Temperature dependence of early and late currents in human cardiac wild-type and long Q-T DeltaKPQ Na+ channels.

Na+ current (INa) through wild-type human heart Na+ channels (hH1) is important for normal cardiac excitability and conduction, and it participates in the control of repolarization and refractoriness. INa kinetics depend strongly on temperature, but INa for hH1 has been studied previously only at room temperature. We characterized early INa (the peak and initial decay) and late INa of the wild-type hH1 channel and a mutant channel (DeltaKPQ) associated with congenital long Q-T syndrome. Channels were stably transfected in HEK-293 cells and studied at 23 and 33 degreesC using whole cell patch clamp. Activation and inactivation kinetics for early INa were twofold faster at higher temperature for both channels and shifted activation and steady-state inactivation in the positive direction, especially for DeltaKPQ. For early INa (<24 ms), DeltaKPQ decayed faster than the wild type for voltages negative to -20 mV but slower for more positive voltages, suggesting a reduced voltage dependence of fast inactivation. Late INa at 240 ms was significantly greater for DeltaKPQ than for the wild type at both temperatures. The majority of late INa for DeltaKPQ was not persistent; rather, it decayed slowly, and this late component exhibited slower recovery from inactivation compared with peak INa. Additional kinetic changes for early and peak INa for DeltaKPQ compared with the wild type at both temperatures were 1) reduced voltage dependence of steady-state inactivation with no difference in midpoint, 2) positive shift for activation kinetics, and 3) more rapid recovery from inactivation. This study represents the first description of human Na+ channel kinetics near physiological temperature and also demonstrates complex gating changes in the DeltaKPQ that are present at 33 degreesC and that may underlie the electrophysiological and clinical phenotype of congenital long Q-T Na+ channel syndromes.

Complementation of GC-AED and ELISA for the determination of diazinon and chlorpyrifos in fruits and vegetables.

Enzyme-linked immunosorbent assay (ELISA) was employed to screen produce samples for diazinon and chlorpyrifos levels. All positive samples were analyzed for these organophosphate insecticides with a newly developed gas chromatographic atomic emission detection (GC-AED) method. Produce was extracted in methanol by Polytron homogenization and cleaned up and concentrated using Sep-Pak C18 cartridges. Fortification and reproducibility studies were performed using apples, blueberries, green beans, lettuce, and tomatoes that were free of detectable levels diazinon and chlorpyrifos. Percent coefficient of variation (%CV) from the reproducibility studies ranged from 1.9 to 17 for chlorpyrifos and 1.9 to 11 for diazinon. Spiking studies demonstrated percent recoveries of 78 to 114% (%CV 3.1 to 13%) for diazinon and 82 to 123 (%CV 1.5 to 12) for chlorpyrifos. Of the 278 fruit and vegetable samples screened for both organophosphate insecticides, 59 were found to have detectable levels of diazinon and 35 samples were found to contain detectable amounts of chlorpyrifos. A correlation study between ELISA and GC-AED demonstrated that the correlation coefficient was 0.987 for diazinon and 0.967 for chlorpyrifos.

[Application of MEDPOR surgical implant in the craniofacial reconstruction].

Since November 1996, 20 cases of craniofacial deformities, either from congenital or traumatic, were treated with MEDPOR surgical implant made from a linear high density polythylene. The animal experiment had shown that the MEDPOR had good organotrophic characteristics allowing tissue ingrowth. The biocompatibility studies in vitro and in vivo had shown that the MEDPOR biomaterial was free from any observable systemic or cytotoxic effect. In the clinical application, it was found that the MEDPOR could be easily modeled and maintained. Because of the ability to induce tissuee ingrowth, the tenacity and stability of the material were enhanced. A total of seven cases of cranial defects, 8 cases of periobital defects or depressed periobital regions, 2 cases of traumatic auricular defects, 2 cases of traumatic saddle nose and 1 case of maldevelopment of mandible angle were treated. All of the cases were followed up for 6 months, the results were satisfactory.

[The study of endothelin and endothelin receptors in cyclic human endometrium].

