Dementia is the major cause of functional dependence in the elderly: 3-year follow-up data from a population-based study.

OBJECTIVES: The purpose of this investigation was to study the role of dementia and other common age-related diseases as determinants of dependence in activities of daily living (ADL) in the elderly. METHODS: The study population consisted of 1745 persons, aged 75 years and older, living in a district of Stockholm. They were examined at baseline and after a 3-year follow-up interval. Katz's index was used to measure functional status. Functional dependence at baseline, functional decline, and development of functional dependence at follow-up were examined in relation to sociodemographic characteristics and chronic conditions. RESULTS: At baseline, factors associated with functional dependence were age, dementia, cerebrovascular disease, heart disease, and hip fracture. However, only age and dementia were associated with the development of functional dependence and decline after 3 years. In a similar analysis, including only nondemented subjects. Mini-Mental State Examination scores emerged as one of the strongest determinants. The population attributable risk percentage of dementia in the development of functional dependence was 49%. CONCLUSIONS: In a very old population, dementia and cognitive impairment make the strongest contribution to both the development of long-term functional dependence and decline in function.

Clonality analysis of cervical cancer on microdissected archival materials by PCR-based X-chromosome inactivation approach.

Clonality of invasive human cervical cancer was assessed in microdissected archival formaldehyde-fixed, paraffin-embedded tissues by PCR-based analysis of X-chromosome inactivation of the androgen receptor gene. In 18 informative cases, cancers from 12 cases including 2 early-invasive cancers were scored as monoclonal. Among them, 8 cases had the combination of random X-chromosome inactivation in normal cervical tissue and non-random in the cancer and in other 4 cases normal and cancer tissue were both non-random but had discordant X-chromosome inactivation. Six cases showed that the non-random inactivation affected the same allele in cancer and normal tissue and thus were considered to be inconclusive. The results suggest the monoclonal origin of cervical cancer and indicate that genetic events are critical in transition of pre-malignant epithelia to invasive cancer in cervical carcinogenesis. Finding of monoclonal early invasive cancer also argues that monoclonality of human tumors is not a late event due to clonal competition or selection. X-chromosome inactivation patterns in normal cervical epithelia and tissues were analyzed in 21 informative cases. When a mixture of normal stroma and epithelium was analyzed, 38% (8/21) of cases showed non-random inactivation pattern, whereas using pure epithelium it was 64% (11/17). The X-chromosome inactivation pattern between mixed epithelium/stroma and matched epithelium differed in 2/8 cases. These results point to a certain amount of overlooked source of error in X-inactivation-based tumor clonality analysis in which peripheral blood monocytes were chosen as control and strongly recommend that control tissues which share close histological origin with analyzed tumors should be selected in any clonality study of human neoplasm. Post-embryological mechanisms of clonal patch in normal cervical epithelia are also discussed.

[A socioepidemiological survey of the oral health status and needs for dental care among elderly population in China].

The purpose of this surrey is to study the prevalence of dental disease and the needs of dental treatment, so as to develop effective programs in improving dental care in the elderly, 475 residents are selected by means of a systematic stratified sampling procedure. The aged people have poor dental health knowledge and habits. Major problems for getting their dental care include: 1. difficult to get registration; 2. long time waiting before seeing a dentist; 3. the actual needs for dental treatment are more than what expected by the elderly themselves. The study reveals that many problems are existed in terms of oral health status and dental care among the elderly, which calls for more attention.

Study on delay two-phase multiple organ dysfunction syndrome.

