[A biomechanical model of local control of microcirculation].

In this paper we analyze the generation and motion of vasoactive metabolites in interstitium, the open and closure of precapillary sphincters, the factors affecting mean arteriolar diameter, and the change of amplitude of arteriolar vasomotion. A biomechanical model of local control of microcirculation has been developed. It is useful in quantitative analysis of microcirculatory local control.

[Determination of the saflor yellow-A in Carthamin tinctorius].

The content of the saflor yellow-A in Carthamin tinctorius medicinal materials was determined by the HPLC method. The C18-ODS(150 x 4.5 mm) column and methanol-acetonitrile-0.7% H3PO4(26:2:72) as a mobile phase were used. The flow rate was 1.0 ml/min and the detective wavelength was 403 nm. The average recovery was 100.72% and the relative standard deviation (RSD) was 1.73%.

[Partial laryngectomy plus radiotherapy versus partial laryngectomy alone for laryngeal carcinoma].

OBJECTIVE: To investigate the effect of radiotherapy in the combined treatment of laryngeal cancer with partial laryngectomy. METHODS: One hundred and seventy-one cases of laryngeal cancer were randomly separated into three groups of partial laryngectomy, partial laryngectomy with preoperative radiotherapy (dose 40-50Gy) and partial laryngectomy with postoperative radiotherapy(dose 51-70 Gy). The surgical procedures included cordectomy, vertical partial laryngectomy, horizontal partial laryngectomy and near total laryngectomy. RESULTS: The overall 5-year survival rate was 82% (123/150). The 5-year survival rates for surgery alone group (56 cases), surgery with preoperative radiotherapy group(65 cases) and surgery with postoperative radiotherapy group(50 cases) were 85.7%, 80.7% and 79.5% respectively. There was no significant difference among the groups(chi 2 = 0.703, P = 0.704). Analysis of survival rates revealed a significant difference among the different stages(chi 2 = 12.248, P = 0.007). All 171 cases except 3 that had near total laryngectomy achieved satisfactory phonation. CONCLUSION: Neither preoperative radiotherapy nor postoperative radiotherapy can improve the 5-year survival rate of laryngeal cancer with partial laryngectomy. All patients with partial laryngectomy got satisfactory phonation.

[The clinical application of stereography in laryngology with "the three-dimensional reconstruction system of medical faultage photograph"].

OBJECTIVE: To study the value of the clinical application of stereography in laryngology. METHODS: Taken by Microtek E6 scanner, the CT faultage images were inputted in personal computer and reconstructed to a three-dimensional stereograph by "the three-dimensional reconstruction system of medical faultage photograph". The software was compiled using IDL5.2, a complete, integrated software environment for data analysis, visualization and application. RESULTS: The three-dimensional laryngeal graph could be incised in random, displayed in optional degree, and was rotatable, semitransparent and measurable. The tumor could identified be identified by a special color and revealed through translucent parenchyma and cartilage. Reconstruction stereographs of laryngeal and hypopharyngeal cancer of 26 patients were investigated. CONCLUSION: It is convenient for popularization because the three-reconstruction process is built on a personal computer without the need of any equipment for image acceleration. It is very helpful for ENT surgeon to define the range of the tumor, to fix on surgical pattern accurately and to anticipate the operational course.

[Supracricoid partial laryngectomy in laryngeal cancer].

OBJECTIVE: To assess the effectiveness of supracricoid partial laryngectomy in the treatment of laryngeal cancer and the reconstructed laryngeal functions. METHODS: This study involved 29 patients operated on from 1978 to 1998 using this surgical procedure. In these cases, 18 were male and 11 were female with a mean age of 55.4 years (range, 38 to 70 years). The disease was classified according to the 1987 UICC system. There were 10 supraglottic cancers (3 T2N0, 1 T2N1, 3 T3N0, 2 T3N1, 1 T3N2), and 19 glottic cancers (12T2N0, 1T2N1, 4T3N0, 2T3N1). Two subtypes of supracricoid partial laryngectomy were performed, one with epiglottis preserved and reconstructed with cricohyoidepiglottopexy (CHEP), the other involved epiglottic resection and reconstructed with cricohyoidopexy (CHP). RESULTS: The voice was good in all cases. Twenty-seven cases were decannulated (93.1%). All cases had overcome the aspiration by practice. The overall 3-year and 5-year survival rates were 89.7% and 75.9%. CONCLUSION: Supracricoid partial laryngectomy was appropriate for selected supraglottic and glottic cancers.

[A clinical study on coincidence with hyperthyroidism and thyroid carcinoma].

