[Pulmonary lymphangitic carcinomatosis].

OBJECTIVE: To explore the clinical manifestations of pulmonary lymphangitic carcinomatosis (PLC), to analyse its associated diagnostic methods, and to improve the understanding of PLC and its diagnosis. METHOD: Retrospective analysis of 4 cases of PLC and review of the literature. RESULT: The clinical manifestations of PLC include: (1) dyspnea and cough; (2) normal or restrictive pattern ventilation; (3) diffuse or local reticulonodular infiltrates in the lung like interstitial fibrosis and pleural effusion on chest radiograph; (4) CT and high-resolution CT (HRCT) scans reveal a beaded chain appearance caused by uneven thickening of the interlobular septa and pleural membrane, polygonal thickening of bronchovascular bundles, and mediastinal lymphadenopathy as well. CONCLUSION: These clinical data suggest that any manifestations similar to pulmonary interstitial fibrosis complicated with pleural effusion and paratracheal lymphadenopathy should be further differentiated from PLC by HRCT and pleural-lung tissue biopsy.

[Living related liver transplantation: a case report].

OBJECTIVE: To study a case of living related liver transplantation. METHOD: The patient was a 10-year-old girl who suffered from congenital diffuse intrahepatic cholangiectasis, recurrent cholangitis and hepatocirrhosis. The donor was the patient's father aged 40. The left lateral lobe of donor's liver was cut and grafted to the patient. Intensive care and treatment as well as follow-up were given after operation. RESULT: For the donor, the operation lasted 400 min, with 410 ml bleeding and 300 g liver removed. The recovery was satisfactory. For the recipient, the operation lasted 652 min, with 1665 ml bleeding, 80 min non-liver stage, 0 min graft hot ischemic stage, and 137 min cold ischemic stage. Immunosuppressive therapy was given using cyclosporin A, azathioprine and adrenocortical hormones. There was an acute rejection 11 days after operation, which was controlled using hormone impulsive therapy. The patient has been surviving 7 months with normal liver function. CONCLUSION: The living related liver transplantation is feasible under modern surgical conditions. It is demonstrated that perfect postoperative management is the key for the successful liver transplantation.

[The indication and mode of surgery for the treatment of patients with severe acute pancreatitis].

OBJECTIVE: To study the indication and mode of surgical management in patients with severe acute pancreatitis (SAP). METHOD: 41 patients with SAP were grouped for conservative treatment or surgical. The main procedures of surgical management were pancreatic debridement, peripancreatic and coeliac drainage, combined with peripancreatic instillation and suction alternatively. RESULT: Conservative treatment was adopted in 22 patients (16 patients in grade I, 6 in grade II), among them 19 recovered, and 3 of grade II died. 19 patients underwent surgical treatment including 7 in grade I, 12 in grade II). 15 patients were cured, 4 patients of grade II died. CONCLUSION: Timely operation is necessary for the patients with infection and complications. Surgical intervention is recommended for patients older than 60 years old. Surgery should be less traumatic, simple and prompt. Surgical or conservative treatment are chosen according to the patients' conditions. The cure rate of SAP can be improved.

[The normal value and clinical applications of the blink reflex].

OBJECTIVE: To study the normal value and clinical applications of blink reflex. METHODS: Using the clinical electrodiagnostic techniques, the orbital nerve was stimulated to elicit the blink reflex. RESULTS: In 20(40 sides) normal subjects, two components (R1 and R2) had been documented. The latencies were about 10.1 ms for R1, 30.4 ms and 30.7 ms separately for R2 and R2'. In 30 patients with trigeminal and facial nerve diseases and acoustic neuromas, the latencies on the affected sides were apparently longer or the reflex disappeared (P < 0.01). CONCLUSION: This study indicates that the simple, harmless electrodiagnostic measurements are of value in the initial investigation of trigeminal and facial nerve diseases and acoustic neuroma.

Development of a new kind of dual modulated QCM biosensor.

To distinguish the mass loading effect from the total frequency change is a problem in the application of a quartz crystal microbalance (QCM) biosensor in the liquid phase. Based on the characteristic damping theory, this paper proposes a new method of dual modulation to solve this problem. Using polyethyleneimine to immobilize anti-SE (staphylococcin enterotoxin) antibody (C2 type), a dual modulated QCM SE biosensor was developed and the experiment proved that it has little cross-reaction with B-type SE. The measuring curve of the sensor was also determined through experiment.

Survival of FimH-expressing enterobacteria in macrophages relies on glycolipid traffic.

Strains of Escherichia coli persist within the human gut as normal commensals, but are frequent pathogens and can cause recurrent infection. Here we show that, in contrast to E. coli subjected to opsonic interactions stimulated by the host's immune response, E. coli that bind to the macrophage surface exclusively through the bacterial lectin FimH can survive inside the cell following phagocytosis. This viability is largely due to the attenuation of intracellular free-radical release and of phagosome acidification during FimH-mediated internalization, both of which are triggered by antibody-mediated internalization. This different processing of non-opsonized bacteria is supported by morphological evidence of tight-fitting phagosomes compared with looser, antibody-mediated phagosomes. We propose that non-opsonized FimH-expressing E. coli co-opt internalization of lipid-rich microdomains following binding to the FimH receptor, the glycosylphosphatidylinositol-linked protein CD48, because (1) the sterol-binding agents filipin, nystatin and methyl beta-cyclodextrin specifically block FimH-mediated internalization; (2) CD48 and the protein caveolin both accumulate on macrophage membranes surrounding bacteria; and (3) antibodies against CD48 inhibit FimH-mediated internalization. Our findings bring the traditionally extracellular E. coli into the realm of opportunistic intracellular parasitism and suggest how opportunistic infections with FimH-expressing enterobacteria could occur in a setting deprived of opsonizing antibodies.

