ventricular septal rupture (5). RESULTS: 1 101 patients were discharged uneventfully. Early death occurred in 9 patients, (0.81%). Complication included low cardiac output (11 cases), introaortic balloon pump (7), perioperative myocardial infarction (2), complication of brain (3), and rethoractomy for bleeding (4). CONCLUSIONS: To identify adequate target vessel and completely revascularize ischemic myocardium are the key of coronary artery bypass. Perioperative treatment is important.

[Safety of direct gene transfer into rat carotid arteries mediated by adenoviral vector].

OBJECTIVE: To evaluate the safety of direct gene transfer into rat carotid arteries mediated by adenoviral vector. METHODS: After being soaked in high concentration of glucose solution containing Adv5-CMV (control group) or Adv5-CMV/LacZ (treatment group) for 30 minutes, soluble stents were inserted into the lumina of rat carotid arteries and end-to-end anastomoses of cut rat carotid were performed with standard microvascular surgical technique. On the 7th day after surgery, anastomotic carotid arteries of the two groups were obtained, and samples of ascending aortas, hearts, brains, livers, lungs, spleens and kidneys of the treatment group were taken for assessing beta-galactosidase activity and histochemical staining. RESULTS: Beta-galactosidase activity was not detected in the carotid arteries in the control group and in those organs not directly exposed to adenoviral vector in the treatment group. The amount of beta-galactosidase expression in the carotid arteries in the treatment group was 10.1 x 10(-3) IU/g tissue. Microscopic examination of sections from vessels in the control group and from the aorta, heart, brain, liver, lung, spleen or kidney in the treatment group revealed no X-gal staining. Microscopic examination of the carotid arteries in the treatment group revealed blue-staining in all anastomotic arteries and in all layers of the arterial wall, including the intima, media, and adventitia. CONCLUSIONS: A recombinant gene could be safely delivered to a local site of the vessel by means of infection with adenoviral vector.

[Studies on immunization strategies for hepatitis B in different endemic areas of China].

OBJECTIVE: To compare the cost-benefit of immunization with hepatitis B (HB) vaccine in different endemic areas of HB and to explore the optimal strategy for it. METHODS: Cost-benefit analysis and comprehensive weighted score analysis (CWSA) were used to screen the optimal immunization strategy for low, medium and heavy endemic areas of HB. RESULTS: Significant economic benefit was obtained in Long-An County, Guangxi Zhuang Autonomous Region, Shanghai and Jinan, Shandong Province from HB immunization programs for infants. The greatest benefit cost ratio (BCR) was provided by the low-dose immunization strategy, which was 49.91, 54.53 and 37.68, respectively for heavy, medium and low endemic areas of HB. The maximal net benefit could be obtained by the high-dose immunization strategy with better immune protective efficacy. CWSA showed that the low-dose immunization strategy was the optimal one for HB immunization. CONCLUSION: It is suggested that the low-dose immunization strategy be implemented in undeveloped areas to obtain a greater BCR, and the high-dose one be implemented in more developed areas to obtain more net benefit and lower prevalence of hepatitis B surface antigen carriage.

[Effects of different levels of calcium intake on bone of pregnant women].

OBJECTIVE: To research some effects of different levels of calcium intake in the mother's bone mineral density. METHODS: On the basis of insignificantly different among energy and other nutrition intake excepting calcium, 35 volunteers in pregnant 18 weeks healthy women were randomly divided into I. II and III group [calcium intake were (550 +/- 150) mg/d, (900 +/- 150) mg/d and (1,500 +/- 150) mg/d]. The tracked study began from pregnant 20 weeks to postpartum 45 days. All over and different body of BMD (Bone Mineral Density) were assessed by dual-energy X-rays absorptiometry. RESULTS: I group of many location BMD were below that of other groups (P < 0. 01). As compared with the same age women's lumbar spine BMD, I group was only maintained within the rang (85.14 +/- 6.61)%, and osteoporosis in some places of bone were found in some individual. II group was (90.74 +/- 6.53)%. III group was (100.44 +/- 5.19)%. Allover different body's BMD were significantly correlated with the average BMD (r > 0.8, P < 0.1), the most significant correlation was third lumbar spine. Calcium intake of women was positively correlated with different body's BMD (r > 0.5, P < 0.01). CONCLUSION: III group, calcium intake was (30.68 +/- 3.56) mmol/d, pass through period of pregnancy, parturition and short-term breast-feed mother body remain the same BMD, and prevent osteoporosis. Reporting pregnant women calcium nutrition pass through BMD, lumbar spine is good representation.

[Influence factors in the clearance of urea and creatinine through gastrointestinal tract].

To investigate the affection of different osmotic density, pH value, and dialytic fluid with drugs on the clearance of urea and creatinine through gastrointestinal tract, we used the renal-artery-ligated rabbits as the model. After the intravenous administration of urea and creatinine through ear vein, fistulas in duodenum at about 5 cm to pylorus and in ileum at about 5 cm to the valva ileocecalis were made to carry out intestinal dialysis. The results showed that the clearance of the urea in treated groups with mannitol, regitine, and oxystarch was higher than that in the control group with normal saline solution(P < 0.05). The clearance of urea and creatinine in treated groups with atropine was higher than that in the control group(P < 0.05). There was no significant difference in the clearance of urea and creatinine among treated groups with different intestine pH value.

[An improvement on determination of the related substances in vindesine sulfate and its injection].

A new method for the analysis of related substances in vindesine sulfate and its injection to improve the ability of separation by RP-HPLC was established. The separation was achieved by applying a reversed-phase C18 column (150 mm x 4.6 mm i.d.) and a mixture of V(MeOH):V(buffer solution) = 70:30 as mobile phase. The buffer solution was an aqueous solution of volume fraction 1.5% diethylamine adjusted to pH 7.5 with phosphoric acid. The UV-detector was set at 267 nm. Four related substances were separated from vindesine within thirteen minutes. The tailing factor of the peak of vindesine is 1.00-1.05. The results showed that this method is more efficient for separating the related substances in vindesine sulfate than the conventional method.
