Selected statistical issues in group randomized trials.

Group randomized trials (GRTs) in public health research typically use a small number of randomized groups with a relatively large number of participants per group. Two fundamental features characterize GRTs: a positive correlation of outcomes within a group, and the small number of groups. Appropriate consideration of these fundamental features is essential for design and analysis. This paper presents the fundamental features of GRTs and the importance of considering these features in design and analysis. It also reviews and contrasts the main analytic methods proposed for GRTs, emphasizing the assumptions required to make these methods valid and efficient. Also discussed are various design issues, along with guidelines for choosing among them. A real data example illustrates these issues and methods.

Central melanocortin receptors regulate insulin action.

Energy balance and insulin action are tightly coregulated. Leptin regulates energy intake and expenditure partly by modulation of the melanocortin pathway in the hypothalamus. Here we demonstrate potent effects of the melanocortin pathway on insulin action and body distribution of adiposity. Conscious rats received week-long infusions of either a melanocortin receptor agonist, alpha-melanocyte-stimulating hormone (alpha-MSH), or antagonist, SHU9119, in the third cerebral ventricle while food intake was maintained constant in each group. alpha-MSH decreased intra-abdominal fat and markedly enhanced the actions of insulin on both glucose uptake and production, while SHU9119 exerted opposite effects. Our findings elucidate a neuroendocrine network that is likely to play a central role in the coupling of energy intake and insulin action.

Overfeeding rapidly induces leptin and insulin resistance.

In common forms of obesity, hyperphagia, hyperinsulinemia, and hyperleptinemia coexist. Here, we demonstrate rapid induction of insulin and leptin resistance by short-term overfeeding. After 3 and 7 days on the assigned diet regimen, rats were tested for their biological responses to acute elevations in plasma insulin and leptin concentrations. Severe resistance to the metabolic effects of both leptin and insulin ensued after just 3 days of overfeeding. During the insulin clamp studies, glucose production was decreased by approximately 70% in control rats and 28-53% in overfed rats. Similarly, leptin infusion doubled the contribution of gluconeogenesis to glucose output in control rats but failed to modify gluconeogenesis in overfed animals. These findings demonstrate a paradoxical and rapid collapse of the leptin system in response to nutrient excess. This partial failure is tightly coupled with the onset of insulin resistance.

Control of clonorchiasis by repeated treatments with praziquantel.

The present study aimed to evaluate control efficacy of clonorchiasis by two schemes of repeated treatment with praziquantel at two endemic villages in China. Residents of one village at Guangxi Autonomous Region were treated and examined 6-monthly and of another at Liaoning Province 12-monthly. In residents that took 25 mg/kg x 3 (total 75 mg/kg) of praziquantel every 6 months for one year the egg positive rate showed a significant drop from 69.0% to 17.1%. In contrast, a group of same praziquantel medication once showed a slight marginal decrease in the egg rate from 18.9% to 12.2% after one year. Of 39 subjects examined 3 times, 56.4% were cured, 7.7% persistently positive, one (2.6%) reinfected after cure or newly infected, but 25.6% were persistently negative. The present finding suggests that 6-monthly medication with 75 mg/kg of praziquantel should effectively lower the prevalence but incomplete for control of clonorchiasis in heavy endemic areas.

A baseline study on the importance of bovines for human Schistosoma japonicum infection around Poyang Lake, China.

We hypothesize that bovine infections are responsible for the persistence of human schistosomiasis transmission in the Yangtze marshlands of China. To test this hypothesis, we are carrying out a comparative intervention among four administrative villages in the Poyang Lake region, Jiangxi Province, two of which are experimental and two are control. The primary design involves treating, at the onset of the study, all the inhabitants in all four villages with praziquantel and all the bovines in two villages (the experimental or intervention villages). Following treatment, rates of reinfection in people of all villages, and in bovines in the experimental villages, will be assessed as will the ongoing prevalence of infection in bovines in the control villages. Before treatment, the prevalence and intensity of infection among humans and bovines was ascertained in the four villages. Our study design and baseline information are presented here, along with a description of the ecology of the study villages.

[A permanent implantable pulsatile impeller heart with a specially devised needle bearing].

The author's implantable pulsatile impeller pump can assist the circulation of the calves for several months. The termination of the experiments was related to wear of the mechanical bearing, which resulted in vibration of the rotor and pump failure. It seemed as if the experiments could have lasted indefinitely if the bearing had not failed. To solve the problem of bearing wear, a specially devised rolling bearing was investigated and applied to the impeller pump. Its service life is more than 10 years due to the wear-proof roller made of ultra-highmolecular weight polythene. This newly devised impeller heart promises to have long-term and permanent applications. Compared with the magnetic bearing, the rolling bearing has the advantages of simpler construction and control, no consumption of energy, and better reliability.