OBJECTIVE: To determine the distribution of endothelin (ET) peptide and endothelin as well as their receptors (ETaR and ETbR) mRNA in human cycling endometrium in order to assess possible role of ET in endometrial reproductive physiology. METHODS: Twenty-three endometrial specimens from gynecological surgery for benign conditions were studied by means of immunohistochemistry, in situ hybridization and computer quantitative image analysis. RESULTS: The expression of ET peptide and ET mRNA in human endometrium varies throughout the menstrual cycle, increasing gradually from proliferative phase to secretory phase, with maximal levels in the pre-menstrual phase. Stromal expression is lower than in glandular epithelium and remains relatively constant in the menstrual cycle. ETaR and ETbR mRNA are detected in normal human endometrium, higher in glandular epithelial cells than in stromal cells. ETaR mRNA expression has no obvious cyclic changes, but ETbR mRNA expressions are higher in secretory endometrium than in proliferative endometrium. The ratio of ETaR/ETbR has also cyclic changes. CONCLUSION: Changes of Expression of ET and ET receptors in the endometrium throughout the menstrual cycle suggest that ET might be important in endometrial regeneration and repair following menstruation, and probably plays a role as the paracrine control of the uterine vascular bed.

[The changes of sex steroid receptors and morphology in Norplant treated endometrium].

OBJECTIVE: To study the relationship between the changes of sex steroid receptors as well as endometrial morphology and abnormal uterine bleeding induced by Norplant. METHODS: Immunohistochemistry, Western Blot, in situ hybridization and computer image analysis were employed to demonstrate the expression of estrone receptor(ER) progesterone receptor(PR) protein and mRNA as well as morphometric analysis of the endometrium in Norplant users. RESULTS: The content of DNA and the numbers of spiral arterioles decreased in Norplant treated endometrium. The percentage of glandular area in Norplant endometrium was much lower than that in cyclic endometrium. A statistically significant correlation was observed between the percentage of area of glands and abnormal uterine bleeding, as well as serum estrogen(E2) level, but E2 level had no direct relationship with the bleeding abnormality. As the time growing, the percentage of glandular area increased. Immunostaining and relative concentration of glandular and stromal ER in Norplant treated endometrium were significantly weaker and less than all the stage of cyclic endometrium. However, PR, especially stromal PR immunostaining was very intense, which was the same or more than that in all the stage of cyclic endometrium. In situ hybridization detected that the ER and PR mRNA was lower in Norplant treated endometrium than that in cyclic endometrium. The ER mRNA expression was more less abundant than PR mRNA expression. CONCLUSION: These results suggested that poor repair and regeneration of endometrium(especially glands and spiral arterioles) and abnormal expression of ER and PR may be related to pathogenesis of abnormal uterine bleeding induced by Norplant implant.

[Immunoradiometric assay of tumor marker CA15-3 and its clinical application].

OBJECTIVE: To develop a new tumor marker immunoradiometric assay and to evaluate its preliminary clinical application. METHODS: With monoclonal antibodies Ma552 and Ma695 from Sweden, a sandwich immunoradiometric assay was developed with Ma552 as catching antibody and Ma695 as tracer. The Ma552 was coated on polystyrene beads, and Ma695 was labeled with 125-I. The reaction was one step at room temperature. RESULTS: Bmax/B0 of the standard curve was 82. The sensitivity of this assay was 0.3 u/ml, CV within assay and CV between assays were 8% and 10%, respectively. Serum level of CA 15-3 in 50 normal women was 11.3 +/- 3.9 u/ml with a false positive rate of 0% based on the cut-off value of 30 U/ml. In 40 patients with benign breast diseases the serum level was 9.6 +/- 5.8 u/ml with a false positive rate of 0%. In 65 cases of breast cancers in different stages it was 88.4 +/- 159.6 u/ml before treatments with a total positive detection rate of 50.8%. Metastases, especially, bone metastases caused significant serum CA15-3 elevation, with 100% positive rate(n = 9). The positive rate of recurrences of beast cancers was 80% (n = 5). CONCLUSION: The newly established immunoradiometric assay of CA15-3 is highly useful in the diagnosis, differential diagnosis, and monitoring of metastases and recurrences of breast cancer, and is superior to CEA.

