[Statistical correlation between lactate concentration and acid-base equilibrium in cases of brain tumors, craniocerebral injuries and meningitis].

Statistical significance of correlations between lactate concentration and pH value as well as pCO2 in the cerebrospinal fluid was assessed in patients with brain tumours, craniocerebral injury and meningitis. After calculation of correlation indices it was found that with increasing lactate level the correlation between this level and the pH and pCO2 of the cerebrospinal fluid increases. Correlation coefficients were far from their maximal values (+1 or -1) which indicates absence of a direct cause-and-effect relationship between lactate concentration and both remaining values. The analysis of results in the group with lactate level exceeding 30 mg/100 ml demonstrated lowest correlation coefficients between these values in patients with meningoencephalitis.

[Occipital nerve neuralgia as postoperative complication. Views on etiology and treatment].

Occipital neuralgia might have a various etiology but the most common cause is hypertrophic fibrosis of subcutaneous tissue following trauma to the occipital region, surrounding the occipital nerve. The other important cause of neuralgia is spondylosis of the upper part of the cervical spine (C1-C2). Rare causes are-diabetes, gout and neoplasm. In the presented material we point out the possibility of the occipital neuralgia after surgery in the treatment of the tumours of ponto-cerebellar region and trigeminal neuralgia. We observed the symptoms in two groups of patients and used pharmacological treatment, local blockade and cutting (neuronectomy) of the occipital nerves trunk when the conservative treatment was unsuccessful. We also present the present concepts of occipital neuralgia treatment. In case of severe symptoms, unsuccessful conservative therapy and poor results of the neuronectomy the most indicated therapy is selective posterior rhizotomy.

[Osmotic serum pressure in patients treated with furosemide and hypertonic mannitol solution].

In patients with expanding intracranial lesion syndrome osmotic pressure of the serum was measured during administration of furosemide or 20% mannitol solution. Mannitol was administered by two methods: a) a dose of 1.5 g/kg during 3--4 hours, b) a dose of 1 g/kg for about 20 min. When mannitol was administered slowly the osmotic pressure of the serum at the end of infusion was 11--15% above the initial one and returned to the initial level 30--60 min. after the end of infusion. With rapid infusion the peak rise of pressure was observed after 30 min. usually after the end of infusion, and after one hour the pressure returned to values approaching the initial ones. Furosemide in a dose of 40 mg intramuscularly or intravenously caused slight changes of osmotic pressure reaching 13 mOsm/l.