OBJECTIVE: To study the injury factors, pathogenic process and clinical features of delay two-phase multiple organ dysfunction syndrome (MODS) in severe burned patients and to replicate a standardized animal model that would accurately imitate the clinical features of MODS. METHODS: Forty-five human patients with burn size larger than 30% total body surface area (TBSA) were analyzed. All of them underwent severe burn shock in early stage and sepsis in late stage. Thirty-two goats were randomly divided into three groups: 1) hemorrhagic shock (group H, n = 6); 2) endotoxemia (group E, n = 6); and 3) hemorrhagic shock plus endotoxemia (group M, n = 20). Hemorrhagic shock was produced according to the method of Wigger (6.7 kPa for an hour, 1 kPa = 7.5 mmHg). Endotoxin (E. coli O111 B4) was given via the portal vein 24 hours after the resuscitation of hemorrhagic shock, in a dose of 30 ng/kg/min for 5 consecutive days. During the observation period of 10 days, all animals were hemodynamically monitored, given standard metabolic support and due cardiac and pulmonary support according to human intensive care. RESULTS: All the patients showed burn shock at 1-3 days and hyperdynamic circulation, hypermetabolism and systemic inflammatory responses over two weeks post-injury. Thirteen cases were found to develop MODS according to the prevailing diagnostic criteria, and 10 of them died with a mortality of 77%. Eighteen animals died in group M with a mortality of 90%, 12 of the 18 developed MODS, with overall incidence of 60%. Most animals in group M showed changes similar to that observed in human cases. The experimentation proved that in the pathogenic process of MODS, there was a two-hit phenomenon in the dvelopment of the syndrome. To prevent the development of MODS, it therefore was imperative to blunt the first hit or the second hit, so that an excessive inflammatory response was alleviated. This postulation has been verified in the treatment of extensive burns. Two patients with burn extent reaching 100% TBSA survived with only mild acute respiratory distress syndrome (ARDS) and renal dysfunction after comprehensive treatment of burn shock, including adequate fluid resuscitation, drugs to remove oxygen free radicals, rapid restoration of pHi, and early extensive excision of burn eschars. CONCLUSION: Both in human patients or animal experimentation, the typical delay two-phase MODS is shown to be produced by two successive insults in the forms of hypovolemic shock and sepsis. This postulation is helpful in formulating the prevention and treatment modality of MODS.

[Clinical guidelines for timing of escharectomy and skin grafting during burn shock stage in extensively burned patients].

OBJECTIVE: To provide practical clinical guidelines to doctors who have no hemodynamic monitoring facilities in performing escharectomy during the shock period in extensively burned patients. METHODS: We analyzed our clinical experiences in 60 patients with extensive burn. RESULTS: Puting forward several clinical indexes for timing of escharectomy during burn shock stage: 1. Amount of fluids in the first 24 h postburn 2.6-3.0 ml.kg-1.1% TBSA-1; 2. Output of urine 80-100 ml/h; 3. Mentally fully conscious; 4. Thirst significantly alleviated and there is no nausea and vomiting; 5. Pulse 100/min; 6. Hb < or = 150 g/L; 7. Hct < or = 0.50. CONCLUSION: With the clinical indexes as guidelines, we assume that escharectomy could be performed during burn shock stage with reasonable safety.

[Influence of escharectomy during burn shock stage upon the changes in endothelin and NO].

OBJECTIVE: To observe influence of escharectomy during and after burn shock stage upon the changes in plasma ET and NO. METHODS: 35% TBSA full-thickness burn was produced in pigs, which were divided into two groups. Group S was a group undergoing escharectomy during burn shock stage at 24 h postburn. Group C was a group receiving escharectomy at 96 h after injury. ET and NO were determined and ET/NO was calculated. RESULTS: The levels of ET and NO were higher after injury than before injury in both groups. The level of ET was lower significantly in group S than in group C from 96 h postburn on. The level of NO was higher in group S than in group C. The ET/NO was lower in group S than in group C from PBH 24 on. CONCLUSION: Escharectomy during burn shock stage was beneficial in reducing injury of endothelial cells, decreasing exudation and edema, decreasing tissue hypoxia and improving microcirculation.

[The effect of certain pro-inflammatory mediators on the pathogenesis of acute gastric mucosal lesion in early burn stage].

