Role of volume-conductor properties of muscles in the formation of evoked contractions during stimulating electrical impedance myography.

Experiments on the preparation of frog sartorius muscle showed that electrical impedance myogram of evoked muscle contractions is the sum of electric processes accompanying contraction of some muscle fibers or muscle regions. Inverted muscle response probably results from heterotopic excitation of distant muscle regions under conditions of reduced excitability of muscle fibers adjacent to the electrode. During centrifugal propagation of contractions, especially in altered muscles, the negative wave on the myogram corresponding to impedance decrease is sometimes followed by a positive wave. The possibility for recording of heterotopic contractions indicates that the impedance of the electrode-muscle interface produces no considerable effects on the shape of electrical impedance myogram. These results hold much promise for the diagnostics of neuromuscular diseases.

Effect of early fasciotomy on intramuscular pressure and electrical excitability of muscles in experimental compartment syndrome.

Experiments on rabbits with compartment syndrome of the leg showed that the main pathogenetic factors of muscle injury are postischemic contracture leading to extravascular microcirculatory disorders and tissue necrosis(early postcompression period) and progressive subfascial edema (18 h-4 days). Therapeutic effect of fasciotomy in the early period consists in restoration of bloodflow due to relaxation of contracted muscles and in the late period in decompression of edematous muscles and prevention of secondary ischemia. Fasciotomy through a small section does not lead to sufficient relaxation of rigid muscles and does not essentially increase the subfascial volume. Fasciotomy decreases subfascial pressure and muscle excitability threshold, however they remain considerably elevated during day 1. An electrodiagnostic test is proposed for early (before subfascial edema) detection of postischemic contracture.

[Use of local hypothermia in complicated spinal injuries (clinical and experimental studies)].

Experimental data on 116 adult mongrel dogs and clinical findings of 121 patients with complicated spinal injury who had been operated on at Moscow Clinical Hospital No. 67 which is a basis of the Clinic of Traumatology, Orthopedics and Military Surgery, I.M. Sechenov Moscow Medical Academy, underlay the authors' research material. Experimental studies of the impact of local cooling of the intact spinal marrow indicated that there were the least changes in the temperature range of +9 to +11 degrees C with a session lasting 2-3 hours. Experimental histological studies of nerve tissue with hemisection demonstrated that two-hour local hypothermal sessions at 2- or 3-hour intervals proved to be optimal within the first 2 postoperative days. Clinically, the intraoperative regional spinal cooling at the last stage showed a 1.5-2-fold reduction in blood loss and exerted a profound hemostatic effect. Its postoperative local cooling produced analgetic and spasmolytic effects and improved cerebral blood flow. The application of local spinal hypothermia by the procedure developed by the authors is within reach at any medical institution.

[Effectiveness of local hypothermia in the complex treatment of patients with complicated injuries of the spine].

Having analysed her observations of 84 scheduled patients with complicated injuries of the spinal column the author comes to the conclusion that local cooling of the spinal cord during the postoperative period exerts positive influence: it contributes to a decrease in excitability of alpha motoneurons of the spinal cord which is manifest by reduced muscular tension, improves the local blood flow in the spinal cord, raises the vascular tension and provides the analgetic effect.