OBJECTIVE: A retrospective study has been carried out to evaluate the prevalence of coincidence with hyperthyroidism and thyroid carcinoma. METHODS: 394 patients underwent surgery for hyperthyroidism and 245 patients suffered from thyroid cancer were chosen for the study in our hospital from January 1983 to June 1998. RESULTS: Thyroid cancer and hyperthyroidism coincided in 12 patients. The incidence of thyroid cancer was 3.0% (12/394) in hyperthyroidism, and the incidence of hyperthyroidism was 4.9% in thyroid cancer. There were 7 female and 5 male, with a diffuse goiter (n = 3), a diffuse goiter with a cold nodule (n = 3), multinodular goiter (n = 6). Among the 12 patients, 7 patients had an occult thyroid cancer with a diameter of 1 cm or less, most of them with papillary carcinoma, less frequently had metastases, 4 patients had thyroid cancer with a diameter of 3 cm or more, and 3 patients had metastases. CONCLUSIONS: The prevalence of coincidence with thyroid cancer and hyperthyroidism is more than that of thyroid cancer in population. Diagnostics for exclusion of thyroid cancer is required carefully even in the presence of hyperthyroidism.

HPV typing and HPV16 E6-sequence variations in synchronous lesions of cervical squamous-cell carcinoma from Swedish patients.

We microdissected 15 specimens of invasive cervical cancer co-existing with some of its precursors. Out of 15 cases, 10 carried HPV16, 2 HPV31, 1 HPV18 and 2 were HPV-negative. We found 3 HPV16 E6 variants among the 10 cases; one was A --> G in nt 131 (one case) and a second was A --> G in nt 276. The third, T;--> G in nt 350, was common, and was found in 5 of the 10 patients infected by HPV16. The type of HPV and the E6 variant were identical in all lesions within the same patient. Viral DNA present in normal epithelium was identical in type and E6 variant to HPV in the same patient's lesions. Multiple samples from invasive cancers with HPV were consistently positive. The data suggest that the originally infecting HPV, including its variant type in the E6 gene, persists unaltered in the whole series of CIN that precedes invasive cancer. Our data are compatible with an essential role of HPV manifested by persistence of the viral genome during the entire natural life history of cervical cancer. We did not confirm previous data on the specific association of invasive cancer with an HPV E6 variant (G at nt 350 rather than T). The discrepancy may depend on the relatively few cases investigated or selection of a special sub-set with progression from CIN to invasive cancer already manifest.

Cutting edge: blockade of the CD28/B7 costimulatory pathway inhibits intestinal allograft rejection mediated by CD4+ but not CD8+ T cells.

The effect of blocking the CD28/B7 costimulatory pathway on intestinal allograft rejection was examined in mice. Murine CTLA4Ig failed to prevent the rejection of allografts transplanted into wild-type or CD4 knockout (KO) mice but did inhibit allograft rejection by CD8 KO recipients. This effect was associated with decreased intragraft mRNA for IFN-gamma and TNF-alpha and increased mRNA for IL-4 and IL-5. This altered pattern of cytokine production was not observed in allografts from murine CTLA4Ig-treated CD4 KO mice. These data demonstrate that blockade of the CD28/B7 pathway has different effects on intestinal allograft rejection mediated by CD4+ and CD8+ T cells and suggest that these T cell subsets have different costimulatory requirements in vivo. The results also suggest that the inhibition of CD4+ T cell-mediated allograft rejection by CTLA4Ig may be related to down-regulation of Th1 cytokines and/or up-regulation of Th2 cytokines.

Blood glycerol is an important precursor for intramuscular triacylglycerol synthesis.

The utilization of blood glycerol and glucose as precursors for intramuscular triglyceride synthesis was examined in rats using an intravenous infusion of [2-(14)C]glycerol and [6-(3)H]glucose or [6-(14)C]glucose. In 24-h fasted rats, more glycerol than glucose was incorporated into intramuscular triglyceride glycerol in soleus (69 +/- 23 versus 4 +/- 1 nmol/micromol triglyceride/h, respectively, p = 0.02 glycerol versus glucose) and in gastrocnemius (25 +/- 5 versus 9 +/- 2 nmol/micromol triglyceride/h, respectively, p = 0.02). Blood glucose was utilized more than blood glycerol for triglyceride glycerol synthesis in quadriceps. In fed rats, the blood glycerol incorporation rates (4 +/- 2, 8 +/- 3, and 9 +/- 3 nmol/micromol triglyceride/h) were similar (p > 0.3) to those of glucose (5 +/- 2, 8 +/- 2, and 5 +/- 2 nmol/micromol triglyceride/h for quadriceps, gastrocnemius, and soleus muscle, respectively). Glucose incorporation into intramuscular triglycerides was less with [6-(3)H]glucose than with [6-(14)C]glucose, suggesting an indirect pathway for glucose carbon entry into muscle triglyceride. The isotopic equilibrium between plasma and intramuscular free glycerol ([U-(13)C]glycerol) was complete in quadriceps and gastrocnemius, but not soleus, within 2 h after beginning the tracer infusion. We conclude that blood glycerol is a direct and important precursor for muscle triglyceride synthesis in rats, confirming the presence of functionally important amounts of glycerol kinase in skeletal muscle.