OBJECTIVE: To appraise the effect of certain pro-inflammatory mediators on the pathogenesis of acute gastric mucosal lesion in early burn stage. METHODS: 21 patients with burn injuries of over 30% TBSA were divided into A and B groups according to the main clinical indexes during shock resuscitation and hemodynamic parameters. Fiberoptic endoscopic examination, determination of intramucosal pH (pHi) and measurement of some mediators were done immediately after admission to the hospital and 4 and 7 days after burn injury. RESULTS: It was demonstrated that the level of LPS in plasma, the content of TNF-alpha, IL-8 and ET in group B at 4 and 7 days postburn were significantly higher than those of group A, while the value of pHi in group B was markedly lower than that of group A. Damaging index of gastric mucosa was negatively correlated with pHi (r = -0.89, P < 0.05), but positively with ET (r = 0.91, P = 0.05). CONCLUSIONS: These findings suggest that the inflammatory mediators and cytokines promoted secondary damage to gastric mucosa during early postburn. It was believed that pHi was a sensitive index, and it played an important role in the development of stress ulceration.

[Effects of supplement Zn on levels of Zn in serum, growth hormone and hydroxyproline].

OBJECTIVE: In order to research the effects of supplement Zn on wound healing, the changes of Zn in serum and skin, growth hormone and hydroxyproline of skin were observed in burn rats. METHOD: 80 SD rats were randomly divided into four groups: Control group of 8 rats, N group (diet containing Zn 40 micrograms/g), W group (application of Zn on burn skin), H group (diet containing Zn 80 micrograms/g). 8 rats of each group were killed on 1, 3, 7 days after burn. RESULTS: The levels of serum Zn declined on 1 day postburn in N and W groups, whereas the content of serum Zn in H group elevated and were 2.4, 2.0 times as much as that of N and W groups respectively (P < 0.01). Zn in burn skin increased significantly on 3, 7 days in W group and was 1.3, 1.5 times as much as that of N group. Serum growth hormone increased in all groups postburn. The highest level appeared in H group. Hydroxyproline in burn skin decreased in all groups, but it began to increase on 7 day postburn in W group. CONCLUSION: Supplementation of Zn by diet rapidly raises the levels of Zn in serum, burn skin and growth hormone. It is beneficial to apply Zn to the wound to improve wound healing. Hydroxyproline increased significantly with application of Zn to burn wound. It is better to supply Zn by diet and topical application simultaneously.

Advances in TCM treatment of hyperprolactinemia.

In recent years, rapid progress has been seen in the treatment of hyperprolactinemia with TCM and materia medica. The old pattern of simply using bromocryptine to treat the disease has been changed. Most of the cases reported in this paper were primary hyperprolactinemia, with other primary diseases excluded. As TCM treatment produces definite therapeutic effects with few side-effects, the methods above reported are worth recommending.

Precancer of the human cervix.

The tumour biology of cervical precancer is unusual. A large variety of individually distinct forms crudely divided into slight, moderate, severe dysplasia and carcinoma in situ exist. Virtually all contain genital human papillomavirus (HPV) either as infectious virions or as episomal or integrated DNA. HPV, which occurs as hundreds of types, subtypes and variants, has a high prevalence in all human populations. Most males are symptomless reservoirs, whereas a proportion of infected women develop condyloma, precancer and subsequently, in a minority, invasive cancer. HPV has unequivocal features of a sexually transmitted infectious agent. Risk of precancer is statistically related to infection with genital HPV, but differences in risk between populations with high and low prevalence of HPV are larger than expected from a direct correlation. Findings fit with HPV as a major risk factor, but other factors must also be operative. These may include shifts in number of target cells, depending on regeneration and infection by various micro-organisms, hormones, smoking and immunity. Final proof of necessity of HPV infection for precancer can probably be delivered only after its elimination by successful vaccination. Genital condyloma, which is not precancerous, is caused by HPV low risk types, typically 6 or 11, in analogy with papilloma formation in skin and mucosa in a large variety of species. This benign lesion is the hallmark of mammalian HPV pathology and a source of interindividual spread of virus. Slight dysplasia is heterogeneous. Many lesions seem to be polyclonal, self limited cell proliferative responses to infection with low grade HPV. A small proportion are associated with either simultaneous presence or subsequent development of higher grades of dysplasia, in situ or invasive cancer. Evidence exists for two mechanisms: clonal selection of cells with increasingly undifferentiated phenotypes, and independent development of different morphological types of precancer. The relative importance of the two is unknown. High risk HPV, typically 16 or 18, is preferentially associated with high grade dysplasia and in situ cancer, either because it increases risk of clonal progression to these forms or induces them de novo. Severe dysplasia, in situ and invasive cancer always present as monoclonal lesions. Genetic links indicate that these pathologies arise by clonal selection from less advanced precursors. The number of potential target cells for precancer confined to a narrow transformation zone is small. Risk of precancer and malignant transformation per target cell is therefore probably far higher than in any other human tissue subject to cancer. Spontaneous mutation rate and physicochemical carcinogens seem insufficient for the creation of a malignant phenotype in cells of the transformation zone. Currently HPV is the only strong candidate for such a feat. Any or all of the following mechanisms may play a role: overexpression of viral E6 and E7 genes, often triggered by disruption of control elements upon integration of viral DNA into the cellular genome, activity of specific (E6?) configurations in certain HPV variants, inactivation of TP53 with decreased capacity for DNA repair and enhanced likelihood of accumulation of "transforming" mutations and viral integration at sites controlling function of cellular oncogenes and/or suppressor genes. Target cells within the transformation zone have the capacity for bidirectional (squamous and/or glandular) differentiation. HPV types seem to drive cells preferentially in different directions after infection/transformation. Low risk types are almost always associated with squamous differentiation, HPV 16 usually also with squamous differentiation and HPV 18 with adenosquamous or adenomatous differentiation. [Mechanisms of early gastro-intestinal ischemia after burn: hemodynamic and hemorrheologic features].