[National epidemiological study on obesity of children aged 0-7 years in China 1996].

OBJECTIVE: To study on obesity of children aged 0-7 years in 11 cities in 1996. METHODS: The sampling size was 208, 513. The children were divided into 9 groups: birth-, 1 mo-, 6 mo-, 1 yr, 2 yr-, 3 yr, 4 yr-, 5 yr-, 6-7 yr. RESULTS: The prevalence of overweight was 4.2% (M4.2%, F4.2%); and of obesity, 2.0%(M2.2% F1.9%). The obesity to overweight ratio (ORR) was 2.1%(M1.9%, F2.2%). The adiposity rebound age (ARA) was 5 years. The adiposity rebound ratio (ARR) was 3.2 for ARR1 (M2.9%, F.5%) and 2.0 for ARR2 (M1.7%, F2.4%) in obese children and 3.5 for ARR1 (M 3.5, F3.5) and 2.5 for ARR2 (M2.8, F2.8) in overweight children respectively. The secular trend of prevalence of childhood obesity during 1986-1996 showed that the prevalence of obesity increased annually at 9.1% (M10%, F8.7%). CONCLUSION: The obesity has already been a severe health and social problem of preschool children in China. The increment of prevalence of obesity during 1986-1996 has been out of control. The earlier of ARA and the larger of OOR is an important risk factor of increasing of obesity. 1 year and 5 years after birth, predicted by ARR, are two critical periods of obesity control in preschool children in China.

[Thymectomy in the treatment of myasthenia gravis].

OBJECTIVES: To review the clinical course of patients with myasthenia gravis following thymectomy and to identify prognostic factors that may influence outcome. METHODS: We follow-up 91 patients with myasthenia gravis surgically treated with thymectomy through a median stereotomy from May, 1983 to October, 1993. The overall 5-year remission and improvement probabilities were obtained by Kaplan-Meier Method. RESULTS: 36 (39.6%) patients showed remission, 27 (29.7%) improvement, 14 (15.4%) no change and 14 (15.4%) worsening. The overall 5-year remission and improvement probabilities of patients were 64% and 88% respectively. CONCLUSIONS: Most patient's symptoms after thymectomy were progressively improved with time, which is probably attributable to thymectomy, and the natural history of myasthenia gravis. Age at surgery, sex, duration of clinical course and pathological findings may be important prognostic factors for patients with myasthenia gravis after thymectomy.

Epidermal growth factor-induced growth inhibition requires Stat1 activation.

Epidermal growth factor (EGF) is a mitogen for most epithelial cells. Paradoxically, the growth of some cultured cell lines, containing high numbers of EGF receptors, are inhibited by EGF. Here we demonstrate that growth inhibition by EGF in several cell lines correlates with the activation of the signal transducer and activator of transcription (Stat) 1. In contrast, in normal fibroblasts and several cell lines that are growth stimulated by EGF, we observed no or very transient activation of Stat1. A causal association between Stat1 activation by EGF and growth inhibition was suggested by the expression of a dominant-negative Stat1 in A431 cells, resulting in the loss of Stat1 DNA binding and concomitant resistance to growth inhibition by EGF. We conclude that, in the cells examined, EGF-induced arrest of growth requires activated Stat1.

Cultured blood dendritic cells retain HIV-1 antigen-presenting capacity for memory CTL during progressive HIV-1 infection.