[The modified process for preparing natural organic polymer flocculant chitosan].

The modified process for preparing chitosan from crab or lobster shells was developed. In the decalcification stage, 10% HCl was used as soaking solution with addition of a small quantity of A as a promoter, and the mass ratio of reactants was 10% HCl:A:crab or lobster shells = 3.5:0.5:1, continuously stirring the crab or lobster shells at 30 degrees C for 3 h in place of simply soaking the crab or lobster shells at room temperature for 16-24 h in the previous process. In the deacetylation stage, 40% NaOH solution was used with addition of a small quantity of B as a promoter, and the mass ratio of reactants was 40% NaOH:B:chitin = 4:0.2:1, keeping reaction at 105 degrees C for 2 h in place of at 115 degrees C for 6 h in the previous process. By this new process, the cost of the raw materials used for preparing chitosan was cut down 49%, the preparation time was shortened by one half, and the main properties of this chitosan such as viscosity, deacetylation and molecular weight all approached or exceeded those of the Sigma' commercial chitosan (Chitosan C-3646).

[Pulsatile rotary pumps with low hemolysis].

As is well known, a pulsatile flow is important in assisted-circulation but it is difficult to produce a pulsatile flow with rotary pump, because excessive hemolysis will be generated. The authors have found that the turbulent shear is the main factor for red cell damage and therefore the key point of pulsatile rotary pumps is to reduce the turbulence by producing a pulsatile flow. In the authors' pulsatile axial pump, the pulsatile flow is obtained by axial reciprocation of constant rotating impeller; the rotation and reciprocation of the impeller are driven separately by a DC motor and a pneumatic device. Though a physiological pulsatile flow could be achieved and turbulence would not increase remarkably because the impeller rotates constantly, a second driver except a DC motor is nevertheless necessary, thus the system will become complicated. In the authors' pulsatile radial pump, a pulsatile flow is achieved by changing the rotating speed of the impeller periodically. Turbulence is minimized by a special design of twisted vanes which enable the blood flow to change its direction rather than its dimension during periodic change of rotating speed. Hemolysis tests demonstrated that the index of hemolysis(IH) of the author's pulsatile radial pump is 0.020, with is slightly more than that of the author's nonpulsatile radial pump(IH = 0.015). Animal experiments indicated that the pulsatile radial pump can assist the circulation of calves for several months without harm to blood elements and organ functions of the recipients.

[Analysis and experimental study of reasons for breakage in the bone fracture plate during internal fixation].

Bone fracture plate has been in clinical use for internal fixation of fracture, however, during healing, the steel plate breaks occasionally. This paper applied the static strength theory to imitate the pressed down combination out of shape that was borne by single limb of human body standing. Fracture was made on low limbs of the fresh dead body, fixed with a bone fracture plate. Strain measure was made on a bone fracture plate. Additionally, fatigue experiment and four-point bending experiment were carried out on a bone fracture plate. The reason for a breakage in the bone fracture plate was analyzed with static strength theory and low period fatigue theory.

[Apoptosis of myocardial cells in dilated cardiomyopathy].

OBJECTIVE: To study apoptosis of myocardial cells in dilated cardiomyopathy (DCM) and its relation with cardiac function. METHODS: Myocardial samples of 21 patients in a DCM group were obtained from hearts of 14 patients undergoing endomyocardial biopsy (EMB) in right ventricular (EMB sub-group) and from the hearts of the 7 remaining patients at autopsy (Autopsy sub-group). Myocardial samples of a control group were obtained from hearts of 5 patients at autopsy, who died of causes other than cardiovascular disease. Apoptosis was evaluated histochemically by in situ end-labeled DNA (Tunel method). RESULTS: (1) Apoptotic index (AI) of the DCM group was significantly higher than that of the control group(P < 0.01), AI of EMB sub-group was significantly lower than that of autopsy sub-group (P < 0.01), but significantly higher than that of the control group (P < 0.01). (2) AI in patients with heart-thorax ratio (HTR) < 0.6, left ventricular end diastolic diameter (LVDd) < 65 mm and left ventricular ejection fraction (LVEF) > or = 40% was significantly lower than that of patients with HTR > or = 0.6, LVDd > or = 65 mm and LVEF < 40% respectively (P < 0.01 for all), but was significantly higher than that of the control group respectively (P < 0.01 for all). CONCLUSIONS: Apoptotic myocardial cells were frequently observed in the myocardial tissues of the DCM and the magnitude of apoptosis increased with the deterioration of cardiac function, indicating that apoptosis may be an important mechanism of loss of myocardial cells and cardiac dysfunction in the DCM.

