[Treatment of acute destructive pancreatitis].

The authors analyse observations over 113 patients (1978-1988) with destructive pancreatitis; there were 54 males and 59 females, whose ages ranged from 18 to 85 years. Complex nonoperative treatment was effective only in 17 patients. Cholecystectomy, cholecystostomy or choledochotomy was performed in 7 patients after their condition had improved. Eighty-eight patients underwent operation in the acute period. Total lethality was 71.6%. In summarizing the results the authors came to the conclusion that operative interventions should be postponed in clinical practice. The optimal time for the operation is 6-9 days after the onset of the disease.

Experience with the treatment of 137 patients with spontaneous internal biliary fistulas of different etiology is presented. Questions of clinical course, preoperative diagnoses and operative technique are discussed.

[Cholelithic intestinal obstruction].

Analysis of the treatment of 25 patients with cholelithic intestinal obstruction is given. Questions of etiology, variants of the development, clinical course, diagnosis, operative treatment of the cholelithic ileus, causes of lethal outcomes are discussed. The authors propose measures of prophylactics .

[Analysis of the lethality in cholecystitis and the ways for its decrease].

An analysis of lethality in cholecystitis has been carried out. The main complications, which had resulted in the death of 238 patients, were analysed. Purulent complications served as the most common cause of fatal outcome. The authors came to the conclusion on the necessity to broaden indication for an early surgical intervention in acute cholecystitis in elderly patients.