Preparation and Characterization of Active Carbon Adsorbents for Wastewater Treatment from Elutrilithe

Active carbon adsorbents were prepared from natural elutrilithe by chemical activation with K2CO3. The effect of pyrolysis temperature and time and K2CO3/elutrilithe ratio on the surface area, porosity, and ash content of the adsorbents was studied. Various prior and post treatments have been tried to improve the quality of the adsorbents. An ideal active carbon adsorbent with a BET surface area of 1236 m2/g and a total pore volume of 0.679 cm3/g has been obtained. The adsorbent is hydrophobic in nature and exhibits large adsorption capacities for various phenolic compounds from aqueous solutions. Copyright 1997Academic Press

[Epstein-Barr virus infection and the expression of bcl-2 proto-oncogene in Hodgkin's disease].

OBJECTIVE: To investigate the relationship between Epstein-Barr virus (EBV) and bcl-2 gene. METHODS: Sixty-four paraffin-embeded cases of Hodgkin's disease (HD) were studied. Immunohistochemistry was performed by using monoclonal antibody to latent membrane protein 1 (LMP-1) and bcl-2. In situ hybridization for EBV encoded small mRNA-EBER on 41 cases. Double staining combined in situ hybridization and immunohistochemistry was performed for dual labelling of EBER and bcl-2 in seven cases. RESULTS: The positive rates of LMP-1 and EBER were 39% and 44%. All cases which were EBER positive, also express LMP-1. The positive rate of bcl-2 protein was 23%. Only seven of these cases were also LMP-1 positive. CONCLUSION: All the results suggest that the bcl-2 expression in the R-S cells in this series did not correlate with EBV-positivity.

[Risk of leukemia among diagnostic x-ray workers in China between 1950 and 1990].

OBJECTIVE: To explore the relationship between radiation and leukemia. METHODS: Leukemia incidences (1950-1990) in 27011 diagnostic X-ray workers and in 25782 other medical specialists employed between 1950 and 1980 in China were compared. RESULTS: The relative risk (RR) of leukemia in the diagnostic X-ray workers was 2.25-fold (95% C1=1.59-3.09, P<0.001) higher than that in the control group. The average age of the emergence of leukemia was 10.4 years younger in X-ray workers than in control group. The peak incidence of leukemia in X-ray workers occurred 10-14 years after engaging in X-ray work. The highest RR of leukemia was found in those who were engaged in X-ray work before 1965 or at age under 20 years. CONCLUSION: Incidence of Leukemia in X-ray workers increased and it was correlated with radiation dosage and duration.

[A laplace transform method for analysis of fluorescence dynamics].

A Laplace transform formula is developed for analysis of a multi-exponential fluorescence dynamic process. A dynamic spectrum function is introduced, calculation of which directly yields parameters of the corresponding dynamic process. The formula is applied to analyze fluorescence dynamics of beta subunit from R-phycoerythrin. It is demonstrated that electronic excited-states of the biomolecular system exhibit two exponential decay processes, the faster of which relates to excitation equilibrium among chromophores, and the slower to radiative relaxation of the chromophores.

[Mulivariate analysis of prognostic factors in non-small cell lung cancer].

OBJECTIVES: To find independent prognostic factor of non-small cell lung cancer (NSCLC), identify the population with "high-risk" of recurrence and death in resected NSCLC, and direct adjuvant therapy for clinical doctors. METHODS: 85 resected patients from NSCLC were followed up. All patients were subjected to the study by light microscope, immunochemistry, PCR-SSCP method, and DNA image analysis. Thirteen clinical, pathological, and biological factors were analysed by Cox regression model. RESULTS: The median follow-up period was 47 months. The overall 3-year survival rate was 66% and the overall 5-year survival rate 61%. Cox regression showed significant univariate predictors (P < 0.05) of early recurrence and death for NSCLC were lymphnode status, pTNM, tumor emboli, microvessel count, Ki-67 antigen labeling, DNA relative content and p53 gene mutation. Microvessel count (P < 0.001) and pTNM (P = 0.006) were independent predictors of early recurrence and cancer-death. According to the multivariate model for predicting early recurrence and cancer death, prognostic index (PI) was calculated. CONCLUSION: The PI may be a useful clinical tool for evaluating prognosis of patients and identifying the population with "high-risk" of recurrence and death in NSCLC.

[Analysis of mutation types of p53 gene and possible carcinogenic factors in lung cancer].

Mutational spectrum of p53 gene was analyzed by polymerase chain reaction (PCR)-single strand conformational polymorphism (SSCP) and direct DNA sequencing technique in tissue specimens of primary non-small cell lung cancer (NSCLC) to study the action of p53 gene, its type of mutations and its possible relation to different carcinogens. Results revealed that mutation of p53 gene was found in 94 of 128 resected specimens of NSCLC, including point mutations in 86 cases, deletion and insertion in seven and mutation of the linking site in one, and 63.9 percent of point mutations were G-T and T-G substitution and 25.6 percent G-A and A-G. It suggested that p53 gene mutation correlated closely to the occurrence of NSCLC. Possible environmental carcinogens were discussed in the paper based on the characteristics of the p53 gene mutation.