Dendritic cells (DC) are potent APC that may be involved in the pathogenesis of HIV-1 infection. We studied the APC function of DC from HIV-1-infected subjects that were derived from monocyte-depleted PBMC by culture in human IL-4 and human granulocyte-macrophage CSF. The cultured cells from the HIV-1-infected subjects had similar morphology and phenotype of mature DC (CD80 = 41 +/- 8%, CD86 = 77 +/- 5%, CD40 = 87 +/- 6%, CD1a = 1 +/- 1%) to DC cultured from seronegative subjects. The yield of these DC was lower than from HIV-1-seronegative subjects (4 +/- 0% vs 11 +/- 2%, p < 0.01), and the lower DC yields correlated with lower numbers of blood CD4+ T cells (r = 0.60, p < 0.01) and higher plasma viral load (r = -0.49, p < 0.01). DC from HIV-1-infected subjects were infected with recombinant vaccinia virus vectors expressing Gag, Pol, and Env and were able to stimulate equal or higher levels of MHC class I-restricted, anti-HIV-1 memory CTL (CTLm) than were similarly treated, autologous B lymphocyte cell lines. DC pulsed with peptides representing HIV-1 CTL epitopes stimulated higher levels of anti-HIV-1 CTLm responses than did DC infected with the vaccinia virus-HIV-1 constructs. Allogeneic, MHC class I-matched DC also stimulated anti-HIV-1 CTLm activity in cells from HIV-1-infected subjects. DC from early and late stages of HIV-1 infection had a similar ability to activate CTLm specific for targets expressing either HIV-1 genes via vaccinia virus vectors or HIV-1 immunodominant synthetic peptides. However, DC from either early or late stages of HIV-1 infection could not overcome the defect in anti-HIV-1 CTLm response in advanced infection.

Persistent elevation of corticotrophin releasing factor and vasopressin but not oxytocin mRNA in the rat after kindled seizures.

Intractable temporal lobe epilepsy is a disabling disorder with far reaching effects on brain function, behavior and neuroendocrine function. Previous work in the kindled-seizure model for temporal lobe epilepsy has shown that these seizures cause vasopressin (VP) release, an increase in resting VP and lasting increases in VP mRNA in the supraoptic nucleus (SON) of the hypothalamus. In this study we used in situ hybridization to examine the effects of kindled seizures on the expression of two other functionally-related, neuroendocrine genes, oxytocin (OT) and corticotrophin releasing factor (CRF). Comparisons in kindled and sham-stimulated controls revealed an increase in VP mRNA but not OT mRNA in magnocellular neurons and an increase in CRF mRNA in parvocellular neurons of the paraventricular nucleus (PVN) of the hypothalamus 1 month after the last seizure. We conclude that kindled seizures induce selective changes in neuroendocrine gene expression in neuroendocrine systems, VP and CRF but not OT.

Anionic phospholipids activate ATP-sensitive potassium channels.

The ATP-sensitive potassium channel (KATP) controls insulin release in pancreatic beta-cells and also modulates important functions in other cell types. In this study we report that anionic phospholipids activated KATP in pancreatic beta-cells, cardiac myocytes, skeletal muscle cells, and a cloned KATP composed of two subunits (SUR/Kir6. 2) stably expressed in a mammalian cell line. The effectiveness was proportional to the number of negative charges on the head group of the anionic phospholipid. Screening negative charges with polyvalent cations antagonized the effect. Enzymatic treatment with phospholipases that reduced charge on the lipids also reduced or eliminated the effect. These results suggest that intact phospholipids with negative charges are the critical requirement for activation of KATP, in distinction from the usual cell signaling pathway through phospholipids that requires cleavage. Mutations of two positively charged amino acid residues at the C terminus of Kir6. 2 accelerated loss of channel activity and reduced the activating effects of phospholipids, suggesting involvement of this region in the activation. Metabolism of anionic phospholipids in plasmalemmal membrane may be a novel and general mechanism for regulation of KATP and perhaps other ion channels in the family of inward rectifiers.

Nucleotide and predicted peptide sequence of feline interleukin-12 (IL-12).

Feline Interleukin-12 (IL-12) is a heterodimeric glycoprotein consisting of two disulfide linked subunits of about 40 kD (p40) and 35 kD (p35). It is a pleiotropic cytokine mediating biological activities on T- and NK-cells. One important function is the induction of a Th1 immune response. Here we report the cloning and sequencing of feline IL-12, the expression of the p40-protein in E. coli and production of monoclonal antibodies. At the nucleotide level, feline IL-12 shows between 87-90%, on the amino acid level between 82-87% identity to the bovine and human IL-12, respectively.