OBJECTIVE: To study the mechanisms of early gastro-intestinal ischemia developed in acute burn period and its relationships with hemodynamic and hemorrheologic changes. METHODS: Twelve pigs were randomly allocated into two groups: group C, a sham group that was subjected to all surgical procedures except burn; group B, 30% TBSA cutaneous thermal injury, and was resuscitated with Parkland formula one hour after burn. RESULTS: MAP remained stable after burn, but RAP, MPAP, PAWP and CI decreased significantly to the lowest level 4-8 hours after burn, and recovered after resuscitation 24 hours postburn. Intramucosal pH declined immediately (1 hour after burn) and remained abnormal throughout observation period. Portal venous blood flow demonstrated similar changes as pHi, and correlated well with intestinal pHi. Whole blood viscosity and plasma viscosity in portal venous blood elevated obviously after burn. CONCLUSIONS: 1. GI ischemia occurred early and recovered slowly during burn shock phase with conventional resuscitation regime. 2. GI ischemia correlated significantly with portal venous blood flow, but did not with systemic hemodynamic variables. 3. Hemorheologic changes in portal venous blood may exaggerate ischemia injuries.

The attenuative effect of purified protein derivative sensitization on T helper 2 reaction and eosinophil infiltration of the lung in ovalbumin sensitized mice.

OBJECTIVES: To identify the effects of tuberculin purified protein derivative (PPD) sensitization on attenuating pulmonary T helper 2 (Th2) reaction and eosinophil infiltration in ovalbumin sensitized mice, and to search for the possibility of its clinical use in the management of asthma in a new way. METHODS: Sixty C57BL/6 mice were sensitized with PPD and then with ovalbumin and aluminum hydroxide, and randomized into 4 groups: ovalbumin (OVA), pre-PPD, post-PPD and control groups. Aerosol PPD were administered 3 h before or after ovalbumin challenge in the pre-PPD and post-PPD groups respectively, and control group received aerosol PPD only. IL-4, IL-5 expression was detected by immunocytochemistry in situ hybridization. Lung slides were stained with eosin and hemotoxylin, and pathological changes were observed. RESULTS: Ovalbumin aerosol inhalation caused a mixed inflammatory infiltration dominated by CD4+ T lymphocytes and eosinophils in the lung of sensitized mice. 87.5%-89.7% and 89.0%-89.2% of the CD4+ T lymphocytes were IL-4 mRNA+ and IL-5 mRNA+ respectively. 88.7%-91.2% of IL-4 mRNA+ cells and 89.8%-90.6% of IL-5 mRNA+ cells were CD4+ T lymphocytes in OVA group. Aerosol administration of PPD markedly suppressed IL-4 and IL-5 expression, and lung eosinophil infiltration. It was more effective in pre-PPD group. 76.6%-78.0% of IL-4 mRNA+ and 73.8%-79.7% of IL-5 mRNA+ cells were CD4+ and 78.1%-84.9% and 78.4%-85.3% of the CD4+ cells were IL-4 mRNA+ or IL-5 mRNA+ respectively in pre-PPD group, both were markedly lower than that of OVA group. CD4+ percentage of IL-4 mRNA+ and IL-5 mRNA+ cells were 80.7%-82.0% and 78.0%-83.9% in post-PPD group, which were markedly lower than that of OVA group. CONCLUSIONS: Sensitization with PPD by intraperitoneal injection and then challenged by PPD inhalation markedly suppressed IL-4, IL-5 expression and eosinophil infiltration, and attenuated pulmonary Th2 reaction in ovalbumin sensitized mice. This induces Th1 type reaction and inhibits Th2 cell differentiation. It may be beneficial for glucocorticoids dependent or resistant patients.