A new monoclonal anti-idiotypic catalytic antibody with a CPA-like activity.

IIF9D8, a new monoclonal anti-idiotypic catalytic antibody with a CPA esterase-like activity was elicited by ID11D7, the monoclonal competitive inhibitory antibody to CPA. The hydrolysis of hippuryl-DL-phenyllactic acid by McAb IIF9D8 follows the Michaelis-Menten kinetics. The Km value and kcat are 0.036 M and 0.598 min(-1), respectively, and the rate acceleration (kcat/kuncat) is 30500. Compared with the previous McAb 32C3 induced by polyclonal antibodies to CPA, McAb IIF9D8 shows higher catalytic efficiency The catalytic antibodies with the catalytic properties similar to natural enzymes could be obtained by this approach.

Multiple oligodeoxyribonucleotide syntheseson a reusable solid-phase CPG support via the hydroquinone-O,O'-diacetic acid (Q-Linker) linker arm.

A strategy for oligodeoxyribonucleotide synthesis on a reusable CPG solid-phase support, derivatized with hydroxyl groups instead of amino groups, has been developed. Ester linkages, through a base labile hydroquinone- O, O '-diacetic acid ( Q-Linker ) linker arm, were used to couple the first nucleoside to the hydroxyl groups on the support. This coupling was rapidly accomplished (10 min) using O -benzotriazol-1-yl- N, N, N ', N '-tetramethyluronium hexafluorophosphate (HBTU) and 1-hydroxybenzotriazole as the activating reagents. Oligodeoxyribonucleotide synthesis was performed using existing procedures and reagents, except a more labile capping reagent, such as chloro-acetic anhydride, methoxyacetic anhydride or t-butylphenoxyacetic anhydride, was used instead of acetic anhydride. After each oligodeoxyribonucleotide synthesis, the product was cleaved from the support with ammonium hydroxide (3 min) and deprotected as usual. Residual linker arms or capping groups were removed by treatment with ammonium hydroxide/methylamine reagent and the regenerated support was capable of reuse. Up to six different oligodeoxyribonucleotide syntheses or up to 25 cycles of nucleoside derivatization and cleavage were consecutively performed on the reusable support. This method may provide a significant cost advantage over conventional single-use solid supports currently used for the manufacture of antisense oligodeoxyribonucleotides.

Conformational analysis of an alpha-galactosyl trisaccharide epitope involved in hyperacute rejection upon xenotransplantation.

alpha-Galactosyl epitopes are carbohydrate structures bearing an alpha-Gal-(1-->3)-Gal terminus (alpha-Gal epitopes). The interaction of these epitopes on the surface of animal cells with anti alpha-Gal antibodies in human serum is believed to be the main cause in antibody-mediated hyperacute rejection in xenotransplantation. In this paper, conformational analysis of an N-linked alpha-D-Galp-(1-->3)-beta-D-Galp-(1-->4)-beta-D-Glcp trisaccharide epitope was conducted in terms of each monosaccharide residue conformation, primary hydroxymethyl group configuration, and interglycosidic conformations. Selective 2D J-delta INEPT experiments have been carried out at three different temperatures to evaluate three-bond, long-range 13C-1H coupling constants for the crucial alpha-(1-->3) linkage. The NMR experimental data were complemented by theoretical calculations. The flexibility and dynamics of the trisaccharide have been studied by Metropolis Monte Carlo simulations. Ensemble-averaged three-bond, long-range 13C-1H coupling constants and nuclear Overhauser effects were in good agreement with the experimental data. The alpha-(1-->3) glycosidic linkage has shown a restricted flexibility as indicated by NMR spectroscopy and molecular modeling.

Occurrence and progression of dementia in a community population aged 75 years and older: relationship of antihypertensive medication use.

