Involvement of electrical coupling in the in vivo ictal epileptiform activity induced by 4-aminopyridine in the neocortex.

In the present study we have investigated the possible role of gap junctions in the induction and manifestation of 4-aminopyridine-induced acute seizure activity both at the primary focus and at the mirror focus in anaesthetized rats by combining electrophysiological, pharmacological and molecular biological techniques. In the course of the intracellular recordings, unusual firing patterns that are assumed to be mediated by electrical coupling and appearing either randomly or in close time-locked manner with the ictal discharges were observed. In another series of experiments, a significant decrease in the intensity of seizure activity of the already active epileptic foci was detected when electrical synaptic transmission was blocked by carbenoxolone either at the primary focus or at the mirror focus. When electrical synaptic transmission was depressed relative to the initial baseline prior to the induction of epileptic focus, only a mild influence on the induction of seizure discharges occurred. The role of the gap junctional communication in the epileptiform activity was further investigated by following the expression pattern of two connexin genes. Both, connexin-32 and connexin-43 mRNA levels were significantly elevated at the primary focus as well as at the mirror focus, after 60 min of repeated ictal discharges. We conclude that gap junction communication probably became a part of the neuronal synchronization both in the primary and in the secondarily-induced acute epileptiform activity in the neocortex in vivo. These results, together with earlier observations, indicate a direction for the development of new drugs targeting gap junctions for therapeutic intervention.

[Mechanical durability characteristics of polyethylene cup in total hip arthroplasty].

The paper presents an analysis of the processes that determine the durability of the polyethylene cup of the Weller total hip prosthesis. Mechanisms of wear of the polyethylene cup have been presented. It has been inferred that the process of loosening of the polyethylene cup is a combination of plastic deformation, tribologic wear, fracture and changes of the polyethylene structure. All these occurrences are connected with reaction of bone tissue to stress and polyethylene debris.

[Use of modified Weber's tension band wiring for treatment of ankle fractures].

Results of operative treatment of 16 ankle fractures using modified Weber's method in patients aged 20-67 (mean 45) are presented. An AO screw placed in tibia plafond as a point of anchorage of wire in Weber's method was used. Mazur's criteria were used to assess functional outcome. Excellent and good results were achieved in 13 cases. 3 rather fair results were observed.

[Experimental in vitro investigations of hardness changes in hip prosthesis polyethylene cups caused by x-rays and aging].

The aim of this paper was to determine the influence of X-rays and ageing on the mechanical properties of polyethylene cups of Weller type hip prostheses. Polyethylene cups produced in the period between 1987 and 1992 were investigated. Some of the cups were exposed to a radiation dose exceeding greatly the average dose used in a routine X-ray examination. Polyethylene cups produced in different years and those subjected to "artificial ageing" (long-term exposure to high temperature) were examined. Brinell's method was used to assess the hardness of polyethylene. The results of this study indicate that both exposure to X-rays and ageing lead to hardening of the cups. This may in turn cause cup fractures--a potential source of complications following hip replacement procedures.

[The last unpublished paper of Ludwik Bierdowski].

In the last year of his life professor Ludwik Bierkowski (1801-1860) introduce a new method of treatment for open fractures, which was not published because of his untimely death. This method was described by his son Włodzimierz Bierkowski (1832-1888) but it is still in manuscript. However the method was used by his students.

[Medical ethics and patient responsibility].

When we say "medical ethics" we understand the responsibility of the physician for his medical education and his attitude to his patient. But Hippocrates is known to have said that the efficiency and good results of the treatment depends not only on the physician but on the patient and his engagement, his observance of the doctors' advice, his attitude to his own psyche and body, both in health as in illness. This is an ethical problem known to every practitioner, the problem of ethics of the patient, which ought to be more widely disseminated in society.
