Physical therapy in conservative (functional) treatment of acute achilles tendon rupture.

1. All patients returned to their daily working activities. 2. We did not record a single case with complications. 3. With regard to other methods of treatment the functional treatment is apparently without an alternative, but at the same time requires methods that seek full engagement of a surgeon, a radiologist and a physiatrist. 4. In order to avoid compromising and make the treatment successful, it is necessary to keep up with the implementation protocol, to have available adequate coeffective shoes or other means, and to possess certain level of clinical experience.

[Abu al-Qasim Az-Zahrawi--a great Arab surgeon] .

History is a witness of the great importance and influence of islamic science from the period of "Golden Age of Arabic Civilisation". A famous scientist said: "Science has no country, it is international; we all share in fruits of investigations of people from different traditions and all ages." There are many worldwide famous arabian scientists: El-Kindi, Er-Razi, Ibn Sina, El-Biruni, Ibn Hajsem, Ez-Zahravi, El-Farabi, Ibn Zuhr, Ibn Ruzd etc. These names, among several hundreds of arabian physicians, attribute "Golden Age" of islamic science. That period was characterised by movements, reprocessing of ideas. That reprocessing of ideas has gained the great minds together, and that process is continuous. That is why we have to be grateful to them. One of them, who is not enough famous within medical professionals, Az-Zahrawi, belong to the greatest surgeons in the arabian "golden age" period.

[Emergency medicine today--quo vadis? (developments and perspectives)].

The author analyses nowday condition of emergency medicine in Federation Bosnia and Herzegovina and all its components. He shows all negativenesses in work according an unorganized and an uneffective system of "reaction and answer". In conclusion, it is given the suggest for benefit of this very important part of health care.

[Subacromial pain syndromes as a possible results of errors in the initial diagnosis and therapy of the shoulder joint].

The author in this paper shows possible diagnostic mistakes in the development of subacromial painful arc syndrome (cases with inadequate diagnostics). A hundred cases with acute shoulder's injury (without fractures and luxationes) who were admitted at the Clinic for bone surgery and Emergency Department of Clinical Centre, CUC Sarajevo in period between 1 January 1998-30 June 1998. Only clinical and X-ray examination in part were performed. A very interesting case with subacromial impingement as a consequence of such kind of treatment is presented in this paper. It can be concluded that it is necessary to use as a routine a comparative advantages of Echosonography, CT, MRI, as non invasive diagnostic methods at clinically suspected intra or extra articular lesions.

[Organization of emergency care of locomotor injuries in the Canton of Sarajevo].

The system of the urgent medicine services in Sarajevo is organised on the principle of the step by step treatment, i.e. with a clear difference between preclinical and clinical levels. The objective of our work is to analyze the existing system of organisation of the urgent medical treatment in situations of loco-motor apparatus injuries, that is to find out how big discrepancy is between diagnostics performed during the pre-clinical period phase and diagnosis which is definitely clinically verified. During the above mentioned analyses were used patients of the Institute for Urgent medical care who were treated in the period of March-June 1999, during the time period from 7 pm-8 am (when surgical examination or x-ray diagnostics were not available). The analyses has been carried out as a short term survey. During the same time period the patients were monitored also by Urgent Medicine Centre of Clinical Centre of Sarajevo University, where were available both suitable surgical and x-ray diagnostics. For that purpose were controlled 34 patients who were treated by the above mentioned institutions with the following monthly frequency: March-13 patient, April-11, May-4, June-6. The ratio between male and female patients was 26:8. Based on the results, we can conclude that there is a great significant discrepancy between the initial diagnosis, verified diagnosis and the final treatment. The discrepancy was present in the following months: Marc-incorrect initial treatment in even 17.6% (6 patients), April 8.7% (3 patients), May-June 17.6% (6 patients). The authors are due to complete evaluation of mistakes caused by the initial diagnoses and their impact on the final outcome of the treatment, including all the consequences caused by them.

[Dilemmas in the classification of foot injuries caused by land mines].

In international classification of injuries, very often there are some specific conditions that are not clearly defined. War initiated that about specific conditions before all injuries a discussion in term of more correct presentation through the code is being made. Very often, some conditions in international classification are being led as INSUFFICIENTLY DEFINED or UNMARKED PART. When we have present large destruction due to wounding with projectiles of high kinetic energy (initial velocity), and especially stepping on land-mines, we have the situations that can not be correctly defined. The amount of these injuries which are about to follow in the future period authors will declare in few examples which have been surgically treated in Traumatological Clinic in Sarajevo. All injuries were result of stepping on land-mines and in all cases it was about foot. The stated injuries we were not able to code as amputations because the distal part of the injury was vascularized, later on we found out that there are possibilities of surgical reconstruction in meaning of functional foot, since we do not have open (explosive) fracture here. May-be, as a clinical formulation or for initial observation a more appropriate name is (term analogue in surgery of other regions beside limbs) INTERCALARY amputation of limbs. The same one could be moved from deficiency to a complete lack which surely must be linked to its causality.