[Study of Bax gene in laryngeal carcinoma and it's clinical relationship].

The Bax gene in laryngeal carcinoma, vocal polyp and normal laryngeal tissue were investigated respectively by the SABC immunohistochemistry. Results showed that Bax positive products were brownish yellow granules which were located in the cytoplasm of cancer cells. The positive rate of laryngeal carcinoma, vocal polyp and normal vocal gland were 60.0%(57/95), 50.0%(5/10) and 83.3%(5/6) respectively. There is a significant difference in Bax gene positive expression in laryngeal carcinoma between nonsmoking and smoking patients (P < 0.05). The 3. 5 and 10 years survival rates of Bax positive patients were 86.6%, 84.1% and 79.9% respectively. As to the negative ones, the above rates were 77.2%, 72.5% and 58.0% respectively. The survival curve in patients shown to be positive is higher than that in the negative one, the wave crest of mortality risk in positive patient is lower than that in the negative ones. These results indicated that Bax expression has applicable value in the diagnosis of laryngeal carcinoma and its prognosis and judgement.

[Clinical analysis of hoarseness after thyroidectomy].

The present paper reports 86 cases of hoarseness after thyroidectomy. In 37 cases, glottic paralysis was confirmed. Among them the injury of recurrent laryngeal nerve were 89.91% (33/37). In 33(36 side) cases of recurrent laryngeal nerve paralysis, left injury was 20 and right was 16. Referring to the literature author consider that: 1. the recurrent laryngeal nerve was injured easy by thyroidectomy because that thyroid gland was located closely with recurrent laryngeal nerve in neck; 2. recurrent laryngeal nerve injury after thyroidectomy was related to the character of thyroid gland tumor and times of operations; 3. incidence of superior laryngeal nerve injure in thyroidectomy was rare; 4. following up 16 cases of glottic paralysis, most of all (13/16) hoarseness was improved with the health side vocal cords overcompensation.

[Histopathological study of surgical resection margins of laryngeal carcinoma].

64 specimens of laryngeal carcinoma were studied by using the step-serial whole organ section and histopathologic examination of the region around tumor. We discussed the situation of lesions spreading from tumor to resection margins, the survival rate of different types of resection margins, the common position of positive resection margins and the correlative factors of positive resection margins combining with histopathology, clinical expression and follow-up results. We concluded that resection margins more than 5 mm far from visible tumor is suitable.

[Deletion polymorphism in the angiotensin converting enzyme gene associated with essential hypertension in Hans Chinese population].

OBJECTIVE: To investigate whether the polymorphism of the angiotensin-converting enzyme(ACE) gene is associated with essential hypertension in Chinese. METHODS: A case-control study was carried out using 134 hypertensive (HT) and 165 normotensive (NT) subjects. The insertion/deletion (I/D) polymorphism of ACE gene was detected by polymerase chain reaction (PCR). RESULTS: The difference of genotype and derived allele frequencies for deletion of ACE gene between hypertensive and normotensive subjects were statistically significant(i.e. 51.9% vs 15.7 and 0.69 vs 0.31 for hypertensive and normotensive respectively, P < 0.05). CONCLUSIONS: The possession of D/D homozygote genotype of the ACE gene might be a marker for genetic susceptibility to Hans hypertension in Chinese.