[The role of insulin-like growth factor-I gene expression abnormality in pathogenesis of diabetic peripheral neuropathy].

OBJECTIVE: To explore the role of insulin-like growth factor-I (IGF-I) gene expression abnormality in neurotrophic causes of diabetic peripheral neuropathy. METHODS: Diabetes was induced in Sprague Dawley rats by alloxan. Various parameters were measured as follows: IGF-I mRNA by reverse-transcriptase polymerase chain reaction (RT-PCR), IGF-I peptide by enzyme-linked immunosorbent assay (ELISA), electrophysiological parameters of nerves by evoked electromyogram, morphometric evaluation of sciatic nerve under light microscope and transmission electron microscope. RESULTS: During the early diabetic stage, IGF-I mRNA (0.430 +/- 0.031 vs 0.370 +/- 0.016 +/- 0.280 +/- 0.010, P < 0.01, < 0.001) and IGF-I peptide contents [(0.430 +/- 0.031) ng/mg vs (30.06 +/- 3.60 +/- 23.71 +/- 2.70) ng/mg, P < 0. 01, < 0.001] in sciatic nerve tissue reduced in diabetic rats with hyperglycemia and varied with severity of diabetes. These parameters were further down regulated in diabetic rats with less duration of diabetes and non-diabetic control rats [IGF-I mRNA 0.320 +/- 0.021 - 0.230 +/- 0.060; IGF-I peptide (28.8 +/- 3.30 - 19.51 +/- 1.80) ng/mg]. They also correlated with nerve functional (sensory nerve conduction velocity: r = 0.714, P < 0.001, amplitude of evoked potentials: r = 0.716, P < 0.001, respectively) and structural abnormality (axonal size r = 0.81, P < 0.001) of the sciatic nerve. No difference was found in the above parameters between diabetic rats with euglycemia and non-diabetic control group. CONCLUSION: IGF-I gene expression in tissues was down-regulated from the early diabetic stage and the down-regulation varied with the severity and duration of diabetic state. The decrement in IGF-I level might contribute to the initiation and development of diabetic neuropathy via autocrine or paracrine pathway.

[Vegetation carbon storage and density of forest ecosystems in China].

To improve the estimation of carbon pool of forest ecosystems is very important in studying their CO2 emission and uptake. The estimation of vegetation carbon pool in China has just begun. There is a significant difference among estimates from different methods applied. Based on forest inventory recorded by age class, the vegetation carbon storage of forest ecosystems in China was estimated to be 3.26-3.73 Pg, accounting for 0.6-0.7% of the global pool. The carbon densities were difference among forest types and provinces, in range of 6.47-118.14 Mg.hm-2. There is an incremental tendency from southeast to north and west. This trend is negatively related with the change in population density in logarithmic mode, which indicates that the actual forest carbon density is prominently determined by human activities.

[Regulatory effect of Bcl-2 on the cortical neurons of primary cultured mice with HSV-1 infection].

OBJECTIVE: To study the effect of Bcl-2 on the cortical neurons of primary cultured mice with HSV-1 infection. METHODS: Analysis of the expression of Bcl-2 on the cortical neurons of primary cultured fetal mice in vitro infected with HSV-1 for 11 hours or exposed to sorbitol for 5 hours was made by flow cytometry and Western blotting. RESULTS: The Bcl-2 of the cultured neurons undergoing HSV-1 infection expressed upregulating compared with the mock untreated neurons. The Bcl-2 protein of HSV-1 infected and exposed to sorbitol neurons expressed upregulating compared with the control group. The Bcl-2 protein of the primary cultured 3 day cells expressed higher than that of the primary cultured 7 day cells. The higher neurovirulence, the higher expression of Bcl-2. CONCLUSIONS: Bcl-2 might protect the primary cultured cortical neurons of fetal mice from apoptosis where infected with HSV-1, and thereby the lifespan of host cells may be prolonged.

Cloning and expression of MP13 gene from rat hippocampus, a new factor related to guanosine triphosphate regulation.

C-Fos and the Fos-related antigens (FRA) are induced by various stimuli. A novel 35-37 kDa FRA was induced much longer after the treatment using kainic acid (KA) and may be very important for neuronal survival after brain damage. To identify this long-term FRA, we have constructed a cDNA library derived from hippocampus after KA treatment and screened it with an antibody highly conserved M-peptide region of FRAs. One gene, MP13, was cloned with a 1662 bp open reading frame and coded for a 554-amino acid protein. MP13 has a leucine zipper region, a glutamine repeat region, and has high similarity to the activator of the small guanosine triphosphate (GTP)ase Rab5. Gel retardation analysis revealed that MP13 functions as a GTP regulation related factor.