[Significance of active and passive drainage methods on healing after use of internal osteosynthesis (animal experiment)].

The results of healing by osteosynthesis at dogs' bones fractures are presented in this paper. The importance of drainages is emphasised in the healing process. In Cantonal veterinary station in Sarajevo in the period January 1995-January 1998, 14 dogs with osteosynthesis were selected. We observed the following parameters: wound's healing, functional status, X-ray examination. In respect of the incidence of complications in the final results of healing there were no evident differences between these two groups with active drainage A and passive drainage B.

[Arthroscopic treatment of fractures of the tibial plateau].

The author presents a new operative approach (arthroscopic assisted treatment and fluoroscopy). It is a minimal invasive surgical procedure which consists of the reposition and fixation of tibial plateau fractures type B/ASIF classification. In the period 1995-1997 on Traumatology Clinic, Clinical centre in Sarajevo 10 patients with tibial plateau fractures have undergone a surgical treatment (9 patients with lateral, 1 patient with medial). Seven patients have an isolated injury and 3 patients have a complex injury. After surgical procedure, the final result was evaluated according to Final Score Scale (FSS) by Lysholm and Radiological Score Scale (RSS) by Jarvinen-Kannus and it was excellent. In the postoperative period an early movement procedure was applied and deep venous thrombosis (DVT) was prevented by A-V impulse system and required heparin administration.

[A fixator-pin-wire-plaster technique in war conditions].

Technique fixateur-pin-wire/plaster is an original method. It is connected very much with the primary treatment in the cases of the improvised war hospitals, where are no elements for internal fixation. In the act "open" fixation the free fragments are connected with Shantz pins. Around the pin is the wire (as inter-fragmentary compression with a screw). In this way, when the fixateur is not necessary it can be removed, but the place of fracture is fixed. During the coming treatment the plaster can be used, but it may not. In this way we can finish the treatment without use of surgery. This method we have used continually as a clinical treatment. In the paper we shall our first good experiences.

[The effect of organization of treatment of injuries on the occurrence of late complications of fractures caused by gunshots and explosions in war].

In this paper we present two approaches we have applied in organization of treatment of the wounded persons during the war in Bosnia and Herzegovina, as well as later complications. We have analysed 300 wounded persons--150 in group A and 150 in group B. They were treated in stage and non-stage form in an urban condition. These complications were expected and there were no significant difference between these two groups.

[Use of the ilio-patellar band as an extra-articular amplifier in the knee joint].

We used a part of the ileopatellar band (IPB) as a complex of ITT in a control of a barrier of anterolateral rotatory instability (ALRI) in three patients with acute lesions of intercalary/collateral joint structures (ALRI). Ileopatellar band transfer was performed as amplification after primary reconstruction of the above mentioned structures. After two year period of observation we concluded that: a) the method was simple and applicable, b) it did not affect the functionality since only a part of the band was used, c) thus ITT could be favored, d) an earlier initial phase of movement was possible, and e) drift did not present a problem ... At the end of the treatment satisfactory stability was achieved and the patients returned to their daily activities.

[A mini circular external fixator--Kosevo type].

In this experiment we showed factors of interest for compression's grade after experimental examination. We used mini circular external fixator type Kosevo and showed it's employment. The conclusion that the compression's grade was 30 kp/cm2 and depend of bone's thickness. At the end you can see clinical and medical economic effects of using thus fixator.

[Injury of the knee joint associated with popliteal artery injury--case report].

In this paper we consider that it is necessary to neglect injuries of blood vessels, because often the surgeon pays all his attention to the instability of knee and in every luxation of injury of knee joint is necessary to take care of vascular status. Necessary is the operative treatment in the optimal time, in team work with the determined order in treatment. Everything done contrary to this, surely leads to the amputation of extremities.

[A complex injury of the knee joint associated with a popliteal artery lesion--case report].

Injuries of the popliteal artery are frequent in the dislocations of the knee joint, and an early diagnosis of a lesion of the popliteal artery is the base for the successful treatment. Generally, in all the traumas in the region or proximity of the knee joint, it should always be taken into consideration a possibility of arterial injury. In our case there was a posterior knee joint luxation with evidently contused and thrombosed popliteal artery. The surgery was done in an inappropriate time (more than 14 hours after the injury), and consisted in a Goretex graft interposition, and following a ligamentary reconstruction a transitory arthrodesis with external fixator (AO) in one level was carried out, what can be a model for similar situations. A control arteriography immediately before the patient's discharge from the hospital showed an arterial passage, while clinically an evidently lesser posterior unstability existed. In the surgical managing of such injuries it is necessary an interdisciplinary approach, appropriate period of time and corresponding sequences of repairs.