OBJECTIVE: To examine whether antihypertensive medication use can affect the occurrence and progression of dementia. SUBJECTS AND METHODS: In a community cohort of 1810 persons aged 75 years and older, 225 prevalent cases of dementia were detected. Among the 1301 persons without dementia, 224 incident cases of dementia were identified during an average period of 3 years. Among the 225 prevalent cases of dementia, 79 were suitable for the analysis of cognitive decline. Information on drug use was collected for the 2 weeks preceding the baseline interview. RESULTS: Subjects taking antihypertensive medication (n = 651, 83.9% of whom took diuretics) had a lower prevalence of dementia than those not taking antihypertensive medication (P<.001). Subjects without dementia who were taking antihypertensive medication at baseline (n = 584) had a reduced incidence of dementia (adjusted relative risk, 0.7; 95% confidence interval, 0.6-1.0; P = .03). Furthermore, subjects taking diuretics (n = 484) had an adjusted relative risk of 0.7 (95% confidence interval, 0.5-1.0; P = .02) for all dementia, and subjects taking diuretic monotherapy (n = 345) had an adjusted relative risk of 0.6 (95% confidence interval, 0.4-0.9; P = .006). The use of other antihypertensive medication (calcium antagonists or beta-blockers), however, was related to a reduced risk of Alzheimer disease (adjusted relative risk, 0.6; 95% confidence interval, 0.3-1.2) only in the subpopulation with a higher baseline blood pressure (n = 458). Patients with dementia at baseline who were not taking diuretics had a 2-fold faster rate of decline in the score on the Mini-Mental State Examination than those taking diuretics. CONCLUSION: The use of diuretics may protect against dementia in elderly persons.

DNA repair in lymphoblastoid cell lines from patients with head and neck cancer.

OBJECTIVE: To analyze and compare components of the 3 primary DNA repair pathways of Epstein-Barr virus-transformed lymphocyte (lymphoblastoid) cell lines derived from 9 patients with squamous cell carcinoma of the head and neck and 11 cancer-free controls. These cell lines were previously characterized by using an established cytogenetic marker of cancer susceptibility (mutagen sensitivity assay). DESIGN: To evaluate nucleotide excision repair (NER), we measured the reactivation level of a tobacco carcinogen-damaged plasmid containing a bacterial reporter gene transfected into these cells. To assess mismatch repair (MMR) and recombinational repair, selected gene transcript levels were quantified by using a multiplex reverse transcriptase-polymerase chain reaction assay. The results of these DNA repair assays were correlated with the previously measured mutagen sensitivity values. RESULTS: The NER capacities of the 2 groups were similar: 25.1% (range, 14.3%-33.3%) for the patient cell lines and 26.0% (range, 9.4%-47.7%) for the control lines. Transcriptase levels for 6 MMR genes (hMSH3, hMSH2, hPMS2, GTBP, hMLH1 and hPMS1) did not differ in the 2 groups. Transcript levels for 4 of 6 recombinational repair genes (XRCC7, XRCC6, XRCC1, and RAD51) were higher in the patient cell lines, though this difference was significant only for XRCC7 (P = .003). The mutagen sensitivity values correlated with the NER capacity (P = .05) and the expression of XRCC4 (P = .01) and RAD51 (P = .06) genes. CONCLUSIONS: As revealed by the above-named assays, these lymphoblastoid cell lines derived from patients with head and neck cancer had minor alterations in DNA repair function. However, these differences in DNA repair do appear to affect the cytogenetic marker of cancer susceptibility, mutagen sensitivity.

Changes in blood pressure, body mass index, and salt consumption in a Chinese population.

BACKGROUND: The prevalence of hypertension among the population of Tianjin is one of the highest in China. We attempted to assess the changes in blood pressure, body mass index, and salt consumption in Tianjin from 1989 to 1996. METHODS: Two independent cross-sectional population surveys with the representative samples of 14,046 persons in 1989 and 2,000 persons in 1996 were carried out in Tianjin. A salt surveillance system was established with four salt consumption surveys (in 1992, 1993, 1994, and 1996) to estimate the changes in salt consumption at the household level. RESULTS: From 1989 to 1996, the prevalence of hypertension and obesity decreased among people aged 45-64 years in both genders. Mean systolic blood pressure decreased significantly in women and leveled off in men. Diastolic blood pressure increased significantly. Body mass index remained unchanged. The unfavorable trends were mainly attributable to the negative changes in younger people (15-34 years) in both genders. There were no changes in salt consumption between 1992 and 1996 at the house level. CONCLUSIONS: There was a favorable development in prevalence of hypertension and obesity among individuals in the age group of 45-64 years in the study population. Health education needs to pay more attention to young people.