[Treatment of esophageal and cardial stricture with TiNi memory alloys srent].

A thermal-shaped memory metal stent, which is made of Nitinal, used in 95 patients with malignant tumors and 35 patients with anastomosis stenosis tumor resection. All patients had no opportunities of surgical operation and no response to dilation radiotherapy, chemotherapy, lasertherapy and traditional Chinese medicine. Dysphagia ameliorated in all patients after stent therapy. The mean dysphagia grade varied from 3.71 +/- 0.45(range 3-4) to 1.08 +/- 0.59 (range 0-2)(P < 0.01). Complications occurred in 49 patients (31.5%), including chest pain in 36, local bleeding in 5, tumor overgrowth in 4, stent migration in 2, and food obstraction in 2. In summary, treatment with placement of a Nitinol stent is effective, safe and simple in our patients with dysphagia which were due to malignant esophageal and cardiac strictures and is feasible for some benign esophageal and cardiac strictures as well through our clinical practice and observation nearly three years.

[Hepatitis B virus DNA detection by means of polymerase chain reaction in patients with chronic hepatitis and hepatocellular carcinoma].

HBV immunological detection and HBV DNA detection by polymerse chain reaction (PCR) or molecular hybridization with 32P labeled probe were done in 61 patients with chronic hepatitis (CH) and 47 patients with hepatocellular carcinoma (HCC) to investigate the relationship between HBV replication and serum markers. The HBV DNA was detected in 90.50% of CH patients and 50.00% of HCC patients with HBsAg, HBeAg and anti-HBc positive; in 45.40% of CH patients and 7.14% of HCC patients with HBsAg, anti-HBe and anti-HBc positive; in 60.00% of CH patients and 40.00% of HCC patients with HBsAg and negative HBeAg, anti-HBe positive; in 20.00% of CH patients and 22.22% of HCC patients with HBsAg negative and anti-HBe or anti-HBe or anti-HBs positive; in 0 of CH or HCC patients lack of HBV serum markers. Our data suggested that the most active HBV replication was correlated to the presence of both HBsAg and HBeAg, there were some extent of HBV replication in CH or HCC patients with positive HBsAg, the inhibition of HBV replication was related to anti-HBe, the less active HBV replication was seen in HCC patients when compared with CH patients.

Secondary prevention of cardiac events following myocardial infarction: effects of atenolol and enalapril. Beijing Collaborative Study Group.

OBJECTIVE: To investigate the actions of beta-blocker (atenolol) and ACE inhibitor (enalapril) for the secondary prevention of the main cardiac complications after acute myocardial infarction (AMI). METHODS: 1106 cases of AMI from 7 hospitals in the Beijing area were collected and were divided randomly into three groups: control (group C), atenolol (group A), and enalapril (group E). Drugs for investigation were administered 2-4 weeks after the onset of AMI, and the subjects were followed up for a median period of 19 months. All patients were given aspirin 50 mg/day. The end points of observation were cardiac events and non-cardiac events. Cardiac events included sudden cardiac death (SCD), heart failure death, total cardiac deaths, and myocardial re-infarction. RESULTS: The clinical conditions of the three groups were compatible. Sixty-six cardiac events (6.0%) occurred. Comparing with group C, the rate of SCD decreased significantly by 68% in group A after atenolol treatment for 28 months. Both atenolol and enalapril significantly increased left ventricular ejection fraction (LVEF), whereas in group C the LVEF did not change during the follow-up period. There was obvious decreasing tendency of the survival curve in group C, compared with the other two groups. Totally drugs decrease one cardiac death 0/00/month. But the rate of myocardial re-infarction was the same in the three groups. No serious side effects on blood pressure or heart rate were observed. CONCLUSIONS: Both atenolol and enalapril (domestic products) are beneficial to the secondary prevention of SCD and heart failure death after AMI, but not to re-infarction. Both drugs should be continued for a prolonged period to be effective. Drugs given 2-4 weeks after acute stage are also effective, with no serious side effects.