Comparison of efficacy and side effects of combination therapy of angiotensin-converting enzyme inhibitor (benazepril) with calcium antagonist (either nifedipine or amlodipine) versus high-dose calcium antagonist monotherapy for systemic hypertension.

The present 2 multicenter studies were designed to evaluate whether patients with essential hypertension derived equal benefits from use of combination therapy with a calcium antagonist and angiotensin-converting enzyme (ACE) inhibitor as from doubling the dose of the calcium antagonist. After a 2-week washout and a 2-week single-blind placebo run-in period, a total of 1,390 patients were treated with either nifedipine 30 mg (study 1) or amlodipine 5 mg (study 2) once daily for 4 weeks. The 1,079 patients whose diastolic blood pressure remained between 95 and 115 mm Hg were randomized to 8 weeks of double-blind therapy with amlodipine 5 mg/benazepril 10 mg, amlodipine 5 mg/ benazepril 20 mg, nifedipine 30 mg or nifedipine 60 mg (study 1), and amlodipine 5 mg/benazepril 10 mg, amlodipine 5 mg/benazepril 20 mg, amlodipine 5 mg or amlodipine 10 mg (study 2). Both doses of the calcium antagonist/ACE inhibitor combination therapy lowered diastolic pressure as much as the high dose and significantly better than the lower dose of calcium antagonist monotherapy (with either nifedipine or amlodipine). However, 15% of patients in the nifedipine high-dose monotherapy group and 24% in the amlodipine high-dose monotherapy group presented with some form of edema. In contrast, the incidence of edema was similar for patients treated with both combination therapy and low-dose calcium antagonists. Thus, combination therapy with a calcium antagonist and an ACE inhibitor provides blood pressure control equal to that of high-dose calcium antagonist monotherapy but with significantly fewer dose-dependent adverse experiences such as vasodilatory edema. Construction of transgenic mice with tissue-specific acceleration of mitochondrial DNA mutagenesis.

Transgenic mice having rapid accumulation of mitochondrial DNA (mtDNA) mutations specifically in the heart were created. These mice contained a transgene encoding a proofreading-deficient, mouse mitochondrial DNA polymerase (pol gamma) driven by the promoter for the cardiac-specific alpha-myosin heavy chain. Starting shortly after birth greater than 95% of all pol gamma mRNA in the heart was transgene derived; expression in other tissues was low or absent. Mutations in cardiac mtDNA began to accumulate by 7 days after birth. At 1 month of age the frequency of point mutations was 0.014% as determined by DNA sequencing of cloned mtDNA. By long-extension PCR multiple different deletion mutations that had removed several thousand basepairs of genomic sequence were also detected. Sequencing of two deletion molecules showed that one was flanked at the breakpoint by direct repeat sequences. The expression of proofreading-deficient pol gamma had no apparent deleterious effect on mitochondrial DNA and protein content, gene expression, or respiratory function. However, associated with the rise in mtDNA mutation levels was the development of cardiomyopathy as evidenced by enlarged hearts in the transgenic mice. These mice may prove to be useful models to study the pathogenic effects of elevated levels of mitochondrial DNA mutations in specific tissues.

An intervention study on screening for breast cancer among single african-american women aged 65 and older.

PURPOSE: Older single African-American women are the population that is least likely to use screening procedures because of cognition-related, income-related, social-support-related and medical care-related barriers. This study aims to evaluate a breast screening intervention program developed according to socioeconomic, cultural, psychological and behavioral characteristics of older single African-American women.METHODS: Ten public housing complexes were randomly assigned to either intervention or control group. African-American women aged 65 and over were recruited into the study if they were widowed, divorced, separated or never-married in the preceding year, and did not have a history of breast cancer (n = 325). Delivered by lay health educators, the intervention program targeted increasing knowledge on breast health and breast screening, reducing emotional or psychological problems, and increasing support from the significant others of study women. Breast screening-related cognition and behavior were measured at pre-intervention and post-intervention.RESULTS: Comparisons of the pre-intervention and post-intervention measurements showed that while the proportion of women who had a clinical breast examination or mammogram in the preceding year was decreased at the post-intervention in the control group, it was increased in the intervention group. However, the differences did not reach a significant level. No consistent patterns could be found in changes of variables in knowledge, attitudes and beliefs. These results remained similar when potential confounding factors were adjusted using mixed model regression analyses.CONCLUSIONS: These results did not suggest significant effects of an intervention program which used lay health educators to promote breast cancer screening in older single African-American women.
